1.Study on Korean Daily Walking Activity <1>.

Since there is no report of daily walking activity of Korean, authors studied on normal adult daliy walking activity in korean of various kinds of occupation, personal life and patients. Total 134 cases were studied and in which there were 10 patients, 7 retired elderlies, 4 house wives, 48 employees of compaines, 30 nurses, 4 laboratory technicians, 2 pharmacists, 7 house staff doctors, 12 chief doctors, 4 cases of hicking and 6 cases of 18 holes golf players. The daily walking steps were measured with pedometer in 167 times in total and following results were, obtained. 1. Bed rest patients ; less than 1000 steps/day 2. Retired elderly ; 2,724(2,150-3,500) steps/day 3. House wife ; 5,437(3,030-7,920) steps/day 4. High ranking, old clerk and chief doctor ; 5,000-6,000 steps/day 5. Ordinary non-active clerk and non-clerk employee ; 6,000-9,000 steps/day 6. common young active clerk and nurse ; 10,000-15,000 steps/day 7. Very active clerk and non-clerk employee ; 15,000(13,000-20,000) steps/day 8. Heavy worker ; more than 20,000 steps/day

3.Bile duct changes in rats reinfected with Clonorchis sinensis.

This study describes an evaluation of the sonographic, cholangiographic, pathological, and immunological findings, and the protective effect shown by rats reinfected with Clonorchis sinensis. Eight experimental rat groups were, namely, a normal control, a primary infection control, a reinfection I (reinfection 7 week after treatment following 3-week infection), a reinfection II (reinfection 2 week after treatment following 8-week infection), a reinfection III (exploration of the intrahepatic bile ducts 1 week after reinfection 4 week after treatment following 4-week infection), a superinfection, a secondary infection control, and an infection following immunization group. Sonographic and cholangiographic findings showed moderate or marked dilatation of the bile duct confluence in the primary infection control, reinfection II, and secondary infection control groups. Juvenile worms survived in the intrahepatic bile ducts 1 week after reinfection following treatment in the reinfection III group. It was concluded that reinfecting juvenile worms found during the first week following reinfection failed to survive or grow further. Anatomical, pathophysiological, or immunological changes may induce protection from reinfection in rats.

4.Clinical Observation of Neonatal Sepsis according to Onset of Disease.

We have experienced 113 cases of neonatal sepsis comfirmed by clinical manifestations and blood cultures from Jan. 1988 to Dec. 1992 at the Neonatal Intensive Care Unit of Ulsan Dong-Kang Hospital and observed the incidence, predisposing perinatal factors, clinical manifestations, associated illnesses, laboratory findings, isolated microorganisms, antibiotics sensitivity test and mortality rate of neonatal sepsis according to onset of disease. The result were as follows: 1) The incidence of neonatal sepsis was 1.39% and male to female ration was 1.38:1. The incidence and sex difference between early onset and late onset disease were not significant. 2) Neonatal sepsis was more prevalent in premature infants (2.47%) than in fullterm infants (1.28%) and nore prevalent in low birth weight infants(3.01%) than in normal birth weight infants (1.25%). In premature infants, neonatal sepsis was more prevalent in early onset (63.2%) than in late onset diease (36.8%). In low birth weight infants, neonatal sepsis was more prevalent in early onset (64.8%) than in late onset dieases (35.7%P). 3) Predisposing perinatal factors, such as meconium staining, birth asphyxia, difficult delivery, premature rupture of membrane, maternal infection, toxemia and postpartum bleeding were slightly frequent in early onset disease. 4) Among the clinical manifestations, jaundice, respiratory symptoms, pallor, lethargy, poor feeding and hepatosplenonegaly were slightly frequent in early onset disease, but temperature instability and gastrointestinal symptoms were slightly frequent in late onset disease. 5) Among the associated illness, pneumonia, disseminated intravascular coagulopathy, amnionitis, hyaline membrane disease and osteomyelits were more common in early onset disease, but gastroenteritis, urinary tract infection, necrotizing enterocolitis, wound infection and meningitis were mors common in late onset disease. 6) The difference of laboratory findings between early onset and late onset disease was not significant. 7) Causative organisms were gram positive organisms in 87 cases(77.0%), gram negative organisms in 22 cases (18.6%) and mixed infections in 5 cases (4.4%). Among them, coagulase negative staphylococcus was the most common one and staphylococcus aureus was the second. The incidence of infections caused by coagulase negative staphylococcus and staphylococcus aureus, between early onset and late onset disease, was not significantly different. Streptococcal infection was more prevalent in early onset disease, especially all group B streptococcus caused early onset disease. 8) Gram positive organisms ware sensitive to Cephalothin (92.9%), Chloramphenicol (90.0%) and Ceftriaxone (88.9%). Gram negative organisms were sensitive to Amikacin (91.3%) and Colistin (82.6%). The difference of antibiotics sensitivity for organisms causing early onset and late onset diease were not significant. Gram negative organisms causing early onset disease were resistant to gentamicin and terramycin, but those organisms causing late onset disease were more sensitive to gentamicin (88.9%) and tobramycin (77.8%). 9) The mortality rate was 7.96%. It was higher in gram negative infections (23.8%) than in gram positive infections (4.6%). No significant difference of mortality rate between early onset and late onset disease was found.

5.A study on renal damage in rats induced by different concentrations and osmolarities of diatrizoate.

