1.Eccrine Nevus on the Neck: A Case Report and Review of the Literature

Eccrine nevus is an extremely rare benign cutaneous hamartoma that usually occurs in childhood and adolescence. It has a wide range of clinical manifestations, and histological findings reveal a proliferation of structurally normal eccrine ducts. Herein, we present a case of eccrine nevus on the neck of an 8-year-old girl. Our literature review reveals that the head and neck region is a rare anatomical location for eccrine nevus as it has a predilection for extremities. Our review also suggests that overlying skin changes are common in eccrine nevus regardless of accompanying localized hyperhidrosis.

5.Immunohistochemical Profiling Reveals Distinct Inflammatory Landscape in Rosacea Subtypes

Background:
The immunological and histopathological understanding of rosacea subtypes remain unclear. This study aimed to characterize erythematotelangiectatic rosacea (ETR), papulopustular rosacea (PPR), and granulomatous rosacea (GR) immunologically through facial punch biopsy samples.
Objective:
Our goal was to investigate the immunohistochemical profile of rosacea subtypes, providing insights into pathogenesis for targeted therapies.
Methods:
Biopsy samples from 52 rosacea patients and 25 controls were stained for antibodies retrospectively. Statistical analyses identified expression differences.
Results:
In the rosacea group (average age, 55.0; male-to-female ratio, 1:2.1), Langerhans cell count, p53, and vitamin D receptor expression showed no differences from controls or among subtypes. Claudin-1 and occludin expression decreased in rosacea compared to controls, with no variance among subtypes. Demodex mites were present in 40.4% of rosacea cases vs. 8.0% in controls (p=0.003), more frequently in PPR than GR, and absent in ETR. Neutrophil elastase expression mirrored the Demodex pattern. The CD4/CD8 ratio averaged 2.15 in rosacea without differences among subtypes. CD20 and CD68 expression increased in rosacea, escalating in the order of ETR, PPR, and GR, mirroring matrix metalloproteinase-2 (MMP-2). ETR exhibited insignificant CD20 and MMP-2 expression.
Conclusion
Our findings validate a CD4+ T cell-driven response across all rosacea subtypes. Increased neutrophils, B cells, and macrophages, likely influenced by Demodex, were observed in PPR and GR. Demodex mites may recruit additional B cells and macrophages, potentially linked to MMP-2 expression. This comprehensive characterization offers additional insights into the immunopathogenesis of rosacea, paving the way for targeted interventions.
6.Does Immunotherapy of Viral Warts Provide Beneficial Effects When It Is Combined with Conventional Therapy?.

BACKGROUND: Cryotherapy has been accepted as the mainstay in treating periunugal and palmoplantar warts. The major drawback of cryotherapy is the requirement of several unbearably painful treatment sessions. OBJECTIVE: This study aims to assess the efficacy of immunotherapy in viral wart treatment, as an adjunctive method to cryotherapy. METHODS: Retrospective chart review was performed on 124 patients visiting the hospital from January to December 2009 for the treatment of periungual and plantar warts. We analyzed the number of cryotherapy sessions necessary for treating warts and assessed the clinical benefits from the addition of other treatment modalities, by adjusting the various confounding factors. RESULTS: Of the 124 investigated patients, immunotherapy with diphenylcyclopropenone (DPCP) was performed in 14 patients (11%), together with cryotherapy. After adjusting the factors related to the therapeutic difficulties of wart, the average number of cryotherapy sessions for the immunotherapy-combined group was significantly lower (3.58+/-1.25) than that for the cryotherapy only group (5.10+/-0.44) (p=0.026). However, there were no differences in the number of treatment sessions of cryotherapy when topical 5-FU/salicylic acid agents were added to the treatment. CONCLUSION: Immunotherapy may be a successful adjuvant to cryotherapy in reducing the number of agonizing cryotherapy sessions.

7.A Case of Subcutaneous Fat Necrosis of Newborn (SCFNN) Associated with Meconium Aspiration Syndrome.

Subcutaneous fat necrosis of newborn (SCFNN) is a rare disease. Characteristically, full-term infants with associated perinatal problems such as hypoxia, hypothermia, and obstetric trauma are affected in the first week of life. We report a case of SCFNN, which presented as an erythematous subcutaneous mass on the back of a male neonate who suffered from meconium aspiration pneumonia with neonatal sepsis.

8.Intense Pulsed Light and Low-Fluence Q-Switched Nd:YAG Laser Treatment in Melasma Patients.

BACKGROUND: Recently, low fluence collimated Q-switched (QS) Nd:YAG laser has drawn attention for the treatment of melasma. However, it needs a lot of treatment sessions for the substantial results and repetitive laser exposures may end up with unwanted depigmentation. OBJECTIVE: We evaluated the clinical effects and safety of the combinational treatment, using intense pulsed light (IPL) and low fluence QS Nd:YAG laser. METHODS: Retrospective case series of 20 female patients, with mixed type melasma, were analyzed using medical records. They were treated with IPL one time, and 4 times of weekly successive low fluence Nd:YAG laser treatments. At each visit, digital photographs were taken under the same condition. Melanin index (MI) and erythema index (EI) were measured on the highest point on the cheekbones. Modified melasma area and severity index (MASI) scores were calculated by two investigators using digital photographs. RESULTS: The mean values of MI and EI decreased significantly after treatments. The modified MASI score has decreased by 59.35%, on average. Sixty percents of the participants did not require any more treatments, and no clinical aggravations were observed during the follow-up period (mean 5.9 months). CONCLUSION: IPL and low fluence laser may elicit a clinical resolution in the mixed type melasma with long term benefits.

9.A Case of Annular Polycyclic Type of Subacute Cutaneous Lupus Erythematosus.

We report an annular type of subacute cutaneous lupus erythematosus(SCLE) which is rare in Korea, A 23-year-old female had erythematous annular rashes with a mild itching sensation on the face, trunk and both extremities. She also had painless oral mucosal ulceration, diffuse hair loss and mild arthralgia. Serologic abnormalities of the patient were positive anti-Ro Ab, ANA and Anti-ds-DNA Ab. The histopathologic findings showed vacuolar degenerations in the basal cell layer, mononuclear cell infiltrate in the upper dermis, and dermal edema. Direct IF revealed granular deposition of IgG and IgA at the dermo-epidennal junction.

10.Analysis of Theory-applied Research in the Journal of Korean Academy of Nursing Administration (2007~2016)

PURPOSE: This study was done to analyze the trend of theory application in articles published in the Journal of Korean Academy of Nursing Administration (JKANA) over the last 10 years. METHODS: Four hundred and eighty-five articles were reviewed to determine the scope of theory-applied research. General characteristics, study design and keywords classification by nursing meta-paradigm were analyzed using established analysis framework. RESULTS: Twenty-four articles (4.9%) were identified as theory-applied research publications. More than half of the articles (54.2%) stated that a theoretical framework was used; the number of theories used in the 24 articles was 27 including nine nursing theories. The majority of the studies were conducted in hospital settings. In the keyword classification by nursing meta-paradigm, 28 environment-related keywords were identified but only 7 keywords were related to health. CONCLUSION: The results show that theory application in Korean nursing administration research is very limited. It is necessary to create a positive educational environment for graduate students to learn about nursing theories as well as theories from other disciplines. In addition, different levels of nursing theories should be continuously developed in the field of nursing administration.
