3.Alternatives of anterior and posterior approaches for cervical spondylotic myelopathy.

OBJECTIVETo discuss on the effect of cervical spondylotic myelopathy before and after surgery and assess its indications and efficacy.
METHODSFrom June 2002 to June 2006, 125 patients with cervical spondylotic myelopathy were analyzed retrospectively involving 58 anterior routine and 67 posterior routine, of which 71 cases of males, 54 cases of females, aged from 28 to 69 years (average 53.4 years). The course was 0.5 to 48 months (means 14 months). According to JOA score system preoperative and postoperative nerve function were analyzed, summarized anterior and posterior cervical spine surgery.
RESULTSAll patients were followed up for from 6 to 30 months (average of 18 months). According to JOA score criteria: anterior cervical surgery preoperative JOA score was (8.78 +/- 2.43) points, postoperative JOA score was (14.68 +/- 2.37) points, the results were excellent in 40 cases, good in 10 cases, effective in 6. Posterior surgical group preoperative JOA score was (8.49 +/- 2.58) points, postoperative JOA score was (14.26 +/- 2.83) points, the results were excellent in 42 cases, good in 12 cases, effective in 8. Invalid operation occurrenced in 6 cases included 2 of anterior, 4 of posterior. The postoperative symptoms had worsed in 1 case of posterior operation. The two groups had no statistical difference in efficacy, but there were differences statistically in the same approach comparing preoperative and postoperative.
CONCLUSIONBoth anterior and posterior approaches have fine effect to the treatment of cervical spondylotic myelopathy (CSM). But the selection of the indication is very important, the patients with the pressure from anterior, the short-level changes, and the main symptoms of pyramid trac compression, adopt anterior approach on principle. While the patients with the pressure of spinal cord from posterior, multilevel changes, main symptoms of sensory disturbances, and accompanied by cervical spine canal stenosis mainly is introduced posterior approach.

4.Clinical observation on cervical spondylosis of neck type treated with acupuncture at original and terminal points of trapezius.

OBJECTIVETo compare the differences of therapeutic effect of cervical spondylosis of neck type treated with acupuncture at starting and ending points of trapezius and the Jiaji (EX-B 2) points.
METHODSSixty cases of cervical spondylosis of neck type were randomly divided into an observation group and a control group, 30 cases in each group. In observation group, the acupoints related with starting and ending points of trapezius, such as Tianzhu (BL 10), Fengchi (GB 20), Quyuan (SI 13), Jugu (LI 16) and Ashi were punctured; in control group, Jiaji (EX-B 2) points of neck were punctured. The treatments were applied 5 times a week and 2 weeks made one course. The international simplified McGill scale was adoped to assess the scores of Pain Rating Index (PRI), Visual Analogue Scale (VAS) and Present Pain Intensity (PPI) before treatment and after one course treatment, and the therapeutic effects were measured.
RESULTSAfter treatment, all the scores were obviously reduced in both groups: (all P < 0.01), and the PRI-sensory subscore, PRI-affective subscore and total score in observation group reduced more obviously than those in control group, and there were statistically significant differences between groups (all P < 0.01). There were no statistically significant differences between groups in VAS and PPI score (both P > 0.05). The total effective rate of 96.7% (29/30) in observation group was superior to that of 70.0% (21/30) in control group (P < 0.01).
CONCLUSIONThe therapeutic effect of cervical spondylosis of neck type treated with acupuncture at the starting and ending points of trapezius is positive, which is better than that of acupuncture at the Jiaji (EX-B 2) points.

5.Cervical spondylosis of vertebral artery treated with thermosensitive moxibustion with different dosages: a randomized controlled trial.

OBJECTIVETo compare the difference in the clinical efficacy on cervical spondylosis of vertebral artery type (CSA) treated with thermosensitive moxibustion at different dosages.
METHODSSixty cases of CSA were randomized into a saturated moxa dosage group and a regular moxa dosage group, 30 cases in each one. The thermosensitive moxibustion was adopted in the two groups. The mild suspended moxibustion was applied at two acupoints with the strongest thermosensitization. In the saturated moxa dosage group, the moxibustion time was determined by the disappearance of thermosensitization. In the regular moxa dosage group, 15 min was required on each acupoint. The treatment was given twice a day for first 4 days in the two groups. Since the 5th day, the treatment was given once a day, continuously for 10 times, and totally 14 days were required. The score of symptoms and function and clinical efficacy were compared between the two groups before and after treatment as well as 6-month follow-up after treatment.
RESULTSThe curative and effective rate was 56.7% (17/30) after treatment and 60.0% (18/30) in 6-month follow-up after treatment in the saturated moxa dosage group, which were superior to 26.7% (8/30) and 30.0% (9/30) in the regular moxa dosage group respectively (P < 0.01, P < 0.05). The scores of clinical symptoms and function after treatment and in follow-up were improved apparently as compared with those before treatment in both groups (all P < 0.01). The scores of clinical symptoms and function after treatment and in follow-up in the saturated moxa dosage group were increased much more apparently than those in the regular moxa dosage group (after treatment: 22.32 +/- 4.64 vs 17.43 +/- 3.21; in follow-up: 23.01 +/- 4.76 vs 18.32 +/- 2.13, both P < 0.01).
CONCLUSIONThe thermosensitization moxibustion of saturated dosage achieves the superior short-term and long-term efficacies in the treatment of CSA as compared with the regular moxibustion dosage.

7.Application of finite element analysis in Chinese cervical manipulation biomechanics.

