Central retinal artery occlusion occurs rarely as a complication of spine surgery in prone position, but is quite tragic. The suggested cause are hypotensive anesthesia and increased external ocular pressure by headrest, sand bag or others. We experienced a case of left central retinal artery occlusion(CRAO) after surgery of C4-5 fracture-dislocation using a horseshoe headrest and report this case.

3.Surgical Treatment and Clinical Study of Postoperative Maxillary Cyst.

BACKGROUND: Postoperative maxillary cyst(POMC) occurs as a delayed complication following Caldwell-Luc(C-L) operation. Revisional C-L operation has been preferred by most surgeons. Recently, marsupialization with intranasal endoscope has been increased in the management of POMC. OBJECTIVES: The aim of this sutdy was to evaluate the clinical characteristics of POMC and the relationship between location of the cyst on axial PNS CT and surgical approaches. MATERIALS AND METHODS: The 24 patients(26 sides) were reviewed from 1990 to 1996. According to the classification methods of Nakamura, sixteen cases were located in medial, two cases in posterolateral and eight cases in anterolateral of the maxilla. RESULTS: The 22 cases including medial(16 cases), posterolateral(2 cases) and Anterolateral(4 cases) types were treated with marsupialization via intranasal endoscopic approach, 4 of 8 cases which were located in anterolateral type were treated with Caldwell-Luc approach. There was no complication and showed improvement in all patients. CONCLUSION: We think that the classification methods of cyst was useful in deciding surgical approaches. The endoscopic intranasal marsupialization is an effective therapeutic method of POMC especially in medial and posterolateral types.

5.Assessment of Internal Fitness on Resin Crown Fabricated by Digital Light Processing 3D Printer

BACKGROUND: Recently, three-dimensional (3D) printing has been hailed as a disruptive technology in dentistry. Among 3D printers, a digital light processing (DLP) 3D printer has certain advantages, such as high precision and relatively low cost. Therefore, the latest trend in resin crown manufacturing is the use of DLP 3D printers. However, studies on the internal fitness of such resin crowns are insufficient. The recently introduced 3D evaluation method makes it possible to visually evaluate the error of the desired area. The purpose of this study is to evaluate the internal fitness of resin crowns fabricated a by DLP 3D printer using the 3D evaluation method.METHODS: The working model was chosen as the maxillary molar implant model. A total of 20 resin crowns were manufactured by dividing these into two groups. One group was manufactured by subtractive manufacturing system (PMMA), while the other group was manufactured by additive manufacturing system, which uses a DLP 3D printer. Resin crowns data were measured using a 3D evaluation program. Internal fitness was calculated by root mean square (RMS). The RMS was calculated using the Geomagic Verify software, and the mean and standard deviation (SD) were measured. For statistical analysis, IBM SPSS Statistics for Windows ver. 22.0 (IBM Corp., USA) was used. Then, independent t-test was performed between the two groups.RESULTS: The mean±SD of the RMS were 41.51±1.51 and 43.09±2.32 for PMMA and DLP, respectively. There was no statistically significant difference between PMMA and DLP.CONCLUSION: Evaluation of internal fitness of the resin crown made using a DLP 3D printer and subtractive manufacturing system showed no statistically significant differences, and clinically acceptable results were obtained.

6.Sclerotherapy using 1% sodium tetradecyl sulfate to treat a vascular malformation: a report of two cases.

Vascular malformations are the most common congenital and neonatal vascular anomalies in the head and neck region. The demand for simple and esthetic vascular malformation treatments have increased more recently. In this study, two patients that were diagnosed with venous malformations were treated with sodium tetradecyl sulfate as a sclerosing agent. Recurrence was not found one year after the surgery. This article gives a brief case report of sclerotherapy as an effective approach to treat vascular malformations in the oral cavity.

7.Arthroscopic Anterior Cruciate Ligament Reconstruction using Bone - Patella tendon - Bone ( B - T - B ) Allograft: Preliminary Report.

