Objective To study the related factors of arrhythmia in patients with different types of coronary heart disease(CHD).Methods 330 patients with coronary heart disease were studied,including acute coronary syn-drome(ACS)of 210 cases (ACS group)and chronic ischemic syndrome (CIS)in 120 patients(CIS group).The slow arrhythmia occurrence types were detected by the dynamic electrocardiogram(DCG),and analyzed the correlation of different types of CHD occurrence of arrhythmia and serum brain natriuretic peptide BNP,left ventricular ejection fraction(LVEF),LVDD,serum potassium level and the number of diseased coronary artery.Results There were no significant differences in the incidence of atrial arrhythmias and ventricular arrhythmias between the two groups(χ2 =0.982,2.136,all P>0.05),and there were no significant differences in the proportion of all kinds of arrhythmia(all P>0.05).ACS patients with arrhythmia types and BNP and LVEF,LVDD level and coronary artery lesion counts had no significant differences(χ2 =0.044,0.061,0.347,all P>0.05).Related to the type of arrhythmia and serum potassium levels,patients in a low blood potassium levels,the incidence of ventricular arrhythmias increased signifi-cantly(χ2 =4.376,P<0.05).And increased BNP level,LVEF decreased and LVDD,atrial arrhythmias occurred rate were significantly higher than those of ventricular arrhythmias(all P<0.05),gradually increased with the number of diseased coronary artery,cardiac arrhythmia occurred rate increased.CIS patients arrhythmia type with BNP and LVEF,LVDD level (all P <0.05 ).And elevated BNP levels,LVDD and LVEF decreased levels increased and ventricular arrhythmia occurrence rate was significantly higher than that of the atrial arrhythmia(χ2 =5.811,5.968, 4.060,all P<0.05).The rhythm types of arrhythmia and serum potassium levels and coronary artery disease variable count had no significant difference(χ2 =2.336,P>0.05).Conclusion The arrhythmia types of ACS are irrelevant with BNP,LVEF,LVDD and coronary artery lesions,and hypokalemia patients prone to ventricular arrhythmia.CIS is related with BNP increase,LVEF decrease and LVDD increase,prone to ventricular arrhythmias,low blood potassium patients prone to atrial arrhythmias.

Objective: To observe therapeutic effect of the method for regulating and tonifying the kidney and removing blood stasis and toxic substance on chronic renal failure. Methods: 111 cases were divided into treatment group of 71 cases and a control group of 40 cases. They were reslectively treated with Yishen Granules and Niaoduqing. Before and after treatment, blood creatinine (SCr), endogeneous creatinine clearance rate (Ccr), blood urea nitrogen (BUN). P. Ca, hemoglobin (Hb), 24-hour urine protein. total cholesterol (TC). triglyceride (TG). high density lipoprotein-cholesterol (HDL-C), low density lipoprtein-cholesterol (LDL-C). plasma endothelin (ET). calaitonin gene related peptide (CGRP) and tumor necrosis factor-? (TNF-?) were determined. Results: In the treatment group. 38 cases were markedly effective. 23 cases were effective and 10 cases were ineffective, with a total effective rate of 85. 91%. and in the control group 2 cases were markedly effective, 7 cases were effective. 11 cases were ineffective, with a total effective rate of 72. 5%. Objective:To explore the reliability, availability and security of peel-away plastic film sheath combination of ureteroscopy pneumatic lithotripsy treatment of female lower segment complexity ureteral calculi.Methods: The treatment group(40 cases) used peel-away plastic film sheath establish a bridge sheath channel between the urethra, bladder and the ureteral opening, combining under ureteroscopy pneumatic lithotripsy treatment. The control group(40 cases) with traditional under ureteroscopy pneumatic lithotripsy treatment. The clinical data of two groups of patients were analyzed.Results: The cure rate of the treatment group was significantly higher than the control group(x2=5.54,P<0.05); and the stones operation time, the bleeding, hospital stays and hospital costs was much better than control group; the complications of surgery (except the ureteral perforation) was significantly less than that of control group.Conclusion: Treatment of female lower segment complexity ureteral calculi by using the peel-away plastic film sheath establish a bridge sheath channel between the urethra, bladder and the ureteral opening, combining under ureteroscopy pneumatic lithotripsy treatment, with safe, convenient, practical, low cost, little injury, fewer complications, etc. was worthy of clinical application.

Objective:To investigate the effect and potential mechanism of combined paclitaxe and tumor necrosis factor-related poptosis-inducing ligand(TRAIL) on apoptosis of human gastric cancer cell in vitro.Methods:SGC7901 cells were cultured with paclitaxe at different concentrations(1,5,10,20,40?g/ml) and different time(12,24,36,48h).Methyl thiazolyl tetrazolium (MTT) assay was used to measure the anticancer activity of paclitaxe.The ability of TRAIL alone,paclitaxe alone and TRAIL in combination with paclitaxe to induce apoptosis was measured by a flow cytometer.Expression of DR4,DR5 mRNA was examined by RT-PCR.Results:The apoptosis rate of control group(C group),paclitaxe group(P group),TRAIL group(T group) and combination group(T+P)in 24h were 2.09%,10.65%,7.79% and 24.51% respectively.T+P group was significantly higher than that in C group,T group and P group(P

