2.Generalized Epilepsy with Febrile Seizures Plus(GEFS+) and a Mutation in the Voltage-gated Sodium(Na+)-Channel Beta 1 Subunit Gene(SCN1B).

PURPOSE: It was reported that gene locus for generalized epilepsy with febrile seizures plus(GEFS+) exist in chromosome 19q13.1, and has relationship with a 387 C G mutation in the SCN1B gene. This study is to determine whether there is the 387 C G mutation in the children with GEFS+ and simple febrile seizures(FS). METHODS: The sample group consisted of 16 patients with GEFS+ and 10 patients with FS who were diagnosed by our department of pediatrics from Jan. 1998 to Dec. 1999. The control group consisted of 15 children who do not have seizure disorders. Genomic DNA was extracted from peripheral blood and a segment of the SCN1B exon 3 was amplified by PCR technique. Purified PCR products were treated with restriction enzyme, Hin P1. The restriction pattern was analyzed by sequencing analysis. RESULTS: Sixty nine%(11 of 16) patients with GEFS+ had family history for epilepsy, and epilepsy phonotypes were generalized tonic-clonic seizures in 82%(13 of 16), on the other hand 12%(2 of 16) and 6%(1 of 16) had absences and atonic seizures respectively. EEG findings showed generalized spike and wave in the all patients with GEFS+. in this study, however we could not observe a 387 C-->G mitation of the SCN1B in the children with GEFS+ and febrile seizures. CONCLUSION: The gene for GEFS+ may have a heterogenetic characteristics, and there may be racial differences in mutation frequency. Expanded studies involving large number of different families are required.

4.Inflammatory Pseudotumor of the Liver: A case report.

The term "collagenous colitis" was coined by Lindstrom in 1976 to describe the patient with chronic watery diarrhea who had normal rectal mucosa by proctoscopy but who had a thick subepithelial collagenous deposit on biopsy, and now collagenous colitis is recognized as one of the more common causes of chronic diarrhea of obscure origin. But in this country only a few case has been reported. We have seen a 68-year-old man with chronic watery diarrhea with abdominal pain. Physical examination, laboratory and radiologic studies were no abnormal finding. Colonoscopy disclosed grossly normal mucosa through the entire colon but by histologic examination there are chronic inflammation in the lamina propria and thickened subepithelial collagen layer. Symptoms and pathologic findings of patient improved after treatment with sulfasalazine and prednisolone. (Korean J Castraintest Endosc 17: 79-84, 1997)

5.Hybrid Treatment of Coexisting Renal Artery Aneurysm and Abdominal Aortic Aneurysm in a Gallbladder Cancer Patient.

Renal artery aneurysm (RAA) is uncommon, and the coexistence of an abdominal aortic aneurysm (AAA) is an extremely rare condition with potentially high life-threatening mortality in case of rupture. Aneurysms can be treated by endovascular intervention or open surgery. Although most of aneurysms are treated by endovascular intervention, open surgery is often necessary for RAAs associated with the proximal renal bifurcation or the branches in the distal renal arteries. We report a rare case of coexisting RAA with AAA treated by hybrid method, consisting of endovascular aneurysm repair for AAA and open surgery for RAA located adjacent to the distal branches of the renal artery.

6.Effects of Some Drugs and Toxins on Positive Rate of Y-Body in Leukocytes of Rabbits.

This study was carried out to investigate the response of interphase Y chromosome to some drugs and toxins by observing the rate of leukocytes showing Y-body in the peripheral blood. The interphase Y chromosomes of blood leukocytes were stained with quinacrine mustard and the animals used were rabbits, rats and guinea pies. Y-bodies of leukocytes were studied in the animals as well as in man. Changes in positive rate of Y-body in leukocytes and total leukocyte count were observed in the rabbits administered with drug or toxin. The results concerning the rate of Y-body were as follows: 1. Y-bodies were present in the blood leukocytes of the animals. However positive rates in mononuclear and polymorphonuclear leukocytes were lower in animals that in man. The positive rate of Y-body was higher in mononuclear leukocytes than in polymorphonuclear leukocytes in the animals as in man. 2. Bacterial toxins such as typhoid, D.P.T. and cholera vaccines and anticancer drugs such as busulfan and endoxan reduced the positive rates of Y-body both in mononuclear leukocytes and in polymorphonuclear leukocytes. 3. Benzene known as bone marrow toxin reduced the positive rate of Y-body in mononuclear leukocytes, but not that in polymorphonuclear leukocytes. Quinine known as general protoplasmic poison reduced the positive rate of Y-body not only in mononuclear leukocytes but also in polymorphonuclear leukocytes. 4. Antibiotics such as tetracycline and chloramphenicol and steroid hormones such as estrogen, testosterone and prednisolone had no effects on the positive rate of Y-body both in mononuclear leukocytes and in polymorphonuclear leukocytes.