Clinical advantages of Chinese spinal manipulation therapy (CSMT) were recognized for spinal chronic lesions of soft tissues and bones, such as cervical spondylosis, etc. However, the security of CSMT and the hypotheses of practice mechanisms were questioned for lacking of the relevant basic researches. Researches have proved that these methods could be used to observe the dynamic effects of spine with application of finite element analysis (FEA) computer technology. Combining with other biomechanical experimental methods and applying advanced FEA technology for mechanical problems of CSMT, we may not only find the mechanisms of action and provide theoretical supports for the traditional Chinese therapy, but also standardize the key techniques and optimize the treatment options improving clinical outcomes, and even promote spreading of CSMT. Computer models are ideally suited for studying phenomena that cannot be satisfactorily investigated with other models. However, computer models of CSMT practice remain to be further refined. The results which had been acquired so far not only verified some of the traditional points of view, but also revised and specified some perspectives of the past. This paper intends to review FEA studies with Chinese cervical manipulation therapy (CCMT) for cervical spinal chronic lesions of soft tissues and bones, involving different effects for cervical spine joints (pulling/traction and thrusting) with practice techniques and cervical spine soft tissues (including vessels and its hemodynamics, muscles and fasciae, etc).

8.Efficacy comparison between needle-knife therapy and acupuncture-cupping for cervical spondylosis of cervical type.

OBJECTIVETo compare the efficacy differences between needle-knife therapy and acupuncture-cupping for treatment of cervical spondylosis (CS) of cervical type.
METHODSSixty cases of CS were randomly divided into a needle-knife group (30 cases) and an acupuncture-cupping group (30 cases). The needle-knife therapy was applied at points among superior nuchal line of occipital bone, bilateral neck muscle, neck centerline, trapezius and medial border scapula for only once. In the acupuncture-cupping group, acupuncture was applied at Fengchi (GB 20), Fengfu (GV 16), Tianzhu (BL 10), Dazhui (GV 14), Jianjing (GB 21), Jiaji (Ex-B2, from C4 to C6), Houxi (SI 3) and Ashi point, followed by cupping on local skin, once every other day for totally six times. The score of neck stiffness and visual analogue scale (VAS) were observed before and after treatment, in follow-up of 1, 3 and 6 months after treatment in the two groups, and the efficacy was compared.
RESULTSIn the needle-knife group, 9 cases were cured, 12 cases were markedly effective, 8 cases were effective and 1 case was failed; the total effective rate was 96.7% (29/30) and the cured and markedly effective rate was 70.0% (21/30). In the acupuncture-cupping group, 8 cases were cured, 9 cases were markedly effective, 11 cases were effective and 2 cases were failed; the total effective rate was 93.3% (28/30) and the cured and markedly effective rate was 56.7% (17/30). The difference of total effective rate in the two groups was not statistically significant (P > 0.05), but the cured and markedly effective rate of needle-knife group was significantly superior to that of acupuncture-cupping group (P < 0.05). The needle-knife therapy was significantly superior to acupuncture-cupping on improvement of neck stiffness in the follow-up of 1, 3, 6 months after treatment (P < 0.05, P < 0.001); both treatments were effective on relief of neck pain, but the needle-knife group had better effects in the follow-up of 3 and 6 months after treatment compared with acupuncture-cupping group (both P < 0.01).
CONCLUSIONThe needle-knife therapy has better effects on neck stiffness and pain relief than acupuncture-cupping, and it is more treatment time saving.

10.Chronic neck pain of cervical spondylosis treated with acupuncture and moxibustion in terms of the heart and kidney theory: a randomized controlled trial.

OBJECTIVETo evaluate the clinical efficacy of acupuncture and moxibustion on chronic neck pain of cervical spondylosis in terms of the heart and kidney theory.
METHODSOne hundred and eleven cases were randomized into a heart-kidney acupuncture group (55 cases) and a conventional acupuncture group (56 cases). In the heart-kidney acupuncture group, acupuncture was applied to Bailao (EX-HN 15), Shenmen (HT 7) and Wangu (SI 4). Afterwards, the direct moxibustion was applied to Dazhui (GV 14), Xinshu (BL 15) and Shenshu (BL 23). After moxibustion, the intradermal needling therapy was provided at Bailao (EX-HN 15), Xinshu (BL 15) and Shenshu (BL 23). In the conventional acupuncture group, acupuncture was applied to Bailao (EX-HN 15) and Zhongzhu (TE 3) at first. Afterwards, the direct moxibustion was done at Dazhui (GV 14), Bailao (EX-HN 15) and Jianzhongshu (SI 15). After moxibustion, the interdermal needling therapy was provided at Bailao (EX-HN 15) and Jianzhongshu (SI 15). The northwick park pain questionnaire (NPQ) was adopted as the main efficacy index for the evaluation.
RESULTSAfter the intervention and during the follow-up visit period, NPQ scores were all reduced remarkably as compared with those before the intervention for the patients in two groups (all P < 0.001), but the differences were not statistically significant in groups (all P > 0.05). By the age stratification analysis for the patients in two groups, the program in terms of the heart and kidney theory achieved the superior efficacy for the patients over 45 years old as compared with those in the conventional acupuncture group (P < 0.05, P < 0.01). By the stratification analysis of the sick duration, the program in terms of the heart and kidney theory achieved the superior efficacy for the patients with over 7 years sick duration as compared with those in the conventional acupuncture group (P < 0.05, P < 0.01).
CONCLUSIONThe acupuncture and moxibustion therapeutic program in terms of the heart and kidney theory achieves the superior efficacy on chronic pain of cervical spondylosis for the patients over 45 years old and with over 7 years sick duration. It is one of the optimized programs for the treatment of this disease.
