We report a case of a 56-year-old woman with breast cancer, ovarian cancer, and diffuse large B-cell lymphoma with a BRCA1 gene mutation. Evidence is mounting that there is a large increase in the risk for hematologic malignancies among patients with genetic changes in the BRCA pathways. The genomic analysis demonstrated a frameshift mutation in the BRCA1 gene: 277_279delinsCC (Phe93fs). It is a novel BRCA1 mutation that has never been reported, and caused malignant lymphoma as well as breast and ovarian cancer.

PURPOSE: The purpose of this study was to investigate the relationships among psycho-social well-being, and health perception and behavior in clinical nurses. METHODS: The data were collected from 362 nurses employed at five general hospitals. A self-reported questionnaire was used to assess demographics and job characteristics, health perception and behavior, depression, and psycho-social stress. The data were analysed using Cronbach's alpha, descriptive statistics, t-test, and ANOVA. RESULTS: While nurses perceived themselves to be in good health, represented by a high mean for health perceptions, actual health behavior was poor as the mean for health behavior was low. Forty two percents of the subjects were categorized as having major depression and high-stress levels. There were statistically significant differences in psycho-social well-being according to age, marital status, years of employment, and monthly salary. Also, there were significant differences in psycho-social well-being according to health perception, regular meals and exercise, sufficient sleep, job related injury and accident, and absences for ill. CONCLUSION: In order to prevent depression and improve psycho-social well-being among clinical nurses, organizational support and health management needs to be focused on high-risk nurses. Furthermore, it is essential to consider the health perception and behavior of clinical nurses.

5.A Case of Paraneoplastic Limbic Encephalitis Associated with Primary Adenocarcinoma of Lung.

Paraneoplastic limbic encephalitis is a rare disorder that is characterized by personality changes, irritability, depression, seizures, memory loss and dementia, and is commonly associated with small cell lung cancer. The cause is unknown but it is believed to be an autoimmune disorder that develops secondary to a carcinomatous process. We report a patient with the clinical feature consistent with limbic encephalitis. A 64-year-old women developed disorientation, memory loss and general weakness. She was diagnosed with NSCLC (adenocarcinoma) with a brain metastasis 1 year earlier and was treated with radiation and chemotherapy. Although the lung mass and brain metastatic lesions had improved, the brain T2-weighted MRI showed high signal intensity in the right temporal region. This lesion consisted of with limbic encephalitis and was negative to the other viral and immune markers. The patient's symptoms did not improve after steroid treatment. Our case demonstrated that a NSCLC (adenocarcinoma) also can be associated with paraneoplastic limbic encephalitis.

6.Evaluation of Korean Internet Websites for Postnatal Care(Sanhujori).

PURPOSE: In this research the informational contents of websites related to postnatal care were evaluated. This was done in order to inform people of qualified information on postnatal websites. METHOD: Instruments from Oh(2001) and the Korea Institute for Health and Social Affairs(2000) were utilized to evaluate the contents in respect to purpose, reliability, easiness, authoritativeness, feedback, and maintenance. Seventy-three postnatal websites were evaluated for this research conducted from June 10 to July 10, 2006. RESULT: There were no portal sites connected with purely informational postnatal websites. In the evaluation of postnatal websites, the lowest and the highest scores were 11 and 42, respectively. The average score was 24 with 52.1% scoring below the average. By category, the scores of purpose, reliability, and feedback were relatively high while easiness, authoritativeness, and maintenance showed very low scores. As a result, it was revealed that there were no specific postnatal websites with sufficient postnatal care information. CONCLUSION: Thus, the establishment of a professional postnatal portal system through a professionally certified organization is required in order to supply correct information to people who wish to get information on postnatal care.

7.Gemcitabine and Infusional 5-Fluorouracil in Advanced Pancreatic Cancer: A Clinical Benefit Response-Oriented Phase II Study.

PURPOSE: Gemcitabine and 5-fluorouracil (5-FU) are two compounds with reproducible activity against advanced pancreatic carcinomas. To evaluate the activity and feasibility of this combination chemotherapy, a multi-institutional phase II study was performed. MATERIALS AND METHODS: Twenty patients (male: female 15: 5, median age: 60.5 years), with histologically verified locally advanced or metastatic pancreatic carcinomas, were enrolled between April 2000 and March 2002. Gemcitabine was administered by intravenous injection at the doses of 1, 000 mg/m2 on days 1, 8 and 15, and 5-FU 800 mg/m2/day, was given by continuous intravenous infusion on days 1~5. The treatment was repeated every 4 weeks. The clinical benefit response (CBR) was a composite of the pain, Karnofsky performance status and body weight change measurement. RESULTS: Nineteen of the twenty patients were assessable for response. The median follow-up duration was 4.6 months (0.4~15.2 months). Five patients achieved a partial response and eight a stable disease. The overall response rate was 25.0%. The CBR was assessable in 12 patients. The overall CBR was 41.7% (5/12). The median survival of all the patients was 8.0 months. Grade 3~4 toxicities included neutropenia (9.3%) and thrombocytopenia (5.3%). CONCLUSION: This study suggested that gemcitabine, combined with infusional 5-FU, was well tolerated, and produced modest antitumor activity and symptomatic relief in advanced pancreatic cancer patients.

Expression of estrogen receptors (ER)-alpha and -beta, as well as androgen receptor (AR), in hepatocellular carcinoma (HCC) is thought to be correlated with prognosis, survival, and male prevalence of HCC. These hypotheses are based on investigations of European patients; however the expression patterns of these receptors in Asian patients are largely unknown. In this study, we collected liver carcinoma and peritumor tissues from 32 patients (9 females and 23 males) in South Korea. The expression of ERs and ARs was studied using RT-PCR. Wild-type ER-alpha and AR were expressed in all of the samples investigated, and their expression was independent of the causal virus or patient sex. Expression of the ER-alpha variant was independent of sex (100% female vs. 91.3% male) and HCV and HBV status (91.3% vs. 100%). Wild-type ER-beta was expressed more often in HCV patients than in HBV patients (95.7% vs. 44.4%; p < 0.05). In conclusion, the stronger ER-alpha variant expression in HCC tissues implies that this variant has an important role in HCC development. However, at least in Korean patients, expression of the ER-alpha variant (vER-alpha) is not related to male HCC prevalence. In addition, the predominant expression of ER-beta in HCV patients suggests that it plays an important role in HCV-induced liver disease.

9.Expression of the RERG Gene is Gender-Dependent in Hepatocellular Carcinoma and Regulated by Histone Deacetyltransferases.

Ras-related, estrogen-regulated, and growth-inhibitory gene (RERG) is a novel gene that was first reported in breast cancer. However, the functions of RERG are largely unknown in other tumor types. In this study, RERG expression was analyzed in hepatocellular carcinomas of human patients using reverse transcriptase PCR analysis. In addition, the possible regulation of RERG expression by histone deacetyltransferases (HDACs) was studied in several cell lines. Interestingly, the expression of RERG gene was increased in hepatocellular carcinoma (HCC) of male patients (57.9%) but decreased in HCC of females (87.5%) comparison with paired peri-tumoral tissues. Moreover, RERG gene expression was increased in murine hepatoma Hepa1-6 cells, human breast tumor MDA-MB-231 cells, and mouse normal fibroblast NIH3T3 cells after treated by HDAC inhibitor, trichostatin A. Our results suggest that RERG may function in a gender-dependent manner in hepatic tumorigenesis and that the expression of this gene may be regulated by an HDAC-related signaling pathway.