Objective To investigate the exfoliative toxin serotype genes and antibiotic resistance of Staphylococcus aureus (SA) isolated from children with staphylococcal scalded skin syndrome (SSSS). Methods In total, 108 strains of SA were isolated from 36 patients with SSSS, 36 patients with impetigo and 36 patients with abscess. Multiplex PCR was used to detect the staphylococcal exfoliative toxin A, B and D genes, Kirby-Baner method to test the susceptibilities of SA strains to 20 antibiotics. Results All the 36 SA isolates from SSSS patients were ET-positive, and 2 (6%) produced ETA, 7 (19%) ETB, 27 (75%) both ETA and ETB; of the 36 isolates from patients with impetigo, 78% produced ET, and 14% produced ETA, 64% produced both ETA and ETB, while no single ETB-producing strain was found; ET was detected in only one (2.8%) SA isolate from abscess patients, which produced both ETA and ETB. ETD was detected in none of the SA isolates. There was a statistical difference in the distribution of ET serotype among the three diseases (χ2=89.4, P < 0.01) and the proportion of ET-producing strains in SSSS group was signifi-cantly higher than that in impetigo group (χ2=9.0, P < 0.01) and abscess group (χ2= 68.1, P < 0.01). All the SA isolates were highly resistant to penicilin, ampicillin, macrolides and clindamycin, but sensitive to other 15 common antibiotics such as cephalosporin. Two strains of MRSA were found in patients with abscess. Conclusion In Chongqing, ET-producing SA is the common pathogenic bacteria of SSSS and impetigo, and most of SA strains produce both ETA and ETB.

Cerebral venous sirras thrombosis is rare clinical practice, but there has been controversial as to whether anticoagulant therapy should be performed or not. The bone of contention is whether anticoagulant therapy is safe and effective indeed. This article discusses the above problem.

Objective:To explore the influence of cardiac pacing at different sites on left atrial structure and function . Methods:A total of 62 patients ,who needed cardiac pacing therapy because of bradycardia in our hospital from Jan 2012 to Jan 2014 ,were selected .According to the implanted sites of pacing electrode ,patients were equally divided into right ventricular cardiac apex group (RVA group) and right ventricular outflow tract group (RVOT group) . Left atrial volume before atrial contraction (LAVp) ,minimum left atrial volume (LAVmin) ,maximum left atrial volume (LAVmax) ,left atrial active emptying fraction (LAAEF) and left atrial passive emptying fraction (LA-PEF) etc .were compared between two groups before and after cardiac pacing therapy .Results:There were no sig-nificant difference in all indexes between two groups before treatment ,P>0.05 all .Compared with RVA group af-ter six-month pacing treatment ,there were significant reductions in LAVp [ (29.5 ± 3.2) ml vs .(27.1 ± 3.5) ml] , LAVmin [ (18.6 ± 2.0) ml vs . (17.2 ± 2.7) ml] ,LAVmax [ (48.9 ± 4.8) ml vs .(46.1 ± 5.1) ml] and LAAEF [(47.2 ± 4.5)% vs .(43.5 ± 4.2)% ] ,and significant rise in LAPEF [(40.9 ± 5.3)% vs .(43.6 ± 3.9)% ] in RVOT group ,P<0.05 or <0.01. Conclusion:Effect of right ventricular outflow tract pacing is more than that of right ventricular cardiac apex pacing and may be used as a replacement of right ventricular cardiac apex .

The corneal asphericity refers to most normal corneal surface is an aspherical flat oval,which is helpful to improve the visual quality.In most cases,corneal aspherical coefficient Q-value is negative,and different normal Q-value is reported in literatures,corneal Q-value is influenced by many factors,quadrants have great effects on it,the influence of age is weak,there are correlation between myopic degree,ocular axial length,radius of curvature,intraocular pressure and Q-value.The corneal refractive surgery is one of the most important methods to correct refractive errors,but it has negative effects on the corneal asphericity,and then results in glare,visual quality degradation and other issues.Different corneal refractive surgeries would have different effects on the corneal asphericity.Wavefront aberration-guided femtosecond laser in situ keratomileusis(LASIK) surgery and Q-value guide LASIK are superior to standardized LASIK,the change of Q-value after femtosecond lenticule extraction (FLEx) surgery is less than that after wavefront aberration guided LASIK.The influence of femtosecond laser in situ keratomileusis (FS-LASIK) and small incision lenticule extraction (SMILE) on Q-value are rarely reported.In addition,the Q-value changes from negative to positive after corneal refractive surgery of myopia,the variation tendency is opposite after hyperopia surgery.Q-value variation is highly correlated to spherical equivalent,the impact of laser and healing still needs further exploration.In order to further exploring of the corneal asphericity and getting a better effect of surgery,here is a review about normal corneal asphericity and the effects of the corneal refractive surgery on it.

Objective　To study the law of nutrition support on trauma and critical illness,to prevent immunodysfunction,incurable infection and multiple organ dysfunction syndrome resulting from nutrition failure.　Methods　Retrospective analysis on 240 cases of trauma and critical illness.Trauma group included 126 polytrauma,35 compound injury;non-trauma group included 50 critical illness.　Results　213 recovered,27 died.62 cases trauma complicated with MODS,52 were given TPN,12 died;6 were offered EN,2 died.　Conclusions　Nutrition support (metabolic support,metabolic intervention) was important for curing trauma and critical illness.TPN was given to provide enough substrate benificial to recovery.With patients' condition improved,early enteral feed should be taken to made better amelioration.
