2.A Case of Multidrug-Resistant Salmonella enterica Serovar Typhi Treated with a Bench to Bedside Approach.

We report a relapsed case of a 25 year-old man with multi-drug resistant Salmonella serovar Typhi (MDRST) bacteremia who had recently returned from travel in India. Due to unresponsiveness to ciprofloxacin and ceftriaxone, we examined the strain's resistance to quinolones and extended-spectrum beta-lactamases (ESBLs). The strain had a single gyrA mutation at codon 83 (Ser83Phe), which explains its decreased susceptibility to fluoroquinolone and resistance to nalidixic acid. In the screening tests of ESBLs, TEM-1 was positive, which is beta-lactamase but not ESBL. The patient was finally successfully treated with meropenem and aztreonam. In the presence of clinical unresponsiveness despite favorable sensitivity tests, further laboratory evaluations are needed, which should include studies of genes related to antibiotic resistance and ESBLs. In addition, further prospective trials should be done about the possible inclusion of antibiotics not yet mentioned in the current guidelines. With MDRST on the rise worldwide, the most optimal and effective line of antibiotic defense needs to be devised.

3.Percutaneous Treatment of Renal Cysts with OK-432 Sclerosis.

PURPOSE: The aim of this study was to demonstrate OK- 432 sclerotherapy efficacy for treatment of simple renal cysts. MATERIALS AND METHODS: Twenty patients with 25 symptomatic or large simple cysts were treated by ultrasonography (US)-guided percutaneous aspiration and injection of OK-432 (8 men and 12 women, mean age 63.6 years, SD 9.5). Six patients presented with flank pain, 14 presented with renal mass; renal cyst location was right, left, or bilateral sided in 9, 8, and 8 kidneys, respectively. Patients were evaluated by clinical assessment, US, or CT scan 3 months following the procedure. Complete and partial success was defined as symptom resolution with either total cyst ablation or greater than 70% reduction, respectively. Failure was defined as 30% of cyst size recurrence and/or persistent symptoms. RESULTS: Average reduction was 93.0%. Complete and partial resolution occurred in 11 (44.0%) and 13 (52.0%) cysts, respectively. One case was defined as failure, with a 64.2% size reduction from 10.9cm to 3.9cm (volume reduction rate 95.4%). Renal pain improved in all patients, regardless of complete or partial resolution. Minor complications occurred in 3 patients, 2 developed leukocytosis and 1 had mild fever (< 38.5 degrees C) following aspiration and sclerotherapy. Successful treatment was achieved with conservative measures and NSAID therapy. CONCLUSION: Percutaneous treatment of simple renal cysts with OK-432 sclerotherapy was found to be a safe, effective and minimally invasive procedure.

4.Bilateral Nephroureterectomy with Radical Cystectomy for Urothelial Tumor Involving the Renal Pelvis, Ureter and Bladder in a Patient Receiving Hemodialysis.

Transitional cell carcinoma such as renal cell carcinoma is the relatively common urinary tract cancer in patients who are on dialysis. A 66-year-old male patient, who had been on maintenance hemodialysis for 5 years, was suffering from gross hematuria. The subsequent image studies revealed multiple masses at the right renal pelvis, the right distal ureter and the trigonal area at the bladder. We performed cystoscopy to evaluate the multiple bladder papillary masses and their blood clots. The patient then underwent bilateral radical nephroureterectomy and radical cystectomy. Histological examination revealed the papillary urothelial carcinoma. Our case may imply that dialysis patients have an increased susceptibility to urological malignancies. Physicians should always raise the possibility of urological malignancy when encountering a dialysis patient with gross hematuria. Because of the high recurrence rate, a more extensive operation and aggressive follow-up protocols should be done for these patients on dialysis.

5.Presacral Abscess with Bilateral Sciatica in a Patient with Crohn's Disease.

Crohn's disease can cause intestinal strictures, perforations, fistulas, or abscesses. Although fistulas and abscesses are common complications of Crohn's disease, a presacral abscess with neuromuscular complications is very unusual. A delayed diagnosis and treatment may increase morbidity. The diagnosis is frequently delayed, because the clinical features of presacral abscess are variable and nonspecific. We experienced a case of a presacral abscess with bilateral sciatica in a 25-year-old male patient. He was diagnosed with Crohn's disease of the terminal ileum 2 months previously and has been managed with mesalazine. He visited our hospital due to pain in the buttocks and severe neuralgia in both thighs. We confirmed enteric fistulas, resulting in a presacral abscess, which extended symmetrically through both sciatic notches and the gluteus medius muscles. He was managed with antibiotics after a small bowel segmentectomy, right hemicolectomy, fistulectomy, and surgical drainage. The bilateral radicular pain resolved completely within 2 weeks of surgery. The patient has remained in remission and asymptomatic 1 year after surgery.

6.Alcohol Drinking, Cigarette Smoking and Risk of Colorectal Cancer in the Korean Multi-center Cancer Cohort.

