1.Reference Values for the Revised Anti-Müllerian Hormone Generation II Assay: Infertile Population-based Study.

Anti-Müllerian hormone (AMH) is now accepted as an important clinical marker of ovarian reserve and is increasingly measured as an initial evaluation at infertility clinics. The aim of this study was to establish reference values for the revised second generation (Gen II) assay using population-based data. In this population-based cohort study, AMH data from unselected infertile women aged 25–45 years from June 2013 to June 2014 (n = 15,801) were collected. The AMH values were measured using the revised Gen II assay. We established and validated 5 AMH-age regression models. Based on the optimal AMH-age model, reference values and centile charts were obtained. The quadratic model (log AMH = 0.410 × age − 0.008 × age²− 3.791) was the most appropriate for describing the age-dependent decrease in AMH measured using the revised Gen II assay. This is the largest population-based study to establish age-specific reference values of AMH using the revised Gen II assay. These reference values may provide more specific information regarding the ovarian reserve estimation of infertile women.

2.Mother's Parenting Style at Meal Time and Their Preschooler's Dietary Behavior.

OBJECTIVES: This study was conducted to evaluate the nutrition quotient (NQ) by mother's parenting style which may influence the NQ in preschool children. METHODS: Subjects were 310 mothers and their 4-6 year old children. The questionnaire composed of demographic characteristics, mother's parenting style at meal time and eating behavior as measured by NQ questions. The NQ questions consisted of 19 food behavior checklist items and all items were grouped into 5 factors: balance, diversity, moderation, regularity, and practice. Mother's parenting style was classified by using words for nutrition education at meal time. All data were statistically analyzed by SPSS program (Ver. 23) and the statistical differences in variables were evaluated by Student's t-test, χ²-test, One-way ANOVA. RESULTS: We observed that in children whose mothers use the parenting style at meal time of ‘explanation’ and ‘compliment & cheer up’ had high dietary regularity, diversity, practice. The children of mothers who use the parenting style at meal time of ‘persuasion’ and ‘reward’ were found to have a lower degree of balance, diversity, and practice. Especially, children of ‘reward’ style mothers had lower moderation of dietary life. On the other hand, among the parenting style at meal time of ‘comparison & demand’, ‘treating’ and ‘faire’, there was no significant difference in the NQ factor by each group. NQ grade was higher among those who used more explanation (p < 0.001) and persuasion (p < 0.01) and with use of less persuasion (p < 0.01) and reward (p < 0.01). The positive association observed between the frequency of dietary education of mothers and higher NQ grade indicated the degree of dietary practices of those children. On the other hand, the children of mothers who rarely practice the dietary education at home had lower NQ grade (p < 0.001). CONCLUSIONS: In order to promote children's proper dietary behaviors, it is important to provide nutrition education to children as well as provide guidance on parenting style at meal time.

3.Headache associated with polyneuritis cranialis

Polyneuritis cranialis (PNC), a variant of Guillain-Barré syndrome (GBS), is a very rare disorder of multiple cranial nerve palsies without any peripheral nerve involvement or ataxia. While pain is frequent in GBS, headache is not known in the patient with PNC. We report a 72-year-old man who presented acute bilateral multiple cranial neuropathy, including neuropathies of cranial nerve III-XII and severe and prolonged headache. The brain MRI, cerebrospinal fluid examination, and peripheral nerve conduction studies showed no abnormalities. The headache improved in five weeks. All cranial neuropathies resolved completely 8 months later. We suggest that headache should be considered as an additional cardinal feature of PNC along with multiple cranial neuropathies
4.Disagreement between direct and indirect blood pressure measurements obtained from minipigs

Blood pressure (BP) measurement plays a pivotal role in veterinary medicine for diagnosing cardiovascular disorders and monitoring anesthesia of animals. Although indirect BP measurement has been widely applied to monitor BP because of its convenience and non-invasiveness, it is still unclear whether indirect BP measurement is compatible with direct BP measurement in minipigs. In addition, the effect of animal posture during BP measurement is not well understood in minipigs despite its importance to cardiovascular performance. Therefore, both systolic and diastolic arterial BPs in minipigs were measured via femoral artery catheterization for direct BP measurement and using a compressive cuff as an indirect BP measurement under the dorsal or right lateral recumbent postures. Numerical values were processed by the Bland-Altman method to calculate the bias ± SD and the limits of agreement (LOA). In accordance with the American College of Veterinary Internal Medicine guidelines, the results between direct and indirect BP measurements were determined as apparent disagreements in both systolic and diastolic arterial BPs under all postures because of large bias ± SD and wide LOA. The results of the present will help prevent misinterpretation of the anesthetized patient's condition during monitoring of BP by indirect measurement.

5.Influence of Partnerships with Nurses and Social Support on Readiness for Discharge among Mothers of Premature Infants

PURPOSE: The purpose of this study was to investigate partnerships with nurses, social support and readiness for discharge among mothers of premature infants in the neonatal intensive care unit (NICU) and to examine the factors associated with readiness for discharge.METHODS: A survey was conducted among 85 mothers of premature infants hospitalized in a NICU in Seoul, Korea. The collected data were analyzed with descriptive statistics, the t-test, one-way analysis of variance, Pearson' correlation coefficients, and multiple linear regression using SPSS version 25.0.RESULTS: The results of the regression analysis showed that partnerships with nurses (β=.32, p=.011) and parenting experience (β=.32, p=.001) were significantly associated with readiness for discharge.CONCLUSION: To improve the readiness for discharge among mothers of premature infants, developing strategies to strengthen their partnership with nurses and to provide family-centered care will be needed.

