1.Morphometric Analysis of Preeciamptic Nephropathy with Focal Segmental Glomerulosclerosis.

To evaluate the structural characteristics that might be related to the clinical features noted in preeclamptic patients with focal segmental glomerulosclerosis(FSGS), we analyzed post-partum renal biopsies of 8 preeclamptic patients with FSGS (group 1) by morphometry and glomerular studied the structural-functional relationships. These findings were also compared with those from three postpartum cases with minimal change lesion(group 2) and normal age-matched women(group 3). Mean glomerular volume (MGV) in group 1 and group 2 was (2.64 +/- 0.49) x 10(6) micrometer3 and (2.56+/-0.25)x 10(6) micrometer3, respectively. MGV in both groups was significantly increased compared with that of the control group [(1.11+/-0.22)x10(6) micrometer3](p<0.0005). The volume density of the mesangium/glomerulus [Vv(mes/glom)] in the group 1 patients was significantly increased (p<0.0001) when compared with that of the group 2 and the control group patients. The increment of Vv(mes/glom) was related to both the mesangial cell proliferation and expansion of mesangial matrix. The volume density of the capillary lumen/glomerulus [Vv(cap/glom)] in group I was significantly decreased(p<0.0001) when compared with that of group 2 and the control group. Vv(cap/glom) was directly related to Ccr in group l(r=0.70, p=0.05). These results suggest that reduced capillary luminal area caused by mesangial interposition is related to the decreased glomerular filtration rate in preeclamptic FSGS.

4.Glomerular Hypertrophy in Focal Segmental Glomerulosclerosis.

It is not clear whether glomerular hypertrophy is related to the pathogenesis of focal segmental glomerulosclerosis (FSGS). We analyzed renal biopsies from 20 adults with FSGS by morphometry, and the data were compared with those from age- and sex-matched patients with minimal lesion. Mean glomerular volume in the FSGS group was significantly larger than that in the minimal lesion group[(3.4 + 1.1 vs 2.5 0.5)x10(6) micrometer3, P<0.01]. The percentage of glomeruli with global and segmental sclerosis in FSGS group was significantly correlated with the mean glomerular volume (r=+0.66, P<0.001). Relative interstitial volume of renal cortex in the FSGS group was correlated with the serum creatinine concentration(r=+0.5, P<0.05). These results suggest that glomerular hypertrophy observed in our patients with FSGS was related to nephron loss caused by glomerulosclerosis. The interstitial fibrosis may lead to obliteration of postglomerular interstitial capillary network with secondary elevation of glomerular capillary pressure resulting in progressive loss of renal function.

6.DIMENSIONAL ACCURACY OF DENTURE BASE USING LASER SCANNER OF REVERSE ENGINEERING TECHNIC.

The purpose of this study was to evaluate and compare the fit of denture bases processed by injection pressing technic using laser scanner of reverse engineering technic. The auther duplicated 20 maxillary edentulous models and 20 mandibular edentulous models, which were scanned on HYSCAN 45C 3D SCANNER(Hymarc Co., Canada). The scanned data was stored in the personal computer using SURFACER (Imageware Co., U.S.A.) software program. After 40 dentures were cured by PERform Inkovac system. SR-Ivocap system, Palajet system, and Sulfon system, they were stored in water at room temperature for 24 hours. The dentures were scanned on HYSCAN 45C 3D SCANNER(Hymarc Co. Canada). The scanned data were stored in the personal computer using SURFACER (Imageware Co., U.S.A) software program. By overlapping two images using the same program, the fit between two surfaces was scaled by positive and negative errors. The obtained results were as follows : 1. In the upper denture, most of the positive errors occurred on the lingual side of anterior alveolar ridge and the negative errors were on the flange of denture bases. 2. In the lower denture, most of the positive errors occurred on the inner side of lingual flange and the negative errors were on the border of anterior labial flange areas. 3. There were no statistical differences among the positive errors of the four types of inject-ion denture curing methods and also no statistical differences between negative errors except only in negative maximum errors. 4. In PRERform system and SR-Ivocap system, they have the tendency of inaccurate fit lower denture bases comparing to that of upper denture bases. 5. The negative error scales were greater than the positive error scales in all types of inject-ion denture curing methods.

9.Inhibitory control of angiotensin-converting enzyme by ramipril in migraine

Background: Renin-angiotensin systems (RAS) are involved in the physiology of migraine. Ramipril
is an angiotensin-converting enzyme inhibitor. We tested whether ramipril has an effect on migraine.
Methods: The study was designed as a prospective open-labeled trial in a single center. All patients
were asked to maintain a headache diary. Ramipril was administered at 5mg/day (2.5mg twice a day)
and subjects were checked every 4 weeks up to 12 weeks. Results: The mean number of headache
days was 19.9 ± 11.2 days per month at baseline, and 12.0 ± 11.5 at 12 weeks (p<0.001 vs. baseline)
with a responder rate of 41.9%. Stratified analysis by migraine frequency (15 days a month > vs. <15
days a month) did not show a difference. Mean blood pressure was not altered.
Conclusions: Ramipril prevented migraine attacks independently from blood pressure. This result
supports a link between renin-angiotensin system and migraine pathophysiology (Clinicaltrials.gov
