1.Immunohistochemical Study for Ki-1 and EMA Antigens in Large Cell Lymphoma including Anaplastic Large Cell Lymphoma.

To evaluate the frequency of EM A and Ki-I antigen expression in the large cell lymphoma and to define the histologic characteristics of Ki-1 positive anaplastic large cell lymphoma, 40 cases of malignant lymphoma, diffuse large cell type were immunostained by Ki-I and EMA monoclonal antibodies. Eight cases of large cell lymphomas expressed EMA, among which 4 cases were positive for Ki-I antibody as well. The positive rate for EMA was much higher in T cell lymphomas than in B cell lymphomas. Among 4 cases of Ki-I positive lymphomas, 2 cases showing membrane staining of Ki-1 with prototypic histologic feature of anaplastic large cell lymphoma were classified as Ki-1 positive anaplastic large cell lymphoma(ALCL). Ki-I positive ALCL were T-cell in one and non-T, non-B cell type in the other, respectively. The remaining 2 cases of Ki-1 positive lymphomas showing cytoplasmic staining were classified as both B-cell centroblastic/centrocytic lymphoma and T-cell pleomorphic large cell lymphoma.
2.Phenobarbital dosage and blood level for control of neonatal seizure.

Twenty-five neonates were observed for the relationship between phenobrbital loading dose, plasma concentrations and control of seizures. Nineteen neonates (76%) had seizures controlled by phenobarbital alone. Effective plasma concentration were 10.0~31.1microgram/ml. Although 14 neonates had seizures controlled by phenobarbital concentration below 20microgram/ml, neonate required level above 30microgram/ml. We found that continued benefit was obtained until phenobarbital plasma concentrations reached 40microgram/ml and there was no adverse effects on cardiorespiratory function. We recommend that plasma phenobarbital concentrations should equal or surpass 40microgram/ml before additional anticonvulsants are used for neonates with seizure.

3.A Study on Stroke Patients' ADL, Depression, Self-Efficacy and Quality of Life.

This study was a descriptive research in search of a nursing intervention scheme to improve stroke patients' quality of life by understanding the relationship of stroke patients' ADL. depression, self-efficacy and quality of life with various variables and identifying factors that affect their quality of life. Each subject was interviewed one to one based on a structured questionnaire. With regard to research tools, Modified Barthel Index (MBI: Fortinsky et al., 1981), which was translated by Choi, Hye-sook (996), was used in measuring ADL, Randloff's (1977) tool. which was translated by Choi, Soon-hee, was used in measuring depression, and the tool developed by Sherer et al. (1982), which is to measures self-efficacy under general conditions not limited to specific conditions, and modified by O, Bok-ja (1994) was used in measuring self-efficacy. The quality of life was measured using the scale of satisfaction of life developed by Diener et al.(1985). The results of this study were as follows: 1. The means of ADL of the subjects was 79.5+/-1.9, depression 26.8+/-0.4. self-efficacy 47.1+/-5.7, and the quality of life 12.3+/-.9. 2. The subjects' quality of life showed a statistically significant difference according to gender (t=7.9, p= .006), satisfaction with income (F= 5.8, p = .004), the burden of medical fee (F=3.7, p= .028) and the period of disease (F=2.8. p= .042). 3. With regard to relationship among ADL, depression, self-efficacy and the quality of life, ADL was in a relatively low positive correlation (r= .293, p= .003) with and the quality of life, depression in a high negative correlation (r=- .634. p= .000) with the quality of life. and self-efficacy in a positive correlation with the quality of life (r= .388. p= .000). 4. Factors that made a significant influence on the quality of life were depression (8=- .309. p= .001) and satisfaction with income (8=-2.611. p= .001). Based on these results, this study made following suggestions: 1. It is necessary to run rehabilitation programs to improve stroke patients' ADL, depression and self-efficacy. 2. It is necessary to perform research of monitoring stroke patients' quality of life in various areas using measuring tools.

4.Sonographic changes of the gallbladder wall in cholecystitis: a sonographic-pathological correlation

To assess the pathological basis of the sonographic changes of the gallbladder wall in cholecystitis, thesonographic appearances of the gallbladder wall were analysed in 17 patients with acute cholecystitis and 27patients with chronic cholecystitis, and correlated with pathological specimens removed at surgery. In acutecholecystitis, a thin sonolucent layer within the echogenic gallbladder wall corresponds to subserosal edema,hemorrhage and inflammatory cell infiltration: in chronic cholecystitis it corresponds to subserosal edema,hemorrhage and inflammatory cell infiltration: in chronic cholelcystitis it corresponds to subserosal edema,hemorrhage and inflammatory cell infiltration, in chronic cholelcystitis it corresponds to muscular hypertrophy.Indistinctness and/or a low echogenicity rind along the inner margin reflects mucosal sloughing or obliteration ofthe mucosal folds. Uniformly decreased echogenicity of the wall is caused by severe inflammatory cell infiltrationwith sloughing of the mucosa or obliteration of the mucosal folds. These sonographic singns are considered to bevaluable sings of cholecystitis.