7.Prevalence of Dental Anomalies in Patients with Non-syndromic Cleft Lip with or without Cleft Palate

This study aimed to assess the prevalence and distribution pattern of dental anomalies in the permanent teeth of patients with non-syndromic cleft lip with or without cleft palate. Additionally, it aimed to compare differences in dental anomalies between cleft and non-cleft areas, considering gender and cleft phenotype. Panoramic radiographs of 164 patients diagnosed with non-syndromic orofacial clefts were retrospectively analyzed by a single examiner to confirm dental anomalies. The dental anomalies investigated included tooth agenesis, supernumerary teeth, microdontia, rotation, ectopic eruption, and enamel hypoplasia. Cleft phenotypes were categorized into 7 types based on medical and dental records. A significantly higher prevalence of supernumerary teeth was observed in males than females within non-cleft areas (p = 0.017), with no significant differences in other dental anomalies. In non-cleft area, patients with cleft palate exhibited a high prevalence of tooth agenesis (p < 0.0001) and microdontia (p = 0.012) compared to other cleft phenotypes. Maxillary incisor rotation was closely associated with adjacent tooth agenesis in unilateral cleft lip and palate cases (p = 0.034). This study suggests that the additional subphenotype based on dental anomalies in patients with orofacial cleft may serve as applicable clinical markers.
8.The Serogroup and Antimicrobial Resistance of Salmonella spp. Isolated from the Clinical Specimens During 6 years in a Tertiary University Hospital.

BACKGROUND: Recently, non-typhoidal salmonellosis is increasing and it constituted over 90% of total salmonellosis in 1990s. The antimicrobial resistance of non-typhoidal Salmonella gets higher. So we described the change of serogroup and antimicrobial resistance of Salmonella isolated from clinical specimens in Ewha Womans University Mokdong Hospital during 6 years. METHODS: Clinical specimens were submitted from 1997 to 2002. Stool cultures were inoculated onto MacConkey (MAC) agar and Salmonella-Shigella (SS) agar and into Selenite F (SF) enrichment broth. Identification of Salmonella were performed by Vitek GNI card (BioMerieux, Marcy-I'Eltoile, France) and serotyping were done. Antimicrobial resistance test were performed by Vitek GNS card (BioMerieux, Marcy-I'Eltoile, France). RESULTS: From 1997 to 2002, 594 strains of Salmonella were isolated. Non-typhoidal Salmonella and Salmonella typhi constituted 94.4% and 5.6%. Non-typhoidal Salmonella were mainly composed of group B (21.5%) and group D (48.0%), but in 2002, group C (12.4%) and group E (27.9%) were increased in number. The antimicrobial resistance rate of non-typhpoidal Salmonella were 28% to ampicillin, 4.1% to SXT, 0.2% to ciprofloxacin and 0.7% to ceftriaxone. The animicrobial resistance rate of group B and D Salmonella showed 37.5% and 32.6% to ampicillin, 7.8% and 4.2% to SXT, respectively. CONCLUSIONS: Serogroup B and D Salmonella were most frequently isolated, but group C and E Salmonella have been increased in 2002. Antimicrobial resistance of group B and D Salmonella were higher than other serogroups and have been increased year by year.

9.Microcyle Conidiation in Filamentous Fungi.

The typical life cycle of filamentous fungi commonly involves asexual sporulation after vegetative growth in response to environmental factors. The production of asexual spores is critical in the life cycle of most filamentous fungi. Normally, conidia are produced from vegetative hyphae (termed mycelia). However, fungal species subjected to stress conditions exhibit an extremely simplified asexual life cycle, in which the conidia that germinate directly generate further conidia, without forming mycelia. This phenomenon has been termed as microcycle conidiation, and to date has been reported in more than 100 fungal species. In this review, first, we present the morphological properties of fungi during microcycle conidiation, and divide microcycle conidiation into four simple categories, even though fungal species exhibit a wide variety of morphological differences during microcycle conidiogenesis. Second, we describe the factors that influence microcycle conidiation in various fungal species, and present recent genetic studies that have identified the genes responsible for this process. Finally, we discuss the biological meaning and application of microcycle conidiation.

10.Epidemiology of sepsis in Korea: a population-based study of incidence, mortality, cost and risk factors for death in sepsis

OBJECTIVE: To investigate the epidemiology of sepsis in Korea and identify risk factors for death in sepsis.METHODS: We conducted a longitudinal, population-based epidemiological study of sepsis in Korea from 2005 to 2012 using the National Health Insurance Service-National Sample Cohort, a population-based cohort representing 2.2% of the Korean population. The primary objective was to assess the incidence, mortality and cost of sepsis. The secondary objective was to identify the risk factors for death in sepsis. Claim records of admitted adult patients (aged ≥15 years) were analyzed. Sepsis was defined as 1) bacterial or fungal infection or the conditions they often complicate, 2) prescription of intravenous antibiotics, and 3) presence of any organ dysfunction. Comorbidities were defined using the Charlson/Deyo method. Risk factors for 6-month mortality were assessed using multivariable logistic regression.RESULTS: A total of 22,882 cases were identified. Both incidence and 6-month mortality increased from 265.7 (95% confidence interval [CI], 254.7 to 277.1) to 453.1 (95% CI, 439.0 to 467.5) per 100,000 person-years (P-trend <0.001) and from 26.5% (95% CI, 24.4% to 28.8%) to 30.1% (95% CI, 28.4% to 31.9%), respectively. After standardization, the increasing trend of incidence was slower but still significant (P-trend <0.001), while that for mortality was not (P-trend 0.883). The average cost increased by 75.5% (P-trend <0.001). Multivariable logistic regression identified various risk factors for mortality.CONCLUSION: The burden of sepsis in Korea was high and is expected to increase considering the aging population. Proactive measures to curtail this increase should be sought and implemented.
