1.A Survey of Intestinal Protozoan Infections among Gastroenteritis Patients during a 3-Year Period (2004-2006) in Gyeonggi-do (Province), South Korea.

The incidence and etiology of parasite-associated gastroenteritis during 2004-2006 in Gyeonggi-do (province), South Korea was determined by means of antigen detection ELISA on 6,071 stool specimens collected from 6 general hospitals. At least 1 parasitic agent was detected in 3.4% (208/6,071) of the stool samples. Among these, Giardia lamblia was the most numerous (152 cases; 2.5%), followed by Entamoeba histolytica (25 cases; 0.4%), Cryptosporidium parvum (23 cases; 0.4%), and mixed infections (8 cases; 0.1%). Patients aged 1-5 years had the largest proportion (69.2%; 144/208) of parasite-positive stool specimens. Parasite-mediated gastroenteritis was most common from June to September. The detection rate gradually increased from 2004 to 2006. This study shows that parasite-mediated gastroenteritis may be significant among children in Korea and that parasite infection surveillance should be constantly performed.

3.A Pathology-proven Case of Schilder's Disease.

Schilder's disease or myelinoclastic diffuse sclerosis (MDS) is a rare variant of multiple sclerosis. We report a 66-year-old woman with progressive motor weakness and diffuse white matter degeneration on MRI, which satisfies the Poser's restrictive criteria of MDS. As previously reported cases of probable Schilder's disease did not meet the criteria correctly, we consider our patient is the first pathology-proven case of MDS in Korea.

Hydrothcrax can be a cause of respiratory insufficiency and is needed adequate management and therapy. In this report, a patient of respiratory distress due to hydrothorax that occurred during explo-laparotomy and drainage of liver absceas under general anesthesia was treated with water seal drainage insertion in the 7th intercostal space and general management. Literatures was reviewed, and the importance of early recognition and vigorous treatment has been stressed.

The endodermal sinus tumor of ovary is a rare, malignant germ cell tumor. It principally affects children and young adults. Earlier reports before combination chemothery described it was fatal and the prognosis was poor. The advances in treatment of endodermal sinus tumor have been achieved with the development of combination chemotherapy with monitoring tumor marker, serum AFP. The authors experienced a case of endodermal sinus tumor of ovary which showed complete remission following primary conservative surgery and three courses of combination chemotherapy(BEP) with normal full-term spontaneous delivery. With the case report, a brief review was made.

7.Teriparatide therapy without surgical treatment for medication-related osteonecrosis of the jaw: a report of two cases

Medication-related osteonecrosis of the jaw (MRONJ) is a severe complication associated with antiresorptive or anticancer agents.Surgical intervention is generally recommended for advanced stages, but some patients may be ineligible for surgery due to systemic conditions or anatomical limitations. We present two patients who had been treated for osteoporosis with bisphosphonates and were diagnosed with stage 2 MRONJ. Their chief complaints included refractory pain, purulent discharge, and exposed bone following tooth extraction. Despite conservative treatment, the MRONJ worsened, prompting the initiation of teriparatide therapy. After teriparatide administration, the patients experienced symptom improvement, spontaneous sequestrum removal, and significant bone regeneration of vertical osteolysis. Teriparatide, a bone anabolic agent, is typically recommended as adjuvant therapy alongside surgical treatment for MRONJ. This case report illustrates the effectiveness of teriparatide monotherapy in the absence of surgical intervention, particularly for patients who cannot discontinue antiresorptive agents due to low bone mineral density.