There has been few papers regarding the pathologic changes of kidney induced by contrast media, especially in terms of iodine concentration or osmolarity. In order to evaluate histopathologic changes, a series of rat kidneys, after injection of iodinated contrast media, were examined. A total of 220 rats were divided into two groups: those given Urografin-60% by 6.3 ml/Kg(1840mg/6.3ml):those given Urografin-76% by 5ml/Kg(1850mg/5ml). (the same amount of iodine but at different iodine concentration). The kidneys were removed and microscopically examined on 1, 2, 3, 5, 7, 10th days, 2nd, 3rd, 4th, 6th and 12th weeks after injection of contrast media, respectively. The results were as follows: Renal pathologic changes induced by contrast media were congestion and ectatic change of the interstitial vessels, epithelial degeneration and necrosis of the collectiong ducts. Congestion of interstitial vessels and epithelial degeneration and necrosis of the collecting ducts were severe in the higher iodine concentration Urografin-76% group(2100 Osm/Kg H2O) than the Urografin-60% group(1500 Osm/Kg H2O). And above pathological changes persisted for 12weeks without significant interval changes. The authors conclude that the renal damage induced by ionic contrast media becomes more severe in higher concentrations or osmolarities in spite of the same amount of iodine and that the pathologic changes persisted until 12 weeks without improvement.

6.Familial Sarcoidosis, The First Report in Korea.

Sarcoidosis can affect two or more members of the same family, and the reported occurrence of such familial sarcoidosis is variable from 0.5 to 14%. Recently we have experienced familial sarcoidosis affected mother and daughter, for the first time in Korea. Mother was diagnosed as Stage II sarcoidosis 4 years ago by transbronchial lung biopsy and cervical lymph node biopsy with compatible BAL finding in our hospital. This time, the daughter was admitted with bilateral hilar enlargement and anterior uveitis. Even though she had positive tuberculin skin test and atypical BAL finding(lymphocyte: 61%, CD4/CD8: 1.22). Transbronchial lung biopsy and mediastinal lymph node biopsy revealed noncaseating epithelioid granulorna without AFB. Slit lamp examination of the eyes showed severe anterior uveitis. Systemic steroid therapy was started due to progressive uveitis with antituberculous medication.

7.Effect of cutting flute length and shape on insertion and removal torque of orthodontic mini-implants.

OBJECTIVE: The purpose of this study was to evaluate the effect of length and shape of cutting flute on mechanical properties of orthodontic mini-implants. METHODS: Three types of mini-implants with different flute patterns (Type A with 2.6 mm long flute, Type B with 3.9 mm long and straight flute, Type C with 3.9 mm long and helical flute) were inserted into the biomechanical test blocks (Sawbones Inc., USA) with 2 mm and 4 mm cortical bone thicknesses to test insertion and removal torque. RESULTS: In 4 mm cortical bone thickness, Type C mini-implants showed highest maximum insertion torque, then Type A and Type B in order. Type C also showed shortest total insertion time and highest maximum removal torque, but Type A and B didn't showed statistically significant difference in insertion time and removal torque. In 2 mm cortical bone thickness, there were no significant difference in total insertion time and maximum removal torque in three types of mini-implants, but maximum insertion torque of Type A was higher than two other Types of mini-implants. CONCLUSIONS: Consideration about length and shape of cutting flute of mini-implant is also required when the placement site has thick cortical bone.

8.Ultrasonographic findings of periappendiceal abscess

Although the ultrasonography has been regarded as a important procedure in the diagnosis of intraabdominalabscess, there were relatively few papers concerning the ultrasonographic findings of periappendiceal abscess.Nineteen cases of surgically proven periappendiceal abscess caused by perforated appendicitis were studied byultrasonography at the Kyung Hee University Hospital during last 34 months. The results were as follows; 1.Diagnostic accuracy of the real-time ultrasonography was 94.7%(18/19). There were only one false positive and onefalse negative. 2. The location of abscesses were ; periappendiceal 68.4%(13/19), pelvic 21.0%(4/19), subhepatic5.3%(1/19) and subphrenic 5.3%(1/19) in order of frequency. 3. Variable echo-patterns of abscesses wereencountered. But irregular, thick walled, posteriorly reinforcing, echo-free or mixed echo-patterns were mostcommon.

9.Two Cases of Plexiform Neurilemmoma.

Plexiform neurilemmoma is a relatively rare, benign peripheral nerve sheath tumor, whieh usually arises in either the dermis or subcutaneous tissue. These tumors may occur singly or as multiple lesions (plexiform neurilemmomatosis), We report an unusual case of plexiform neurilemmomatosis associated with cafe au lait spots reminiscent of neurofibromatosis clinically and another case of plexiform neurilemmoma on the finger. A Biopsy revealed the characteristic palisaded arrangement of spindle cells within well circumscribed elongated nodules, The skin lesions were completely excised without recurrence thereafter.

10.Ensulin Autoimmune Syndrome in a Patient with Methimazole-Treated Graves' Disease: A Case report.

Insulin autoimmune syndrome (IAS) includes fasting or reactive hypoglycemia, hyperinsulinemia and the presence of insulin-binding antibodies in patients who have never been exposed to exogenous insulin. This report concems a 29-year-old male patient with Graves disease who had history of having taken methimazole for two months, without any consequence, 6 months previously. However, when methimazole was administered again for three weeks, the patient suffered hypoglycemia during the next fourth week. He denied history of diabetes mellitus (DM), of taking any oral hypoglycemic agent or of having received insulin injection. Laboratory data showed total serum insulin level > 300 pu/mL, C-peptide reactivity (CPR) 8.0ng/mL and insulin antibody 89%. After stopping methimazole, he was treated with radioiodine (131I). There was no episode of hypoglycemic attack during 8 months of follow-up.
