1.Effect of bacteriophage lysin on lysogens.

OBJECTIVETo study the effect of phage lysin on the growth of lysogens.
METHODSSputum specimens processed by modified Petroff's method were respectively treated with phagebiotics in combination with lysin and lysin alone. The specimens were incubated at 37 °C for 4 days. At the end of day 1, 2, 3 and day 4, the specimens were streaked on blood agar plates and incubated at 37 °C for 18-24 hours. The growth of normal flora observed after day 1 was considered as lysogens.
RESULTSSputum specimens treated with phagebiotics-lysin showed the growth of lysogens. When specimens treated with lysin alone, lysogen formation was avoided and normal flora was controlled.
CONCLUSIONSLysin may have no effect on the growth of lysogens.

3.Treatment of Drug Susceptible Pulmonary Tuberculosis.

Tuberculosis (TB) remains a major global health problem, and the incidence of TB cases has not significantly decreased over the past decade in Korea. The standard short course regimen is highly effective against TB, but requires multiple TB-specific drugs and a long treatment duration. Recent studies using late-generation fluoroquinolones and/or high-dose rifapentine-containing regimens to shorten the duration of TB treatment showed negative results. Extending the treatment duration may be considered in patients with cavitation on the initial chest radiograph and positivity in sputum culture at 2 months of treatment for preventing TB relapse. Current evidence does not support the use of fixed-dose combinations compared to separate drugs for the purpose of improving treatment outcomes. All patients receiving TB treatment should be monitored regularly for response to therapy, facilitation of treatment completion, and management of adverse drug reactions. Mild adverse effects can be managed with symptomatic therapy and changing the timing of the drug administration, but severe adverse effects require a discontinuation of the offending drugs.

4.Evaluation of microscopic observation drug susceptibility for drug susceptibility testing of mycobacterium tuberculosis in smear-positive sputum.

OBJECTIVETo evaluate microscopic observation drug susceptibility (MODS) for mycobacterium tuberculosis drug susceptibility in smear-positive sputum.
METHODSDrug susceptibility of mycobacterium tuberculosis in 275 smear-positive sputum samples collected from TB patients were detected directly by MODS. The susceptibility of seven antimicrobials including streptomycin, isoniazid, rifampicin, ethambutol, levofloxacin, amikacin and capromycin were detected MODS. At the same time the sputum sample were cultured in MGIT 960 tube and the positive isolates were tested for drug susceptibility by MGIT 960 system. The results of MODS were analyzed and compared with that of MGIT 960.
RESULTSOf 275 smear-positive sputum, MODS detected 235 (85.45%). Results of MODS were obtained in a median time of 18 days (5 - 39 d). For the 235 MODS-positive samples, the compliance rates of MODS to MGIT of 7 drugs were 90.21% (212/235), 88.09% (207/235), 93.62% (220/235), 87.23% (205/235), 92.34% (217/235), 88.51% (208/235) and 86.81% (204/235) respectively. The sensitivity of MODS method were 83.33% (90/108), 85.11% (120/141), 90.74% (98/108), 85.71% (78/91), 86.73% (85/98), 76.92% (40/52) and 77.08% (37/48). The specificities of MODS method were 96.06% (122/127), 92.55% (87/94), 96.06% (122/127), 88.19% (127/144), 96.35% (132/137), 91.80% (168/183) and 89.30% (167/187) respectively.
CONCLUSIONMODS is an optimal alternative method for direct and rapid drug susceptibility of sputum with high accuracy in a timely and affordable way in resource-limited settings.

5.Study on drug-resistance of Mycobacterium tuberculosis isolated from coal workers with silico-tuberculosis.

OBJECTIVETo study the gene mutation and streptomycin, isoniazid or rifampicin resistance of Mycobacterium isolated from silico-tuberculosis patient's sputum so as to find a more effective therapy for this disease.
METHODSMycobacteria tuberculosis were separated from 96 coal worker with silico-tuberculosis firstly. Then rpsL, KatG and rpoB fragments of genome were copied with PCR and compared their SSCP profiles with standard strains.
RESULTS67 strains of streptomycin, isoniazid or rifampicin resistant Mycobacteria tuberculosis were found in routine drug resistance test, with the percentages of 80.5% (54/67), 58.2% (39/67) respectively. PCR-SSCP showed that out of 67 drug-resistant strains, 66(98.5%) of rpsL, 47(70.1%) of rpoB and 42(62.7%) of KatG appeared abnormal.
CONCLUSIONMost of the resistant strains appeared gene mutation. The mution rates were higher than the results from routine drug resistance test.

6.The results and drug susceptibility of respiratory secretion culture of children with trachea bronchial foreign bodies.