6.Radionuclide scintigraphy of the scrotum

Radionuclide scrotal scintigraphy with 99m Tc-pertechnetate is an easy, well established, sueful and readilyavailable technique for evaluation of acute scrotum. We studied 41 cases of radionuclide scrotal scintigraphy andthe results were as follows; 1. The over all diagnostic accuracy of scrotal scintigraphy waas 93%(38/41 cases). 2.Scrotal scintigraphy was very useful and accurate in differential diagnosis of epididymo-orchitis and testiculartorsion in patient with acute scrotal pain and swelling, while there was some limitation in differential diagnosisof hematoma from acute epididymo-orchitis or torsion. 3. scintigraphy of epididymoorchitis showed increasedperfusion and radiocativity in the epididymis and/or testis and its diagnostic accuracy was 90%(19/21 cases) 4.Acute testicular torsion showed normal flow in perfusion and cold defect occupying affected testis in staticimage, while missed torsion showed slightly increased flow in perfusion image and cold defect surrounded by anuniform rim of hyperactivity (halo sign). Diagnostic accuracy of testicular torsion was 86%(6/7 cases).

8.Computed tomographic measurement of splenic size in normal Korean adults

Authors analyzed 72 cases of abdominal CT of Korean adults who didn't have any medical reasons to believe thespleen was abnormal. The following criteria were measured with mulitiple transverse scanning of the entire lengthof spleen (height, breadth, thickness) & relationship with fixed midline structure, the spine( the shortestdistance from midline to medial edge of spleen, the longest distance from anterior margin of vertebral body toanterior tip of spleen). The reults were as follows; 1. The average size in adult was 8.0±1.5cm in height,8.6±1.2cm in breadth and 3.4±0.6cm in thickness; in adult female, 7.8±1.1cm, 8.4±1.0cm and 3.4±0.6cm,respectively; total average, 7.9±1.3cm, 8.5±1.1cm and 3.4±0.6cm, respectively. No remarkable difference wasnoted between both sexes and age groups. 2. The shortest distance from midline to medial edge of spleen was4.1±1.1.cm in male, 3.6±1.0cm in female and total average of 3.9±1.1cm. There was remarkable difference betweenboth sexes(p<0.005) but not between age groups. 3. The longest distance from anterior margin of vertebral body toanterior edge of spleen was 2.3±1.7cm in male, 2.0±1.4cm in female and total average of 2.2±1.6cm. Noremarkable difference was seen between both sexes and age groups.

9.Transabdominal ultrasonographic measurement of prostate size and weight in the normal Korean adults

Author analyzed 154 cases of transabdominal ultrasonography of normal adults who had normal urinalysis, no complaint of symptoms related with urinary system and no evidence of prostate disease. The maximum anteroposterior, transverse and longitudinal diameters of prostate were measured. The weight of prostate was computed using the formular for the weight of ellipsoid object (W=SGx4/3 pie r1.r2.r3, w; weight, SG; specificgravity about 1.05 gm/ml, r; radious of prostate in any dimension). The results were as follows; 1. The averagesized of prostate were 4.12±0.68 cm in transverse diameter, 3.46±0.49 cm in longitudinal diameter and 2.36±0.40cm in AP diameter. Transeverse diameter was the longest and AP diameter was the shorteset in all cases. 2. Theaverage weight of prostate in each age groups were 19.25±6.78 gm in the 3rd decade, 16.64±5.18 gm in the 4th decade, 16.28±5.31 gm in the 5th decade, 22.31±6.95gm in the 6th decade and 13.06±8.18 gm in over the 7th decade. The average weight of prostate in groups more than 50 years of age were significantly greater than youngerage groups (p<0.005). Total average weight was 18.96±6.78 gm. 3. The distribution of prostate weighting over 30gm in each groups were 14.29%(5/35) in the 3rd decade, 0%(0/34) in the 4th decade, 2.63%(1/38) in the 5th decade,20%(6/30) in the 6th decade and 23.5%(4/17) in the 7th decade.

Stomach carcinoma is more common disease in korea than western contries. The reported ultrasonographicfindings of gastric carcinoma were thickening of gastric wall and “pseudokidney” sign. The auther analizedultrasonographic findings of 101 cases with gastric carcinoma who were performed ultrasonography and gastroscopyat Kyung Hee Universtiy Hospital from Oct. 1982 to Oct. 1985. The results were as followings; 1. Types of gastriccarcinoma were consisted with infiltrative type 68 cases, infiltrative type with ulceration 16 cases, polypoidtype with ulceration 1 cases, infiltrative and polypoid type 4 cases, linities plastica type 3 cases, lcerativetype 1 case and polypoid type 1 case. 2. Extent of the lesions were in body and antrum 45 cases, entire stomach 18cases, antrum 18 cases, body 12 cases, body and fundus 6 cases. 3. Ultrasonography was useful in demonstrating theextent of the tumor and the presence of materials elsewhere in abdoment.
