Exploring the epidemiological trend of HIV/AIDS is required for making the national AIDS policy. In this study, the trend of HIV/AIDS incidence, rout of transmission and some characteristics of AIDS for the past 11 years in Korea using the reported cases from the national STD screening scheme were reviewed. Based on the results, the trend of main route of transmission according to the year was established by stage and the HIV/AIDS cases in this year was estimated and that to the year 2000 was projected by 'Epimodel' programme. The results were as follows. 1. Of the total infected persons, 76% were in their twenties and thirties, socioeconomically and sexually active age groups while the transmission by sexual contact overseas was decreasing, the infection through domestic heterosexual and homosexual contact was increasing. 2. In the middle of the 1980's, the infected persons were mainly prostitutes infected through heterosexual contact with the HIV positive foreigner in this county(stage l). And in the late of the 1980's the main source of infection was the sexual contact overseas and the domestic heterosexual contact(stage 2). Since the early of the 1990's, the infection through the heterosexual contact with non-regular sexual partner in this country has increased rapidly(stage 3), which was the evidence of the possibility of HIV epidemics. After that, it was expected that the infection through the homosexual contacts, the heterosexual contacts with commercial sex workers outside and the non-regular sexual contact inside of this county would increase continuously. In the result, the occurrence of neonatal infection by vertical transmission was expected(stage 4). 3. The number of HIV/AIDS was estimated at 572 to 2,313 and the projected number of HIV/AlDS to the year 2000 was around 5,800 including 627 AlDS patients. For the further study on the estimation and projection of HIV/AIDS, it was suggested that the sampling survey on the HIV infection rate in the high risk groups and the sentinel hospital surveillance system should be conducted.

PURPOSE: The purpose of this study was to examine the health behaviors (such as smoking, alcohol consumption, exercise, duration of sleep, body weight, eating habit of breakfast and snack) affecting self-perceived health status, and to determine the relationships. RESEARCH METHODS: This study utilized the data from Korean National Health Survey (KNHS) in 1992. The data consisted of random sample of 2,799 individuals (1,304 male and 1,495 females) whose ages were between 20 and 59. The analytic methods for the study were chi square analysis, logistic regression. RESULTS: The results of the study were as follows : (1) There was a negative relationship between duration of smoking and health status in linear pattern; (2) The frequency of alcohol drinking had a significant relationship with health status in quadratic pattern. That is, the health status of moderate drinkers was better than that of non-drinkers and heavy drinkers; (3) There was no significant relationship between the frequency of exercise and the health status. But in further analysis, the relationship between them was positive in linear pattern. That is, the health status of daily exercisers was better than that of the others; (4) Duration of sleep significantly influenced the health status in quadratic pattern. That is, the persons who sleep for 7-8 hours per day were better than that of the others; (5) The health status of persons with normal body weight was higher than that of persons with underweight or overweight; (6) Eating breakfast or not did not influence the health status, and taking snack or not influenced the health status; (7) In mutivariate analysis, The younger the persons were and the longer the level of education was, the higher the health status was in both male and female. In female, the higher the economic status was, the higher the health status was. And the smoking and the weight status significantly and independently influenced the health status in both male and female. And in male, the moderate drinking positively influenced the health status. CONCLUSION: History of no smoking, moderate alcohol consumption, regular exercise, 7-8 hours sleeping and maintaining the normal body weight were health-promotive behaviors. And especially no smoking, moderate drinking and maintaing the normal body weight singnificantly influenced the self perceived health status.

This report is a part of the health survey, which was carried out in the summer of 1973 on all adults older than 15 years of age, inhabiting in the sampled area. Thirteen villages out of 31 natural villages in Shindong Myon, Chunseong Gun, Kangwon Province, a typical rural area of Korea were randomly sampled. A little over 70 percent of the population sampled, or 1198 persons were examined. The health survey included laboratory screening tests, history taking, physical examination and consultation to specialists for selected cases. Diseases of circulatory system were classified by WHO ICD A-List (150). Hypertensive heart disease was defined by the criteria that was used in U.S. Health Survey, and hypertension by WHO criteria for the purpose of comparison. Results obtained were as followings: 1. The overall prevalence of the diseases of circulatory system per 100 adults examined was 8.6 for male and 9.9 for female. 2. Age specific prevalence of the diseases showed gradually increasing pattern as age advanced for both sexes. For example, the prevalence for 15~19 years old age group was 2.4/100 adults which increased to 24.8 when age reached over 60 years. 3. The most prevalent disease category, 3.6/100 adults, was A-88: other diseases of circulatory system that was consisted mainly of varicose vein and hemorrhoids; the second one was hypetensive heart disease, 2.4. 4. Female had higher prevalence than male in A-84: other form of heart diseases (4 times, arrhythmia, beriberi heart disease, and heart disease secondary to anemia), in chronic rheumatic heart disease (4.5 times), and in hypertensive heart disease (1.4 times); in other hand, cerebrovascular diseases (4.5 times) and diseases of arterioles and capillaries (1.5 times) occurred more frequently among males than females. 5. The proportion of hypertensive heart disease among persons with definite hypertension was 22.0% for male, 25.8% for female, and it increased gradually as age increased. Hypertensive heart disease among persons with borderline hypertension was 2.2%. Only 27% of hypertensive heart disease patients had previous treatment for hypertension. 6. The prevalence of definite hypertension per 100 adults examined (double blood pressure check with interval of 30~40 minutes) was 9.5 for male, 9.4 for female, with increasing pattern by advancing ages; marked increase occurred after age of 45 years for male and 35 years for female. The prevalence of hypertension in this study was much lower than that of the study on employees of Korea Electricity Comparny when compared with the same age groups. 7. Electrocardiographic findings on 105 heart disease suspects revealed left ventricular hyperspecific change in 4 persons. This study was supported by China Medical Board of New York Inc.

