Thyroid-stimulating hormone receptor(TSHR)is an important autoantigen whose cleavage and shedding can induce autoimmunity.The TSHR is the main functional receptor of the thyroid.The shedding of the ? subunit plays an important role in the forming of TSAb and balancing between TSAb and TBAb.Research on the cleavage and shedding of the TSHR extracellualar domain helps to gain a deep insight into the structure and function of TSHR,illuminate the pathogenesis of autoimmune thyroid disease and find out a new therapy.The structure of TSHR,the mechanism and the significance of its cleavage and shedding are reviewed in this article.
3.Clinical significance of serum metalloproteinase mat rix-9 and its tissue inhibitor-1 in type 2 diabetic patients with nephropathy

Serum metalloproteinase matrix (M MP )-9, tissue inhibitor of metalloproteinase (TIMP)-1 and type Ⅳ collagen level s were measured in patients with diabetic nephropathy (DN). The severity of DN w as determined by urinary albumin excretion rate. The results show that signific ant increases of serum TIMP-1 and type Ⅳ collagen depend on severity of DN, wh ile MMP-9 tends to decline. TIMP-1 and MMP-9 seem to participate in the patha genesis of DN.
4.Relationship between vimentin and glucocorticoid-induced cataract

Long-term use of systemic or topical glucocorticoid can cause posterior subcapsular opacities ( PSO ) , named glucocorticoid-induced cataract ( GIC ) . There are many hypotheses on the pathogenesis of GIC. However, no one has well explained the formation of PSO, which leads to no effective approaches in the prevention and/or treatment. A new opinion is that hormones might affect lens epithelial cells through GR - mediated vimentin changes, which eventually result in the formation of GIC. Therefore, the association between vimentin and lens epithelial cell proliferation and differentiation, maybe a new direction for further studies in the pathogenesis of GIC.
5.Effects of oxidative stress induced by high glucose in endothelial cells

Objective To explore the effects of oxidative stress in porcine iliac endothelial cells(PIECs) induced by high glucose. Methods After being intervened by high glucose for some time, dihydroethidium (DHE) or dihydrorhodamine 123 (DHR123) was used as a reactive oxygen species(ROS) capture. The mean fluorescent intensity(MFI) of above probes which were the products of intracellular oxidation was detected by fluorescent microscopy and flow cytometry, and the level of ROS was thus measured. With lucigenin as chemiluminescence agent, luminescence changes were detected by chemiluminescence analyzer after addition of NADPH to observe the effect of high glucose on activity of NADPH oxidase(NOX). Results Intracellular MFI was markedly elevated with the concentration of high glucose and time of exposure to high glucose. 7. Design and application of fulcrum screws for atlantoaxial dislocation

BACKGROUND: Atlantoaxial dislocation often needs surgery for reduction, and posterior screw-rod fixation fusion is the most commonly used surgery. However, the current screw-rod fixation system often increases the reduction effect by increasing the degree of curvature of connecting rod, causing great difficulty of placing rod, which increases the probability of spinal cord injury. OBJECTIVE: To evaluate the preliminary results of fulcrum screws for atlantoaxial dislocation. METHODS: From January 2017 to December 2018,11 patients with atlantoaxial dislocation were selected from General Hospital of Southern Theater Command of PLA, including 4 males and 7 females, at the age of 8-51 years. All patients were treated with posterior screw-rod fixation fusion: Conventional posterior cervical screws were implanted on both sides of the atlas and pivot screws were implanted on both sides of the axis. Atlantodental interval and Japanese Orthopaedic Association scores were recorded before and after operation. Patients were followed up and underwent X-ray and CT to estimate reduction, fixation and bone graft fusion. This study was approved by the Ethics Committee of General Hospital of Southern Theater Command of PLA. RESULTS AND CONCLUSION: (1) Eleven patients underwent operations successfully without vertebral artery or spinal nerve injury. (2) All patients were followed up for 3-12 months, with an average of 6 months. The symptoms of neck pain and neurological dysfunction were improved in varying degrees. All patients obtained atlantoaxial bone fusion, with an average fusion time of 6 months. (3) At postoperative 1 week, X-ray and CT images showed that atlantoaxial reduction was satisfactory and internal fixation was in good position. (4) Atlantodental interval reduced from preoperatively (6.8±1.5) mm to postoperatively (2.4±0.9) mm (f=17.854, P=0.000). Japanese Orthopaedic Association score increased from preoperatively (14.1±1.3) to postoperatively (15.9±0.9) (f=-10, P=0.000). (5) No complication occurred during follow-up, without recurrence or dislocation. (6) These results showed that using fulcrum screw in posterior screw-rod fixation fusion has an effective reduction for treating atlantoaxial dislocation and its preliminary clinical effects are satisfactory.

9.Observations on the Efficacy of Pricking-cupping Bloodletting in Treating Cervical Spondylotic Radiculopathy

Objective To investigate the clinical efficacy of Yang’s pricking-cupping bloodletting in treating cervical spondylotic radiculopathy and its effect on trapezius tension.Methods Seventy-eight patients with cervical spondylotic radiculopathy were randomly allocated to treatment and control groups, 39 cases each. The treatment group received pricking-cupping bloodletting and the control group, conventional acupuncture plus cupping. The nerve function was scored, the visual analogue scale (VAS) score was recorded and trapezius tension was measured in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups.Results There were statistically significant pre-/post-treatment differences in the nerve function score, the VAS score and trapezius tension in the two groups (P<0.01). There were statistically significant post-treatment differences in the VAS score and trapezius tension between the treatment and control groups (P<0.05). The total efficacy rate was 94.9% in the treatment group and 89.7% in the control group. There was a statistically significant difference in theclinical therapeutic effect between the two groups (P>0.05).Conclusions Yang’s pricking-cupping bloodletting can effectively improve pain symptoms and trapezius tension in patients with cervical spondylotic radiculopathy.
10.Study on high-performance liquid chromatographic fingerprints of different germplasms of Flos Carthami

Objective:To study the high-performance liquid chromatographic(HPLC) fingerprints of different germplasms of Flos Carthami. Methods: HPLC was applied in chromatographic separation of 36 different germplasms of Flos Carthami collected from 11 provinces of China.The chromatography condition was: Agilent Zorbax C_(18) column(4.6 mm?250 mm,5 mm);mobile phase: CAN,0.5% H_3PO_4(from 1090 to 8020);running time: 60 min;flow speed: 1.0 ml/min;detection wavelength: 265 nm;and temperature: 20℃.The data were processed by Chromatographic Fingerprint Station designed by Department of Pharmaceutical Analysis,School of Pharmacy,Second Military Medical University.Different germplasms of Flos Carthami were subjected to cluster analysis and their similarity was also evaluated.Results: The different germplasms of Flos Carthami in this study were grouped into 2 chemical types.Samples from northwest of China were of high similarity;samples from the middle China were of high similarity to those from South China.The similarity of some germplasms of Flos Carthami was significantly different.Conclusion: The chemical components of different germplasms of Flos Carthami are obviously different from each other;selection of germplasm is very important in quality control of Flos Carthami.