OBJECTIVE:
To analyze the distribution and drug resistance of the pathogenic bacteria in respiratory secretion in children with trachea bronchial foreign bodies so as to assist physicians in clinical prescription.
METHOD:
Sputum specimens of 622 children with trachea bronchial foreign bodies were collected,and the drug susceptibility test was peformed.
RESULT:
Pathogenic bacteria were detected in 124(19. 94%) of 622 sputum specimens. Most detected gram-negtive bacilli were highly sensitive to amikacin, ciprofloxacin, ceftriaxone, cefepime and ceftazidime, no strains were resistant to imipenem and meropenem; 42 strains were gram-positive bacilli. The former were highly sensitive to levofloxacin and chloramphenico,the latter were highly sensitive to ciprofloxacin, moxifloxacin and linezolid, no strains were resistant to rifampicin and vancomycin.
CONCLUSION
The frequent pathogenic bacteria in respiratory secretion in children with trachea bronchial foreign bodies include gram-negtive bacilli such as enterobacter cloacae, klebsiella pneumonia, escherichia coli, acinetobacter baumannii, serratia marcescens, and gram-positive bacilli such as streptococcus pneumonia,staphylococcus aureus. The detected gram-negtive bacilli were sensitive to imipenem and meropenem;the detected gram-positive bacilli were sensitive to rifampicin and vancomycin.

7.Observation on therapeutic effect of acupuncture combined with chinese herbs on polycystic ovary syndrome of kidney deficiency and phlegm stasis type.

OBJECTIVETo observe the clinical therapeutic effect of acupuncture combined with Chinese herbs on polycystic ovary syndrome of kidney deficiency and phlegm stasis type and probe into the mechanism.
METHODSSixty-three cases of polycystic ovary syndrome of kidney deficiency and phlegm stasis type were randomly divided in to a combined acupuncture and Chinese herb group (n=32) treated with acupuncture at Qihai (CV 6), Guanyuan (CV 4), et al. and oral administration of Chinese herbs, and a simple Chinese herb group (n=31) treated with oral administration of the same Chinese herbs as in the combined acupuncture and Chinese herb group. The therapeutic effects and changes of follicle stimulating hormone (FSH), luteotropic hormone (LH), testosterone (T) and LH/FSH were compared between the two groups.
RESULTSThe total effective rate was 93.8% in the combined acupuncture and Chinese herb group and 80.6% in the simple Chinese herb group, the former being significantly better than the latter (P < 0.05). The decrease of T in the combined acupuncture and Chinese herb group was significantly su perior to that in the simple Chinese herb group (P < 0.01).
CONCLUSIONAcupuncture combined with Chinese herb therapy is superior to the simple Chinese herb in the clinical therapeutic effect on polycystic ovary syndrome of kidney deficiency and phlegm stasis type and decrease of T level, indicating this method is a better one for polycystic ovary syndrome of kidney deficiency and phlegm stasis type.

8.Rapid testing on drug susceptibility of Mycobacterium tuberculosis with nitrate reducrase assay.

OBJECTIVETo establish a rapid method for testing drug sussceptibility on Mycobacterium tuberculosis.
METHODSTaking absolute Concentration method for drug susceptibility testing of M. tuberculosis as the "gold standard", we examined the drug-resistant of M. tuberculosis strain with nitrate reducrase assay (NRA) and the drug-resistant of M. tuberculosis germ in sputum with NRA.
RESULTSNRA and absolute concentration method was basically comparable with NRA susceptibility as 96.5% and the specificity was 100%, When comparing with traditional absolute concentration method, NRA could shorten the time about 3 weeks. Using NRA to test the drug-resistant of M. tuberculosis germ in sputum, its susceptibility was more than 66.7% and specificity was 100%, within 10-20 days.
CONCLUSIONNRA could be used as a rapid drug susceptibility testing on M. tuberculosis.

9.Application of generalized estimation equations to establish prediction equation for tuberculosis drug resistance in Zhejiang province.

Objective: Drug-resistant tuberculosis (TB) may be resistant to one or multiple anti-TB drugs. We used generalized estimation equations to analysis the risk factors of drug-resistant TB and provide information for the establishment of a warning model for these non-independent data. Methods: The drug susceptibility test and questionnaire survey were performed in sputum positive TB patients from 30 anti TB drug-resistance surveillance sites in Zhejiang province. The generalized estimation model was established by the GENMOD module of SAS, with resistance to 13 kinds of anti-TB drugs as dependent variables and possible influencing factors, such as age, having insurance, HBV infection status, and history of anti-TB drug intake, as independent variables. Results: In this study, the probability of drug resistance at baseline level was 20.26%. Age, insurance, whether being co-infected with HBV, and treatment history or treatment withdrawal were statistically significantly correlated with anti-TB drug resistance. The prediction equation was established according to the influence degree of the factors mentioned above on drug resistance. Conclusion: The generalized estimation equations can effectively and robustly analyze the correlated binary outcomes, and thus provide more comprehensive information for drug resistance risk factor evaluation and warning model establishment.