BACKGROUND: Although hypoxic pulmonary vasoconstriction (HPC) and hypoxic coronary vasodilatation (HCD) have been recognized by many researchers, the precise mechanism remains unknown. As isolated arteries will constrict or relax in vitro in response to hypoxia, the oxygen sensor/transduction mechanism must reside in the arterial smooth muscle, the endothelium, or both. Unfortunately, much of the current evidence is conflicting, especially concerning to the dependency of HPC and HCD on the endothelium and the role of the K+ channel. Therefore, this experiment was attempted to clarify the dependency of HPC and HCD on the endothelium and the role of the K+ channel on HPC and HCD. METHODS: HPC was investigated in isolated main pulmonary arteries precontracted with norepinephrine (NE). HCD was investigated in isolated left circumflex coronary artery precontracted with prostaglandin F2 alpha. Vascular rings were suspended for isometric tension recording in an organ chamber filled with Krebs-Henseleit solution. Hypoxia was induced by gassing the chamber with 95% N2 +5% CO2, which was maintained for 15 - 25 min. RESULTS: 1)Hypoxia elicited a vasoconstriction in NE-precontracted pulmonary arteries with endothelium, but a vasodilatation in PGF 2 alpha-precontracted coronary arteries with and without endothelium. There was no difference between the amplitude of the HPC and HCD induced by two consecutive hypoxic challenges and the effect of normoxic and hyperoxic control Krebs-Henseleit solution on subsequent response to hypoxia. 2)Inhibition of NO synthesis by the treatment with Nw-nitro-L-arginine reduced HPC in pulmonary arteries, but inhibition of the cyclooxygenase pathway by treatment with indomethacin had no effect on HPC and HCD, respectively. 3)Blockades of the TEA-sensitive K+ channel abolished HPC and HCD. 4)Apamin, a small conductance Ca2+/-activated K+ (KCa) channel blocker, and iberiotoxin, a large conductance KCa channel blocker, had no effect on the HCD. 5)Glibenclamide, an ATP-sensitive K+ (KATP) channel blocker, reduced HCD. 6)Cromakalim, an K(ATP) channel opener, relaxed the coronary artery precontracted with prostaglandin F2 alpha. The degree of relaxation by cromakalim was similar to that by hypoxia and glibenclamide reduced both hypoxia- and cromakalim-induced vasodilations. 7)Verapamil, a Ca2+ entry blocker, caffeine, a Ca2+ emptying drug; and ryanodine, an inhibitor of Ca2+ release from SR, reduced HPC, respectively. CONCLUSION: HPC is dependent on the endothelium and is considered to be induced by inhibition of the mechanisms of NO-dependent vasodilation while HCD is independent of the endothelium and is considered to be induced by activation of the K(ATP) channel.

The purpose of this study was to explore and describe the lived experience of the family caregivers with the demented elderly. For investigate purposes of this study, data collection was done from May 9th to October 16th 1997, by means of in-depth interviews with 6 individuals in caregiving families. The research question was "What are the lived experiences of being a family caregiver with the demented elderly? "All interviews were tape-recorded and transcribed for the analysis using Colaizzi's method. The main results of this study were as follows : 1) Family caregivers ascribed the cause of the dementia to 'environmental change' and the 'introverted personality of the elderly'. 2) Family caregivers let the demented elderly alone initially and then they restrained the elderly who was in a fit of dementia. 3) The coping response of family caregivers varied. The coping reponses were (1) having their moods go up and down, (2) enduring and praying, (3) avoiding the elderly, (4) hoping for release from responsibility, (5) enduring their conditions(obligations), (6) accepting the elderly, (7) taking an objective view, and (8) taking safety measures. 4) The long ordeal of coping with a demented elderly person resulted in the (1) loss of physical and psychological well-being, (2) ethical conflicts, (3) family conflicts, (4) become desperate, (5) rejection of the aging process, (6) sympathy and understanding for the elderly, (7) awareness that the support system is important and (8) hope of sharing their responsibility. The results of this study may help nurses to understand the lived experiences of the family caregivers with the demented elderly better, in order to provide more basic data for the development of educational programs for dementia family caregivers. It may help to make the coping process easier and more successful for the family members of the demented elderly.

The stability of the Knowles pinning in the intertrochanteric fractures of the femur was studied to determine which state would best support the load and compared with that of others. Experimental stable and unstable four-part fractures of the intertrochanteric region were produced by manual straight cut in 20 embalmed adult femora. These fracture were stabilized using accurate or inaccurate reduction combined with implant of Knowles pin parallel to calcar femorale or femoral neck. Those specimen were examined for the maximum load supported with the Universal Testing Machine. The results obtained are as follows: 1. Those femora of stable fracture with accurate reduction and Knowles pinning parallel to calcar femorale were failed by load of 140±13kg. 2. Those femora of stable fracture with accurate reduction and Knowles pinning parallel to femoral neck were failed by load of 120±11 kg. 3. Those femora of stable fracture with minimal displacement and Knowles pinning parallel to calcar femorale were failed by load of 110±10.4kg. 4. Those femora of unstable four-part fracture with accurate reduction and Knowles pinning parallel to calcar femorale were failed by 80±9kg.

The Enchondroma is a benign cartilage forming harmartoma arising chiefly from short tubular bones on hands and feet. If on a long tubular bone or flat bone such as femur or ilium, it is most likely to be a true neoplasm such as chondrosarcoma, not a harmartoma. A case of enchondroma was reported. It occurred over right ischium of 19 years old male and was found incidentally by roentgenogram. The lesion was biopsied and diagnos's was confirmed by histologic examination. It was treated by curretage with filling of cavity with bone chips. Post operative-course was uneventful.
