1.A Study of Trauma to the Epiphyseal Plate in Immature Rabbits

The important role of the epiphyseal cartilage in the longitudial growth of long bones is universally accepted. Injury to the epiphyseal plate caused by surgically produced defects or trauma has been observed clinically and in experimental investigations. These growth retarding effect of defects seem to be proportional to the size of the defect, and are caused by the formation of a bone bridge connecting the epiphysis to the metaphysis. This investigation provides an explanation for the intimate relationship between the size of the defect and growth retarding effect. Central destruction of the distal femoral epiphyseal plates on 81 immature rabbits were performed by four different sized drills. Free flaps of vastus lateralis were inserted into the defect of the epiphyseal plate. Left limbs were used as the experimental group, and right limbs as the control. After 8 weeks of operation, animals were sacrificed, and roentgenological and histological observations were made. The results were observed as follows. 1. The amount of bony bridge connecting epiphysis and metaphysis was proportional to the size of the epiphyseal defect. 2. The amount of growth retardation at the distal femoral epiphyseal plate was proportional to the size of the defect produced in the epiphyseal plate. 3. Compensatory overgrowth at the proximal femoral epiphysis seems to be proportional to the size of the defect made in the epiphyseal plate. 4. Insertion of the free flap of vastus laterials into the epiphyseal defect diminished the formation of the bony bridge.

2.Purine Nucleoside Phosphorylase Activities in Sera and Lymphocytes of Peripheral Blood from Patients with Allergic Contact Dermatitis and Drug Eruption.

The present study was designed to measure the activity of purine nucleosiae phosphorylase(PNPase) in sera and lymphocytes af peripheral blood from patients with allergie contact dermstitis and drug eruption since PNPase activities are known to be decreased in cell-mediated immune deficieney diseases. The PNPase activities in sera and lymphocytes of normal subjects were (7.2 +1.05) * 105 unit/L of sera and (1.85+0.38) unit/102 lymphocytes, respectively. In allergic contact dermatitis, the PNPase activities in sera and lymphocytes of patients were (3.9+0.78) *105 unit/L of aera and (0.69+0.23) uoit/102 lymphocyteis, which were signifieantly lower than those of normal subjects. There were no differences in PNPase activities of sera and lymphocytes between drug eruption patients and normal subjects. From these results, it is suggested that the lowered PNPase aetivity in allergie contact dermatitis might be associated with abnormal lymphocytes differentiation or activation or some other unknown mechanism, since lowered PNPase activity in allergic contact dermatitis is in contrast to the generally accepted concept that enhanced status of CMI in ACD will lead to the increase in PWPase activity.

3.Ecology of filariasis on Che Ju Island.

Study of filariasis to determine important factors involved in its ecology was carried out on Che Ju Island for three consecutive years from 1968 to 1970 in seven villages, three coastal villages and four islets remote from the main island. One village which was located in mountainous area far from the coast was surveyed to serve as control area. About 90% of population inhabiting the study area had at least one blood smear during the three-year period; about one third had three blood smears, and a little over one third had two, and the rest only one examination. Animal and mosquito surveys were carried out at the same period. Followings are the results obtained: All human cases but several had microfilariae identical to the description of B. malayi. The several cases who had morphologically different microfilariae from that of B. malayi need further study for definite conclusion. Five persons randomly sampled from Mf positives and bled every two hours demonstrated nocturnal periodicity between 9 p.m. and 3 a.m. Human is considered to be only reservoir host for human filariasis in the area since animal survey and experimental exposure to the infective larvae of human filaria species showed failure to infect animals. Microfilaria rate, microfilaria density, prevalence of elephantiasis varied by area and age with correlation, which indicated cumulative process of the parasite by repeated exposure and development of host immunity to certain extent. Clinical manifestation of filariasis (symptom complex and elephantiasis ) taken from history and inspection was low in its prevalence with range of 0.9% 11.8% of total population. Only 5.2% of 517 Mf positives had the clinical manifestation. 24.8% of 109 persons with clinical manifestation had microfilaria; 42.9% with symptom complex only, 23.1% with both symptoms and elephantiasis, and none with elephantiasis only were microfilaria positive. Ae. togoi was the only species infected with the filaria. Mosquito infection rate by area showed positive correlation to the Mf rate and density of human population; where the Mf rate and density were high, the mosquito infection rate also high.

4.Fixed Sporotrichosis on Nasal Bridge with Bilateral Lymphatic Involvement.

We experienced 53-year-old male patient, who had an eczematous verrucous plaque measuring 6 x 4cm in diameter on the nasal bridge for 4 months. On physical examination, there was no abnormality except of palpable bean sized nodule on the left side of chin for 2 months. The overlying skin of the nodule is a quite normal appearance. The skin biopsy of the nodule showed the charact.eristic a.rrangement of the infiltrate in three zones. And the result of fungus culture of the thin tissue slice on Sabouraud media diagnosed fungal infection due to Sporotherix .schenkii. The treatment began with oral ketoconazole for 4 weeks, and the medication was interrupted for 2 weeks, because of no favorable effectiveness. On his revisit, a chain of painless nodules along lymphatics palpated on the right side of his f ace. We changed the therapy into saturated solution of potassium iodide(KI) with 0.25ml(5 drops) orally there times a day and was gradually increased administration of KI, And no side effects were noted. The patient was treated for a. total of 18 weeks, which included 6 weeks of treatment following clinical resolution.

