3.Clinical trial of leptospires vaccine on its immunogenicity and safety.

Since the reservior of leptospires organism is consisted of a broad spectrum of animals, the best method of prevention is vaccination. The clinical trial of leptospires vaccine conducted on human volunteer for its immunogenicity and safety. Summarized results are as follows: 1. The Oral temperature among vaccinated group ranged from 36.7 +/- 0.34 degrees C, while in placebo injected group it ranged from 36.4 +/- 0.46 degrees C to 36.7 +/- 0.53 degrees C. There wan no association between vaccination and fever (p<0.05). 2. Mild local reactions revealed in vacciness were swelling (50-75%), Redness(75-90%), and induration(25-40%). Placebo injected group revealed only redness in 12.5% in the 1st injection and 37.5% in the second injection. The duration local reactions on injection site for both vaccinees and placebo groups disappeared within 48 hours. 3. Generalized Symptoms complained by the vaccinees were myalgia (25%), back pain(15%), headache(15%), pruritus(15%), and abdominal pain(10%), whereas placebo group complained of headache(25%), myalgia(12.5%), back pain(12.5%), pain in eyes(12.5%), abdominal pain(12.5%) prutitus(12.5%) and nausea(12.5%). 4. The serological test(MAT) of vaccinees showed geometric mean antibody titer as follows: a. L. icterohemorrhagiae lai, 1 week after 1st vaccination: 22.45, 1 week after 2nd vaccination: 111.23, 3 week after 2nd vaccination: 266.64, b. L. canicola canicola, 1 week after 1st vaccination: 24.62, 1 week after 2nd vaccination: 123.92, 3 week after 2nd vaccination: 276.55, c. L. icterohemorrhagiae copenhageni, 1 week after 1st vaccination: 28.28, 1 week after 2nd vaccination: 128.55, 3 week after 2nd vaccinaton: 247.88, Whereas all of the placebo injected group, showed below 1 : 20 titers. The sero-conversion rate of vaccinees were 100 percent.

4.Seropositive Rate to Yersinia enterocolitica - pseudotuberculosis complex among Patients with Acute Gastroenteritis in Children , 1994-1997.

To investigate incidence of acute gastroenteritis (AGE) caused by Yersinia enterocolitica and Y. pseudotuberculosis (referred to as Y. enterocolitica-pseudotuberculosis complex) in children, seropositive rates to major 8 serotypes of Y. enterocolitica-pseudotuberculosis complex antigens among 467 sera of three groups of subjects (298 acute gastroenteritis; AGE, 108 miscellaneous pediatric diseases; MPD, and 61 healthy medical students; HMS) were investigated by bacterial agglutination test using standardized, heat-killed, phenolized, smooth 0 antigens. In addition, cross-reactions between yersinia agglutinins and Widal agglutinins in patients with AGE caused by Y. enterocolitica-pseudotuberculosis complex were examined in paired serum specimens. Seropositive rates to Y. enterocolitica-pseudotuberculosis complex among three groups, AGE, MPD, and HMS, were 36.5%, 13.8%, and 14.7%, respectively. Of 109 seropositive AGE patients, the most common type-specific agglutinin which showed predominating agglutinin titer to one of 8 serotypes of Y. enterocolitica-pseudotuberculosis complex antigens was PO4b (27.5%), followed by PO2a (23.8%), EO3 (16.5%), PO5a (13.7%), PO5b (9.1%), PO3 (4.5%), EO9 (2.7%), and POlb (1.8%), in orders. The Widal agglutinins cross-reacted to Y. pseudotuberculosis PO4b antigen, but the yersinia agglutinins were differentiated from Widal agglutinins by rising agglutinin titers in paired serum specimens taken one to three weeks apart. In conclusion, acute gastroenteritis caused by Y. pseudotuberculosis in children must be regarded as a fairly common enteric disease in children.

9.Ilizarov Method for Knee Arthrodesis in Septic Knee Joint.

Although joint infection with severe destruction remains a serious problem and severe bone loss is relative contraindication of arthrodesis, knee arthrodesis is most commonly a salvage procedure. Its goals are to relieve pain and restore the patient to functional level of activity. The purpose of this study is to evaluate the results and complications of Ilizarov method for knee arthrodesis in the presence of infection. We retrospectively reviewed the records of twelve patients who had managed with knee arthrodesis with Ilizarov method. Indications for the operation included a infected skeletal defect secondary severe open trauma in four patients, an infection at the site of an arthroplasty in three (with failure of previous arthrodesis with monofixator in one), an infected charcot joint in four and one pyogenic arthritis spreading from osteomyelitis of proximal tibia. The average age of the patients at the time of operation was fifty-three years (range twenty-two to eighty years). Follow-up averaged 17 months. The minimum follow up periods was 9 months. Average duration of Ilizarov fixator application was 7.2 months. In 3 cases we performed corticotomy and internal bone transport to treat large bone defect (17cm, 8cm, 6.5cm). Autoiliac bone graft was done in fusion site in two cases, and in three cases we performed bone graft at the docking site. Solid osseous union occured in each patient. There was no reinfection and nonunion. Average shortening was 3.4 cm. Complications were related to pin tract. When last seen, all patients were free of pain and could walk without cruthes or cane. Despite its pin tract problems, bulky cumbersome and expensive apparatus, the Ilizarov method is one of effective method for knee arthrodesis in the presence of infection and large bone loss especially.

10.Unusual manifestation of right upper lober collapse due to bronchogenic carcinoma

In the unusual manifestation of right upper lobe collapse confusing with mediastinal or parenchymal mass, both alteration of the pulmonary vessels and shifting pattern of the collapsed lobe to the periphery on supine positionare the key to the diagnosis of it rather than mediastinal or parenchymal mass. The mechanisms of these unusual manifestation s are obscure, however lobar torsion and gravity factor are considered to be a main process. Authors have experienced 2 cases of unusual manifestations of right upper lobe collapse due to bronchogenic carcinomaduring resent 2 years in Kyung Hee University hospital, and prsent radiologic findings.
