1.A Case of Eccrine Spiradenoma.

Eccrine spiradenoma is clinically characterized by a solitary, movable, tender noduie occurring most frequently on the ventral surfase of the upper body. A striking symptom is paroxysmal pain. Histopathologically, the encapsulated lobules of the tumor are composed of masses of basophilic cells, usually arranged in whorls, cords, and pseudogland.s. Within the whorls and pseudoglands, two types of celIs are notedperipheral intensely staining cells and central pale-staining cells. We describe herein a patient with an indolent eccrine spiradenoma, which had been diagnosed histopathologically.

4.The influencing factors of periotest. value and implant stability quotient.

STATEMENT OF PROBLEM: Periotest. and OsstellTM were known as the most objective and quantitative mobility tests available for evaluating stability of implant in vivo. Although a correlation between PTV widely used and ISQ recently introduced exist, a PTV was corresponded to various ISQ. A correct evaluation of implant stability could be obtained only after one has a thorough understanding of the limitations of devices and factors that affect measurements. PURPOSE: The purpose of this study was to investigate the causes of variables in the values obtained with these two tests. MATERIAL AND METHOD: A total of 333 implants: 134 Bra.nemark, 5 Silhouette and 194 ITI implants were investigated. Result: 1. There was a correlation between PTV and ISQ (Spearman correlation =0.39, p<0.0001) 2. The factors that affected ISQ were diameter of implant fixture, location of implant and implant system (submerged type vs non-submerged type). 3. The factors that affected PTV were diameter of implant fixture, location of implant, and elapsed time after implant placement. 4. There was no significant difference between different surface treatments of RBM, smooth surface and ti-unite on PTV and ISQ. 5. In radiographic finding, no saucerization or bone resorption has been detected in implants with ISQ values that were above the average level of each PTV. These higher values had higher bone densities around the implant fixture. Saucerization was observed in the most impants with ISQ values that were below the average level of each PTV. CONCLUSION: There was a correlation between ISQ and PTV. However, each measuring methods had factors influencing the measured values. PTV were less sensitive to marginal bone resorption and influenced with the striking point on an implant to the level of bone. With ISQ, the height of implant from bone level to transducer should be considered.

6.A Clinicopathological Study of Unilateral Absent Testes in 23 Boys.

We reviewed the medical records of 23 boys with surgically and pathologically proved monorchism. A striking laterality to the left side was identified. In 75 per cent of patients wolffian structures were present. These findings strongly suggest that in most instances of monorchism a testis had been present and had undergone subsequent in utero degeneration. Despite of various diagnostic technique for searching testes, we believe that surgical exploration continues to be the most reliable and more direct approach to the diagnosis and treatment of monorchism.

7.The Problems of Establishing Health Subcenters in Urban Areas.

Since the implementation of the national medical insurance service in 1979, it has become necessary to reconsider medical services from the geographical point of view. As more medical professionals have been concentrated in the metropolitan and urban areas, publicly hired medical practitioners have been appointed to less populated regions (e.g., farms and fisheries) and public medical service has been reinforced. During the past 10 years, the government and the ruling parties have been dragged around by strikes and collective actions of medical organizations, who have been rather dissatisfied with the new policy, namely, the separation of dispensary from medical practice. In an attempt to hold such efforts into check, the government, who has lost its "pride as a leftist," saw the needs to implement the public medical service. Consequently, new regulations were focused on the expansion of both the quantity and shares in the medical service market of the public medical service, which can be directly controlled. The government has either misunderstood the needs for urban public medical service or diverted its underlying principles. Despite some studies that reported the financial loss of urban public medical service as unavoidable, the government established seven exemplary public medical services in November 2005. The inefficiencies of such medical service were first brought to the public attention by the municipal congress of Cheonan City. Moreover, there have been some cases of distorted medical practices in the Seoul area, caused by efforts to record amplified hours of services. Owing to the financial loss and lack of effectiveness of the public medical service, such wrongful actions have been well expected. The government should first institute the underlying concepts of public medical service. The current inefficient public medical service should be scrapped, if the purpose of such service is a "public service," per se. If the public medical service only fills blanks left out by private medical service, the current public institutions have to carry out their responsibilities. The needs for current public medical service should be addressed if the quality and details of such service is to remain inferior to private medical service and keeps wasting tax.

8.A Case of Myopericytoma in the Neck.

A myopericytoma is a rare mesenchymal neoplasm composed of oval to spindle-shaped myoid appearing cells with a striking tendency for concentric perivascular growth. The vast majority of lesion is in the dermis and subcutaneous tissue of extremities. Very few cases have been reported in other locations. We hereby present, with a literature review, a case of myopericytoma in the neck.

10.Fabrication of Backscatter Electron Cones for Radiation Therapy.

PURPOSE: Irradiation cones by using backscatter electrons are made for the treatment of superficial small lesions of skin, oral cavity, and rectum where a significant dose gradient and maximum surface dose is desired. METHODS AND MATERIALS: Backscatter electrons are produced from the primary electron beams from the linear accelerators. The design consists of a cylindrical cone that has a thick circular plate of high atomic number medium (Pb or Cu) attached to the distal end, and the plate can be adjusted the reflected angle. Primary electrons strike the metal plate perpendicularly and produce backscatter electrons that reflect through the lateral hole for treatment. Using film and a parallel plate ion chamber, backscatter electron dose characteristics are measured. RESULTS: The depth dose characteristic of the backscatter electron is very similar to that of the hard x-ray beam that is commonly used for the intracavitary and superficial lesions. The basckscatter electron energy is nearly constant and effectively about 1.5 MeV from the clinical megavoltage beams. The backscatter electron dose rate of 35~85 cGy/min could be achieved from modern accelerators without any modification. and the depth in water of 50% depth dose from backscatter electron located at 6mm for 45degrees angled lead scatter. The beam flatness is dependent on the slit size and the depth of treatment, but is satisfactory to treat small lesions. CONCLUSIONS: The measured data for backscatter electron energy, depth dose flatness dose rate and absolute dose indicates that the backscatter electrons are suitable for clinical use.
