8.Surgical removal of a telemetry system in a cynomolgus monkey (Macaca fascicularis):a 12-month observation study

Background:
Telemetry is a wireless implanted device that measures biological signals in conscious animals and usually requires surgery for its removal when the study is finished. After removing the device, the animals are either used for other studies or euthanatized.Case presentation: Herein, we report the case of a living cynomolgus monkey (Macaca fascicularis) that was used for the entire experimental period, instead of euthanasia, after surgical removal of an implanted telemetry system.Radiography was used to determine the status of the implanted telemetry, following which, a repair surgery was performed for removing the system; clinical signs were used to preserve the life of the cynomolgus monkey. Postoperative clinical signs, food consumption, hematology, and serum biochemistry were examined during the 12-month observational period. No abnormal readings or conditions were observed in the subject after implant removal.
Conclusions
This study may be a useful case report for living cynomolgus monkeys in telemetry implantations used throughout the study period. We suggest minimizing the suffering and improving the welfare of these animals.
9.DICAM-mediated Inhibition of Type 1 Interferon System during Macrophage Differentiation of THP-1 Cells.

OBJECTIVE: We have previously shown that DICAM inhibits LPS-mediated macrophage differentiation. However, less is known about the exact action mechanisms of DICAM on the macrophage function and differentiation. METHODS: To induce differentiation into a resting M0 macrophage, THP-1 cells were cultured with 100 nM PMA for 24 h, and then rested for 3 days. THP-1 cells were infected with 50 moi of control LacZ- or DICAM-containing adenovirus. The RNA expression profile associated with DICAM during THP-1 differentiation was analyzed with a microarray chip and in silico analysis with Ingenuity Pathway Analysis (IPA) program. RESULTS: A disease and function analysis of the microarray data in DICAM-overexpressed THP-1 cells revealed a suppression in the expression of multiple genes involved in the response of myeloid cells and phagocytes, and an increase of genes associated with apoptosis of fibroblast cell-line, and viral infection and replication. The canonical pathway analysis also showed the most prominent changes of signaling pathways that involve inflammation responses. An upstream regulator analysis identifyingmolecules upstream of the genes that potentially explain the observed expression changes revealed that IRF7 and the genes in type 1 interferon system, such as IFNA2 and IFNAR,was significantly attenuated by DICAM. A mechanistic network analysis confirmed a direct causal association between IRF7 and type 1 interferon system. A real-time RT-PCR analysis validating the microarray data verified the significant suppression of IRFs, IFNA2, and IFNB1. CONCLUSION: These results suggest that DICAM can be a critical regulator of type 1 interferon system, which is an essential mediator in the process of intracellular infection and systemic lupus erythematosus.

10.Analysis of Emergency Department Patients Transferred from Geriatric Hospitals: Case Series Descriptive Study.

PURPOSE: Nursing homes continue to increase in number, and more residents are being transferred to emergency departments (EDs). The objective of this study was to investigate the overall characteristics of residents who were transferred to the ED and follow-up their course of treatments. METHODS: We identified nursing home residents aged 65 years and older who had been transferred to the ED of a regional tertiary university medical center from January 2008 to December 2009. Further attention was paid to those who had been diagnosed with sepsis. Meantime, a separate cohort of sepsis patients was collected, made up of geriatric patients aged 65 years or older who had visited the ED voluntarily from home or through medical institutions other than nursing homes during the same period. We then compared the two cohorts of sepsis patients. RESULTS: A total of 321 patients aged 65 years or older were transferred from nursing homes during the study period. Their mean age was 77.6+/-7.2 years, and 172 (54.8%) were females. Seventy six (24.2%) patients whose caregivers did not comply with the necessary procedures or refused aggressive management were discharged home or were retransferred to nursing homes. A total of 41 sepsis patients had been transferred from nursing homes, whereas 64 patients had visited the ER voluntarily or through medical institutions other than nursing homes. Even though the difference between the two groups was not significant statistically, patients transferred from nursing homes tended to be discharged or retransferred to nursing homes with inadequate treatment. CONCLUSION: A considerable number of patients transferred from nursing homes were found to be discharged or retransferred to nursing homes during acute care due to refusal of aggressive treatments. Likewise, the comparison of sepsis patients between the two arms showed a greater portion being retransferred with inadequate treatment as well as a greater rate of mortality among patients transferred from nursing homes.
