2.A Case of Extramammary Paget's Disease on the Scalp.

We treated a case of extramammary Paget's disease that affected the scalp of a 45-year-old female. It is extremely rare that the disease arises in areas other than the anogenital region and the axillae. The lesion was a round erythematous oozing crusted hairless patch. Histopathologically, many pagetoid cells were found within the epidermis and dermis. The cytoplasms of these cells stained with alcian blue at pH 2.5, CEA, EMA and low-molecular-weight-cytokeratin. The patient underwent a wide local excision. We used a mapping technique to reveal the distribution of microscopically involved lesions.

3.Subjective Symptoms and Flicker Test Values in Relation to Chronic Low Dose Organic Solvent Exposure.

This study was conducted to examine the effect of chronic low dose organic solvent exposures in the industries towards then neurobehavioral functions of workers subjective symptoms on neurobehavioral function as well as a visual reaction time test (Flicker test) were administered to 94 exposed and 162 unexposed workers in a oil refinery and some other auto-repair shops. The results obtained were as follows: 1. Symptom complaints were higher and Flicker test values were lower in exposed workers than in unexposed workers. 2. Flicker values were inversely correlated with urinary Hippuric acid concentration in exposed workers (r=-0.26, p<0.05). 3. Flicker values were inversely correlated with subjective symptom score (r=-0.15, p<0.05). Low Flicker value were also related with some subjective symptoms such as "Dimmed vision", "Nightmare", "weakness on extremity" in workers as a whole. While symptoms of "Dimmed vision", "Nightmare" only observed among exposed workers.

6.Evidence of Microstructural Abnormality on Descending Motor Pathway in Cerebral Plasied Children with Periventricular Leukomalacia: Diffusion Tensor MRI Study.

OBJECTIVE: Using diffusion tensor MRI (DTI), to investigate the microstructural abnormality of corticospinal tract in the cerebral palsied children with periventricular leukomalacia on conventional MRI and to recognize the clinical usefulness of DTI. METHOD: Seven patients were studied. DTI was peformed using 1.5T MR scanner (Vision Plus, Siemens, Erlangen, Germany) and fractional anisotropies of corona radiata, posterior limbs of internal capsules, and cerebral peduncles of midbrain in both hemisphere were calculated. RESULTS: The fractional anisotropy was significantly decreased in all corona radiata, posterior limbs of internal capsules, cerebral peduncles of midbrain in six patients except one hemiplegic patient, compared with that of control group. In in the hemiplegic patient, the fractional anisotropy was reduced only in affected hemisphere. In addition, the fractional anisotropy tended to be also increased as gross motor function measure (GMFM) score was increased. CONCLUSION: We believe that DTI may be efficient in evaluating microstructural abnormality on the motor pathway of brain and helpful in providing prognosis of clinical findings in cerebral palsied children with periventricular leukomalacia.

7.A Case of Lymphocytic Hypophysitis Presented with Hypoglycemia after Delivery.

Lymphocytic hypophysitis, an inflammatory disease of the pituitary gland that is often associated with pregnancy, is caused by autoimmune destruction of the pituitary gland evidenced by diffuse inflammatory cell infiltration of the pituitary gland and some kinds of detectable autoantibodies. We report a case of lymphocytic hypophysitis in a 31-year-old woman presenting with severe hypoglycemia and hyponatremia after delivery. Hormonal study revealed panhypopituitarism and magnetic resonance imaging with enhancement showed the bulging contour of the right side pituitary gland with an ill-defined mass-like lesion and nodular thickening of the stalk. The patient's symptoms and biochemical data improved greatly with replacement of L-thyroxine and glucocorticoid. Partial recovery of panhypopituitarism was also seen. The follow-up tests revealed dramatic resolution of the pituitary lesion.

OBJECTIVE: To evaluate the therapeutic effect of a Tibia Counter Rotator (TCR) with toe-out gait plate (GP) upon tibial internal torsion by a comparative analysis of transmalleolar angle (TMA) and gait analysis with GP alone. METHODS: Twenty participants with tibial internal torsion were recruited for this study. Each 10 participants were included in group A with TCR and GP application and in group B with GP application only. The TMA and the kinematic results were used for the evaluation of the therapeutic effects of orthoses. RESULTS: Within each group, TMA showed a significant increase after treatment. Group A showed a continuous improvement up to six months, however, group B showed an improvement up to five months only. Group A showed a significantly higher correction effect than group B after treatment. Regarding kinematic data, both groups showed a significantly decreased mean ankle adduction angle after treatment. However, group A showed a significantly lower mean ankle adduction angle than group B after six months. CONCLUSION: The group with TCR and GP showed a significantly better outcome and continued correction force compared to the group with GP only. Our results suggest that TCR with GP may be useful therapeutic orthoses for children with tibial internal torsion.