BACKGROUND: The present study aimed to examine the association between cigarette smoking, alcohol consumption and colorectal cancer risk among Korean adults. METHODS: Data from the Korean Multi-center Cancer Cohort between 1993 and 2005 were analyzed. The study population comprised 18,707 subjects aged older than 20 years old. The subjects were followed until December 31, 2011 (median follow-up of 11.2 years). The Cox proportional hazard model was used to estimate the hazard ratio (HR) and 95% confidence intervals of cigarette smoking and alcohol consumption for colorectal cancer risk. RESULTS: In men, longer duration and higher average amount of alcohol consumption were associated with elevated risk of colorectal cancer (HR 1.93 [1.17-3.18] for > or = 30 years of consumption compared to non-drinkers; HR 2.24 [1.31-3.84] for > or = 30 g/d). Former smokers showed a non-significantly elevated risk of colorectal cancer in men. There was no apparent association between alcohol consumption or cigarette smoking and colorectal cancer risk among women. CONCLUSIONS: Alcohol consumption was associated with increased colorectal cancer risk among Korean men, and both a longer duration and a higher amount of consumption were associated with elevated risk.

7.Expiratory Unidirectional Valve Malfunction Detected by Capnographic Waveform Change: A case report.

Unidirectional valve (UDV) malfunction causes rebreathing of expired gas during anesthesia. However, the resultant hypercarbia without hypoxemia by UDV malfunction is not easily detected. We experienced a case of severe hypercarbia which caused by sticking expiratory valve at 45 minutes after the induction of anesthesia, in spite of checking UDV function preoperatively. In this case, expiratory UDV malfunction was diagnosed with the change of capnographic waveform. And so, we recommend monitoring of capnographic waveform during every anesthesia, in addition to preoperative UDV checking.

8.Brucella Endocarditis with Splenic Abscess: A Report of the First Case Diagnosed in Korea.

Human brucellosis has a broad spectrum of clinical manifestations, which includes endocarditis, a focal complication that is uncommon yet responsible for the majority of associated deaths. The most successful treatment outcomes of Brucella endocarditis have been reported with usage of both antimicrobial agents and surgery. However, there are few reports on the treatment of Brucella endocarditis using antibiotics only. We report the first case in Korea of Brucella endocarditis with aortic valve vegetations and an accompanying splenic abscess, which were treated successfully with antibiotic therapy alone.

9.Lidocaine Pretreatment with Tourniquet Inflation Ameliorate Pain on Injection of Propofol.

BACKGROUND: Venous lidocaine retention with tourniquet has a possibility to prevent propofol injection pain efficiently. We performed the study to assess the efficacy of various intravenous lidocaine pretreatment methods with tourniquet on reducing propofol-induced injection pain, especially the effect of varying the concentration and dose of lidocaine. METHODS: In order to know the effect of lidocaine pretreatment with tourniquet on prevention of propofol-induced injection pain, one hundred patients were divided into four groups by the method of pretreatment; 1% lidocaine of 1 mg/kg (lidocaine pretreatment, LPT1 n = 25); 0.5% lidocaine of 1 mg/kg (LPT2, n = 25); 1% lidocaine of 0.5 mg/kg (LPT3, n = 25); 5 ml of saline pretreatment (saline pretreatment, SPT, n = 25). After 5 minutes of pretreatment, propofol-induced pain was measured immediately after injection of 1 mg/kg propofol with tourniquet inflation and after deflation of tourniquet, and after a second injection of 1 mg/kg propofol by use of the numerical rating scale and pain score of four categories. We selected maximal values of three times measurement for comparison. RESULTS: All groups of lidocaine pretreatment (pain incidence of LPT1; 20%, LPT2; 16% and LPT3; 36%, respectively) significantly reduced the incidence of propofol-induced injection pain compared to the saline pretreatment group (96%) (P <0.05). Lidocaine pretreatment groups had dramatically lower intensity of pain compared with saline pretreatment (P <0.05). However, there were no differences among the lidocaine pretreatment groups (P > 0.05). CONCLUSIONS: This result indicates that lidocaine pretreatment with tourniquet has an effect on the prevention of propofol-induced injection pain. However, we recommend pretreatment with 0.5 1% lidocaine of 1 mg/kg by use of tourniquet and propofol injection immediately after deflation of the tourniquet in practice.

10.Effects of Dexamethasone on RANTES Expression of Nasal Fibroblast.

BACKGROUND AND OBJECTIVES: Fibroblasts play an indirectly augmenting effector role in allergic inflammatory response by releasing different proinflammatory cytokines, including RANTES, GM-CSF, IL-8 after stimulation by other inflammatory cytokines such as IFN-gamma, TNF-alpha. The aim of this study was to investigate expression of RANTES in allergic and non-allergic nasal fibroblasts after stimulation with IFN-gamma and TNF-alpha, and to study the effect of dexamethasone on the RANTES expression of nasal fibroblast cell. MATERIALS AND METHODS: Using the 3rd passage of fibroblasts taken from the inferior turbinates of allergic and non-allergic patients, we evaluated the RANTES expression of fibroblasts after the IFN-gamma, TNF-alpha stimulation in the presence or in the absence of dexamethasone by ELISA. RESULTS: The expression of RANTES in allergic nasal fibroblasts stimulated by cytokines was stronger than in non-allergic nasal fibroblasts stimulated by cytokines. And dexamethasone suppressed the RANTES expression in allergic nasal fibroblasts stimulated by IFN-gamma. However, dexamethasone did not affect the RANTES expression in allergic fibroblasts stimulated by TNF-alpha and non-allergic fibroblasts stimulated by IFN-gamma and TNF-alpha. CONCLUSION: This study shows different responses of the RANTES production in nasal fibroblasts to dexamethasone, perhaps reflecting heterogeneity of nasal fibroblasts.
