1.A Comparative Study of Immunohistochemistry and PCR-SSCP for Detection of p53 Mutation In Gastric Carcinoma.

Mutation of the p53 tumor-suppressor gene in exons 4 through 9 was examined in 34 cases of primary advanced gastric cancer using PCR-SSCP (polymerase chain reaction-single strand conformation polymorphism) and the results were compared with p53 protein expression as determined by immunohistochemistry (IHC) using a monoclonal antibody(DO-1). p53 protein detected by IHC was observed in 14 cases (41.2%) and genotypic mutation detected by PCR-SSCP in exons 4-9 was observed in 13 cases (38.2%) One case showed an aberrant band on PCR-SSCP both in Exon 7 and Exon 8/9. p53 alteration detected by either IHC or PCR-SSCP was observed in 19 cases (55.9%), but only 8 cases (23.5%) showed both p53 mutation and protein expression. We also tried to obtain the correlation between relative intensity of the shifted bands on PCR-SSCP and percentage of positive cells by IHC, but a significant correlation was not seen between relative intensity of shifted bands on PCR-SSCP and positve cell ratio. A discrepancy between p53 protein expression and p53 mutation is observed in primary gastric carcinomas. The reason for this discrepancy are not apparent. However, examination of gastric carcinomas for mutations in other exons may identify a better correlation with protein overexpression. The results obtained in this study suggest that the negative reaction for p53 immunohistochemistry may not necessarily mean no genetic alteration of the p53 locus.

4.Clinical Manifestations of Exfoliation Syndrome in Korea.

Exfoliation syndrome(XFS)has rarely been reported in Korea even though its worldwide distribution. The purpose of this study is to report clinical manifestations of XFS in Korea and to determine its relationship to the development of glaucoma in patients with XFS. We retrospectively reviewed the medical records of 31 patients with XFS presenting over a 6-year period at Yeungnam university hospital. Patients were considered to have XFS if characteristic fibrillar exfoliative materials were visualized on the anterior lens capsule. We investigated clinical features such as gender, age, combined systemic disease, laterality, presence of cataract or glaucoma, methods of treatment and complications. We could identify 24 males and 7 females with XFS. All the cases were aged 60 years or more except one case, with the peak in the eighth decade(mean 71.5, range from 37 to 92 years). In 11 patients(35%), XFS was bilateral. Cataract was associated in 28 eyes(67%)out of the total 42 eyes and cataract extractions were performed in 8 eyes. Glaucoma developed in 17(55%)out of the total 31 patients, with the predilection toward the case with bilateral involvement(73%)of XFS, and 44%of glaucomatous eyes required trabeculectomy. The clinical features of XFS in Korea were similar to previous reports from other parts of the world, but interestingly male predominance was a unique finding in this study. Our study suggests that XFS occurs not uncommonly among the Korean population.

5.A case of essential mixed cryoglobulinemia.

A 52-year-old woman with esential mixed cryoglobulinemia (type II ), which was confirmed by cryoglobulin elcct,rophoresis and immunofixation elect,rophoresis, had purpuric skin lesions on the legs. The lesions had a tendency of recurrence for the past 2 years, and moderate degrees of arthralgia and myalgia were accompanied in each time. However, the patient had not been suffered from any internal organ distress caused by the disease proress. Histological examinations of a skin lesions revealed a feature of leukocytoclatic vasculitis, and deposits of IgM, IgG and C; were also recognized. A regimen of small doses of prednisolone was effective for the cutaneous lesions in this patient for 4 months follow-up until this writing. It, seems that the patient has had a mild form of the disease among the clinical spectrum of mixed cryoglobulinemia, with a cryocrit value of 2.4% at the time of her initial visit..

6.The Effects of Combination and Method of Admiaietration of Neosrtigmine Methylsulfate and Atropine Sulfate on Heart Rate.

It is common to administrate neostigmiae methylsulfate and atropiae aulfate to counteract the effect of nondepolarising neuromuscular blocker. In case of cardiovascular disease, the effect of bradycardia or tachycardia resultiag from the administration of these drugs may be harmful to the patient. The purpose of this study is to investigate the effects of combinatioa and method of administration of neostigmine methylsulfate and atropine sulfate on heart rate. One hundred and two patients in both sexes were devided into three groups(A, B, C), and each group was devided into three subgroups (AI, AII, AIII, BI, BII, BIII, CI, CII, CIII). In group A, neostigmine methylsulfate and atropine aulfate were mixed and administered intravenously within fifteen seconds. In group B, atropine eulfate was administered thirty seconds after the administration of neostigmine methylsulfate. In group C, the two drugs were mixed and administered over a period of five minutes. In subgroup I the ratio of neostigmine methyliulfate to atropine sulfate was 2: 1(0. 04 mg/kg: 0.02 mg/kg) in subgroup 3 the ratio was 3: 2(0.03 mg/kg: 0. 02 mg/kg), and in subgroup lll the ratio was 1: 1(0. 02 mg/kg: 0. 02 mg/kg). The heart rate was counted just before, 0. 5 minute, 1 miaute, l. 5 minutes, 2 minutes, 3 minutes, 5 minutes, 10 minutes, 15 minutes, 20 minutes, 25 minutes, 30 minutes after the administration of the drugs. The results were as follows: 1) Initial increase in heart rate was significant in lager atropine ratio. 2) The later decrease in heart rate waa less in larger atropine ratio. 3) The maximal increases in heart rate in groap A and B appeared thirty seconds apart, but there was little difference betweea them. 4) The change ia heart rate was the least in group C.

7.Pemphigus Foliaceus: Report of 5 Cases.

Pemphigua foliaceus is a rare, relatively mild and chronic variety of pemphigus which is characterized by superficial flaccid bullae which show acantholytic changes in the upper part of the epidermis histopathologically. We report herein five cases of pemphigus foliaceus. The patients had erythematous and crusted patches chiefly on face, chest and back areas, except for case 2 who had flaccid bullae on the upper extrimities. The diagnosis of all these patients were confirmed by immunofluorescence studies. Among these five patients, one was recognized as a rifampicin induced pemphigus foliaceus, and the other one revealed concurrent lesions of psoriasis.

9.Three Cases of Membranous Reticular Inflammatory Polyp of the Colon.

Inflammatory polyposis of the colon is rare portion of the colon polyp entity. We experienced three patieats who suffered from colitis of unknown'etiology and discovered membranous retkular with/ without filiform polyp in the distal sigmoid colon of the them. Case 1, 56-year-old female, combined with basaloid cercinoma of the rectal canal that confirmed by pathology after abdominoperineal resection. So we obtained specimen of inflammtory polyp end tried to evaluate the mechanism of it. We found healed ulcer under the mucosal bridge. Case 2. 55-year-old male, had been suffered from irritable bowel syndrome. Prior to visit outpatient clinic, he complained tenesmus with diarrhea for a week. Inflammatory polyp was discovered by colonoacopy but his symptoms relieved by tranqulizers only. Case 3. 57-year-old male, has been a good health. He undertook colonoscopy for routine check thst revealed membranous reticular with filiform inflammatory polyp from distal sigmoid colon to hepatic flexure. Such shape of inflammatory polyp was not documented in Korea, so we reported three cases of it with review of the literature.