5.Discoid Semilunar Cartilage

Discoid semilunar cartilage is an abnormality in which the meniscus is discoid rather than semilunar, and first reported by Young of Glasgow in 1889. 13 cases of discoid semilunar cartilages which were experienced at the Department of Orthopedic Surgery, College of medicine, Seoul National University from Jan. 1956 till Aug. 1972 are reported with review of literature. 1) The incidence was remarkable higher in the male than in the female as 11 versus 2, and higher in the age group of second decade (Average 16.4 year old). 2) Most common site was lateral meniscus, and bilateral discoid meniscus were 3 cases. 3) 3 cases out of 13 cases had complicated rupture of semilunar cartilage, and one case had cyst of the meniscus. 4) Meniscectomy were performed in all cases with successful outcome.

6.Triple Osteotomy of the Innominate Bone: Experlence with an adult Paralytie Hip

In the treatment of dislocations and subluxations of the hip in the older children, whether congenital or paralytic, orthopedic surgeons are blessed with a wide variety of commonly used surgical procedures; namely, Salters innominate osteotomy, acetabtloplasties, Pembertons pericapsular osteotomy, shelf operations, Chiaris pelvic displacement osteotomy. and Colonnas capsular arthroplasty. However, with increasing age and soft tissue contractures,these procedures become ineffective, leaving a wide range of age between older children and young adults subject to uncertain or unfavarable prognosis. Steels triple osteotomy (1973) is aimed at coverig this age group, when displacement or in nominate osteotomy is either technically infeasible or likely to fail. It consists of an open reduction with or without soft tissue release and skeletal traction, redirection of the acetabulum to cover the femoral head by osteotonmies of the pelvis that has lost its young cartilagenous resiliency, and preservation and physiological remodelling of the articular cartilage of the acetabulum. We performed Steels osteotomy on a 21-years-old female with a severe paralytic subluxation of the hip associated with pelvic obliquity and paralytized both lower extremities. One and a half year follow-up result was satisfactory with a stable and congruous joint despite paralysis and with the patient walking for the first time in her life.

9.A Case of Fibroepithelioma.

We present a case of fibroepithelioma in a 67 year-old male patient. He was admitted e,t Dey4. of Orthopedic Gurgery due to the trochanteric fracture of the femur. And he was eonsulted to our Department for evaluating the deformity of the left foot. Once he had worked as a miner, he have been suffered from multiple injuries of the left f t, The leeions beeame purulent discharge and crust. He found verrucous lesions at the site. On his visit, we found multiple dark brown verrucoua papules at dorsum of the left foot, d we psrformed the skin biopsy of the lesions. Histopathologic findinga showed a proliferation of long, brsnching and anastomosing skin of loid cells embedded within an edematous fibrous stroma.

10.Clinical and mycological studies on dermatomycosis.

We performed clinical and mycological studies on 1715 cases of dermatomycosis in Chonnam University Hospital for 3 years, from January 1988 to Dccember 1990. The resuts are sumerized as follows . 1. The incidence of dermatomycosis was 13.28% out of a t.otal of 12915 out-patients. 2. Among the age groups, the incidence rate was highest in the third deacde(22.6%). 3. The ratio of male to female patients was 1.4:1(1014:702). 4. The seasonal prevalence of dermatomycosis was highest in t,tu. simmer. 5. The incidence of dermatomycosis was Tinea pedis(29.7%), Tinea versicolor(19.8%), Tinea cruris(11.8% ), Candidiasis(8.6% ), Tinea corporis(8.3% '), Onychomycosis(7.8%), Tinea manus(7.0%)Tinea capit,is(3.6%), Tinea faciei(3.1%) and Sporotrichosis(0.4%) in decreasing order of frequency. 6. Coexisting fungal infections were found in 438 pat.ient,s(25.5% ), and the cases of Tinea manus wit.h Tinea pedis was the most common. 7. In dermatophytosis, a positive rate for the KOH mount examination was 52.4%, and a positive rate of culture on ordinary Saborauds dextrose agar media was 41.4%. 8. Trichophyton rudrum was the most, common causative orgariisn of dermatophytosis(45.9%) with Candida albicans(23.2%), Trichophyton mentagrciphtes(17.1%), Microsporum canis(8.9%), Epidermophyton floccosum(2.5%), Trichophton ferrugineum(1.2%), Microsporum gypseum(0.6% ), Trichophyton violaceum(0.4%), presenting in decreasing order of frequency.
