3.Hypereosinophilic syndrome: Clinical, laboratory, and imaging manifestations in patients with hepatic involvement.

The hyperosinophilic syndrome (HES) commonly involves liver and spleen but only a few literature has reported the imaging features. In this article, we present the imaging features of the liver and spleen in HES patients together with clinical and laboratory features. This study included 5 HES patients with hepatic involvement. Extensive laboratory tests including multiple hematologic, serologic, parasitologic, and immunologic examinations were performed. Imaging studies included CT, ultrasound (US) of upper abdomen and hepatosplenic scintigraphy. All patients were periodically examined by laboratory and imaging studies for 4 to 24 months. The common clinical presentations were weakness, mild fever, and dry cough. All patients revealed leukocytosis with eosinophilia of 40 to 80% and benign eosnophilic hyperplasia of the bone marrow. The percutaneous biopsy of the hepatic focal lesions performed in 2 patients showed numerous benign eosinophilic infiltrates and one of them revealed combined centrilobular necrosis of hepatocytes. All cases revealed hepatomegaly with multiple focal lesions on at least one of CT, US, or scintigraphy. These findings completely disappeared in 2 To 6 months following medication of corticosteroid or antihistamines. The HES involved the liver and CT, US, or scintigraphy. 5.Teleradiology(TELEACE) system: Results of field trial.

We report the results of field operation of TELEACE system between Kyung-Pook National University Hospital and Ul-Jin Goon Health Care Medical Center from December, 1990 to September, 1991, which had been operated as a kind of Integrated Services Digital Nework projects by KOREA TELECOMMUNICATION Inc. Ul-Jin Goon Health Care Medical Center transmitted 414 plain radiographs to our hospital in speed of 9600BPS. Each image was composed of 1024X1024 pixelsX8 bits/pixel. In our hospital, the image files were displayed on high resolution monitor (1280×1024 pixets). Text files of image interpretations were transmitted to the health care medical center. The two radiologists who had interpreted the transmitted images, went to the health care medical center and read radiographic film with blind test method. We obtained the following results: false negative rate of 6.3%, false postitve rate of 2.4%, mean sensitivity of 81.4%, mean specificity of 96.3%, and mean accuracy of 91.3%. In predictive value of 0.05, there was no significant difference between results of these two types of radiographs. In conclusion, TELEACE system was valuable to the clinicians isolated from services of radiologists.

7.US findings of thyroid carcinomas developed in multinodular goiters.

Primary role of ultrasound in patients with thyroid nodule is to determine the multiplicity and detect occult carcinoma. We analyzed US findings of 53 thyroid carcinomas with multiple nodular lesions from january 1988 to december 1991. The results were as follows: 1. 109 malignant nodules in 53 cases and 24 benign in 23 were comfirmed. 2. The nature of the masses were solid in 72 malignant nodules (74.2%), and complex in 25 (25.8%) of which 19 were predominantly solid. 3. The echo pattern of the solid and solid predominant masses were hypoechoic in 78 malignant nodules (80.4%), hyperechoic in 10 (10.3%), and isoechoic in 3 (9.3%) 4. Internal punctate calcifications within the masses were observed in 31 malignant nodules (31.9%). 5. Halo sign was present in 16 malignant nodules(16.5%). 6. The preoperative sonographic diagnosis of thyroid carcinoma was made in 39.4% of 53 cases with multiple nodules on US. In conclusion, the number, and halo formation of nodules were insignificant to differentiate the benign nodule from the malignant. The heterogeneous hypoechoic nodules with size greater than 4cm, multiple stippled calcifications, displacement of the trachea or other surrounding structures of combined lymph node enlargements seem to indicate the possibility of malignancy.

8.Inhalation Induction of Halothane Using a Vital Capacity Breath .