From June 1994 to December 1995, we perfoimed arthroscopic anterior cruciate ligament reconstruction using B-T-B alIograft in nineteen patients and postoperative aggressive rehabilitation program including early range of motion (ROM) exercise and muscle strengthening exercises in all cases. Except six cases of loss of follow-up, thirteen cases were analyzed. Postoperative results were evaluated by Lysholm knee score and M?ler s criteria at average 14 months (range from 6months to 24 moths). The average Lysholm score improved from 56 to 79. According to the Miijer s criteria, the results were excellent in 7 cases, good in 5 cases and fair in 1 case. We concluded that the reconstruction of the anterior cruciate ligament using B-T-B allograft could be an effective method with many benefits in treatment of anterior cruciate ligament rupture and that the results using B-T-B allograft was as good as those using autograft. But, further evaluation and long teim follow-up should be needed.

8.An Alport syndrome boy with Van Wyk-Grumbach syndrome induced by prolonged untreated congenital hypothyroidism

Alport syndrome (AS) is a rare genetic disorder that causes progressive nephritis and is more common among males. Studies have reported an association between thyroid antibodies and hypothyroidism in patients with AS, but the relevance of this relationship is under debate. Prolonged untreated hypothyroidism induces short stature, abnormal pubertal development, and various other symptoms. However, children with long-standing hypothyroidism rarely present with signs of precocious puberty, or Van Wyk-Grumbach syndrome (VWGS). We report the case of a boy, 8 years and 4 months old, who had VWGS caused by prolonged untreated congenital hypothyroidism and AS. The boy had repeated gross hematuria and proteinuria and was diagnosed with AS by renal biopsy and genetic testing. He had normal renal function but severe growth retardation and hypothyroidism. Obesity, bone age delay, hyperlipidemia, and abnormal increased testicle size were also present due to prolonged untreated hypothyroidism. His thyroid antibody titer elevation was unclear, although ultrasonography and thyroid scanning showed a decrease in thyroid volume. We diagnosed the patient with congenital hypothyroidism caused by thyroid dysgenesis. VWGS was diagnosed due to hypothyroidism, delayed bone age, and pseudoprecocious puberty. To the best of our knowledge, this is the first report of a prepubertal Korean boy with prolonged untreated congenital hypothyroidism complicated by VWGS in AS.
9.The Allergenicity of the Storage Mites(Tyrophagus Putrescentiae and Acarus Siro) in Patients with Allergic Rhinitis.

BACKGROUND AND OBJECTIVES: Storage mites, Tyrophagus putrescentiae (TP) and Acarus siro (AS), known as the major causative allergens to people who deal with stored foods and grains, may occur more frequently in house dust than expected. During the recent 6 months, positive reactions to TP and AS were observed as frequently as those to Dermatophagoides pteronyssinus (DP) in patients with allergic rhinitis in Pusan. The purpose of this study was to identify allergenic components within TP and AS, and evaluate the cross reactivity with DP. MATERIALS AND METHODS: Using extracts of DP, TP and AS, prepared after dialysis and lyophilization, allergenic components were identified using SDS-PAGE and western blotting. Cross reactivity among them were evaluated by inhibition tests using pharmacia CAP system. RESULTS: The protein components of DP, TP and AS, showed different patterns in SDS-PAGE. Molecular weight of major IgE binding components of TP were 17 kD, 25 kD and 67 kD, and those of AS were 18 kD, 19 kD, 25 kD and 27 kD. TP- or AS-specific IgE was partially inhibited by DP extract; however, DP-specific IgE was not significantly inhibited either by TP or AS extract. Significant inhibition were noted between TP and AS. CONCLUSION: TP and AS may share common allergens with DP. However, sensitization to TP or AS was also suspected in some cases. The percentage of Acaridae family in Pusan was 12% of total mites. Therefore, storage mites should be considered as causative allergens and included in allergy test battery.

10.Simultaneous Correction of Depressed Scar Using Dermofat Graft Harvested during Scar Revision.

While no scar can be completely erased, most linear facial scars can result in a relatively good scar after revision. However, in case of round shaped depressed scar, the scar is often lengthened following an incisional technique. This study focuses on the technique of dermofat graft harvested from the adjacent scars for correction of depressed scars. 18 patients having multiple facial scars with a depressed scar among 375 patients who had undergone scar revision were treated from June 2003 to May 2004. Dermofat was harvested from the adjacent linear scar, then it was deepithelialized, reshaped, and grafted to the depressed scar through a small incision. Cosmetic results were generally good. Complications were overcorrection in 4 patients; hyperpigmentation occured in 1 patient. However, 13 patients were satisfied with the results. The advantages of our technique are as follows: it does not require additional operation; dermofat graft has low absorbable rate; it can maintain the volume with an adequate texture.