7.Values of Urine Cytology in the Diagnosis of Urothelial Carcinomas.

In order to determine its accuracy the authors conducted cytologic examination on patients found to have urothelial carcinoma by cystoscopy, histopathologic and radiological examinations at the Department of Urology, Catholic Medical College, from January 1971 to August 1981. The results were as follows: 1. Age distribution of the urothelial carcinomas is from 33 to 80 years age and the most frequent age group is 40 to 59 years age. The male: female ratio is 4.3:1. 2. Of the 32 patients, 4 cases (12.4%) were negative, 14 (43.8%) suspicious and 14 (43.8%) positive reports. 3. No significant correlations were found between the urine cytology and histologic grade, but the higher cytologic class had tendency to have the higher histologic grade. 4. In the aspect of the tumor size and cytology, large sized tumor had showed positive findings more frequently than that of, smaller one, but it showed high accuracy rate (77.8%) when small sized tumors were multiple.

9.No Effect of IV Cyclophosphamide in Children with Steroid Resistant Nephrotic Sydrome.

Sometimes a pilomatrixoma on upper neck can be misdiagnosed as primary salivary gland tumor, skin adnexal tumor or metastatic carcinoma. On fine needle aspiration cytology(FNAC), characteristic features are ghost cells, basaloid cells, and calcium deposits, among which presence of ghost cells seems to be the key to recognize it. Here we present a cytologically misdiagnosed case of pilomatrixoma. A 32-year-old man presented a subcutaneous mass on the right posterior neck. It has grown slowly for 1 year, and was nontender, well circumscribed, hard, and movable mass. An initial FNAC yielded only monomorphic population of highly mitotic basaloid cells, without anucleated ghost cells, chronic inflammatory cells or foreign-body giant cells, suggestive of a poorly differentiated carcinoma. However, that was inconsistent with patient's clinical findings. For further correct diagnosis, FNAC was repeated, which yielded the basaloid cells and foreign-body giant cells. The diagnosis of pilomatrixoma could be made and the mass was excised. This case demonstrates that the pilomatrixoma should be considered in the differential diagnosis of subcutaneous aspirates containing not ghost cells but a dominant population of basaloid cells.

10.The Effects Of Thermocycling On The Bond Strength Between Cobalt-Chromium Alloy And Denture Base Resin.

The purpose of this study was to evaluate the effects of thermocycling on the shear bond strength of Co-Cr alloy to denture base resin. PMMA denture base resin such as heat-cured Vertex-RS , self-cured Vertex-SC and 4-META denture base resin such as heat-cured Meta-Dent , self-cured Meta-Fast was bonded to Co-Cr alloy. Samples were divided into 3 groups : no thermocycling group as control, thermocycling between 5degrees C and 55degrees C with 15 second dwell time as group 1, thermocycling with 1 minute dwell time as group2. The shear bond strength was measured and the interface between metal and resin was observed by SEM. The results were as follows. 1. The shear bond strength decreased significantly according to thermocycling and dwell time(P<0.001). 2. The bond strength of Co-Cr alloy and 4-META denture base resin was significantly higher than that of Co-Cr alloy and PMMA denture base resin(P<0.001). 3. IN SEM, there was no gap in control group, but there was much and large gap in group 1, 2. The longer dwell times, the lower bond strength. PMMA denture base resin had more gap than 4-META denture dwell times, the lower bond strength. PMMA denture base resin had more gap than 4-META denture base resin in the interface. These results revealed that thermocycling decreased the bond strength between Co-Cr alloy and denture base resin and dwell time of thermocycling changed the effect of thermocycling. The results suggested that oral temperature change affect the bond strength of prosthesis.