Inhalation induction of anesthesia is seldom used in a routine adult practice because of the long induction time and the prolonged excitement phase with the risk of laryngospasm and vomiting. So in modern practice, anesthesia is usually administered intravenously and produces unconsclousness pleasantly. However there are situaions where intravenous induction may not be ideal, and where rapid induction is still desired. The author wanted to evaluate the clinical application of inhalation induction of halothane using a vital capacity breath as a substitute for intravenous induction of anesthesia. The patients in this study had an ASA physical status of l or ll and presented no cardiopulmonary disease or abnormal laboratory data. The patients were divided into two group: a control group(n=30) and an experimental group(n=30). Control group: Intravenous induction with thiopental sodium. Experimental group: Inhalation induction with halothane using a vital capacity breath. The results are as follows: 1) The control group consisted of 14 males and 16 females. The mean age was 37.8+/-11.5years, and the ages ranged from 16 to 65 years. The mean body weight was 59.8+/-8.0kg, and body weights ranged from 44 to 75kg. in the experimental group, there were 17 males and 13 females. The mean age was 28.9+/-13.7 years, and the ages ranged from 18 to 65 years. The mean body weight was 57.4+/-8.1 kg, and body weight ranged from 43+/-75kg. 2) In the experimental group, the apnea time ranged from 20 to 105 sec, with a mean of 44.5+/-20.4 sec. The mean induction time was 76.7+/-32.1sec. and induction time ranged from 20 to 150 sec. There was no relationship between apnea time and induction time. 3) The hemodynamic changes were as follows: a. There were significantly greater changes in blood pressure and pulse rate during intubation and postintubation in the control group than in the experimental group(p<0.05). b. There were significant changes in blood pressure and pulse rate in the control group(p<0.05), but seemed not to be hazardous clinically. 4) Induction was impossible in two patients in the experimental group due to profuse secretion or excitement. 5) The side effects in the experimental group included coughs(5 cases), arrythmias(4), excitements(4) and secretion(1), respectively. 6) Postanesthetic comments in the experimental group:27 of the 28 patients remembered the anesthetic smells: 3 pleasantly, 20 moderately and 4 unpleasantly. In conclusion, inhalation induction of halothane using a vital capacity breath is a safe, acceptable and practical alternative to intravenous induction in cooperative adult patients.

9.A Clinical and Pathological Observation of Microinvasive Carcinoma of Uterine Cervix.

Microinvasive carcinoma of the uterine cervix(Stage Ia) is the earliest stage of squamous carcinoma. The transition from preinvasive to invasive disease is a crucial juncture in the development of cervical cancer. The clinical experience that microinvasive lesions carry a better prognosis justifies a separate diagnostic category. To investigate the clinical and pathological aspects of microinvasive cervical carcinoma, a retrospective study was made on 84 cases, histologically reconfirmed surgical specimens, which had been treated during the period from January, 1985 to December, 1996 at Department of Obstetrics and Gynecology, Kyung-pook National University Hospital. We defined microinvasive carcinoma as stromal invasion not exceeding a depth of Smm from the base of the epithelium presented by FIGO in 1985. The results obtained were as follows; 1. Mean age of 84 patients was 46.9 year old, all were parous women but not 1 patients. 60 cases(71.4%) belonged to stage Ial and 24 cases(28,6%) to stage Ia2. 2. Chief complaints were postcoital spotting and leukorrhea. 3, The corresponding rate of Pap. smear to histologic diagnosis was approximately 29.8%. However correponding rate within one histologic grade was about 33,3%. 4. According to the colposcopic examination, 26.4% of cases revealed suspected abnormal findings suggesting invasive lesion. 5. As to the reports of punch biopsy, 55.1% of cases showed microinvasive and/or invasive lesion. With additional diagnostic conization of cervix, the preoperative diagnosis were correct in 72%. 6. Operation performed were simple hysterectomy, extrafascial hysterectomy or modified radical hysterectomy with both pelvic LN dissection, Simple hysterectomy was most commonly performed. 7. Post-treatment complications were developed in 73.9% of patients who were done with modified radical hysterectomy with both pelvic LN dissection and bladder dysfunction was developed in 50% of the patients.
