3.Analysis of the Effectiveness in the Hospital Management of Methicillin-Resistant Staphylococcus aureus by Different Isolation Policies.

BACKGROUND: The purpose of this study is to evaluate the evidence for the effectiveness of different isolation policies in reducing the incidence of methicillin-resistant Staphylococcus aureus (MRSA) infection in hospital in-patients. METHODS: We prospectively evaluated the effectiveness of an isolation policy on transmission of MRSA in the 745-beds hospital. First period, all patients with MRSA (March - July 2000) were not isolated Second period, strict isolation policies were performed (August 2000 - January 2002). All patients with MRSA were isolated in separated room, hand hygiene using alcohol handrub, gowning and g1oving, apply of MRSA notice sticker, criteria in isolation remove, separated disinfection and wastement. Third period, semi strict isolation policies were performed (February 2002 - August 2005). Some patients with MRSA were isolated in separated room and others were admitted in general ward for bed shortage. Only some practices were performed in hand washing and separated disinfection in general ward. RESULTS: The rates of MRSA nosocomial infection per patients during 1st, 2nd, and 3rd surveillance were reported 0.56, 0.23, and 0.42 (P<0.05). Patient-days rate of MRSA nosocomial infection during 1st, 2nd, and 3rd surveillance were reported 0.62, 0.27, and 0.38 (P<0.05). CONCLUSION: With many different isolation policies, it was possible to reduce nosocomial infection of MRSA. In this study, strict isolation policies (2nd period) were the most effective practices in reducing MRSA infection.

7.A Case of Asteroid Hyalitis.

Asteroid hyalitis is an uncommon condition in which small, solid, stellate, spherical or disk-shaped bodies are suspended in a substantially normal vitreous. The opacities occurs in strands or as discrete bodies without orderly arrangement. These move when the eyes move but always return to their original position. The authors experienced one case of asteroid hyalitis associated with arteriosclerosis and hypertension. so we report this with brief review of the literature.

8.Perceived Social Support, Instrumental Support Needs, and Depression of Elderly Women.

PURPOSE: The purpose of this study was investigate the relationships among perceived social support, instrumental support needs, and depression of healthy, community dwelling elderly women. METHOD: This study was designed to examine whether the perceived social support and instrumental support were correlated with depression in elderly women. Data were cross-sectional and community- dwelling elderly individuals. The subject was 98 elderly women recruited from Elderly Welfare Center and Community center for Elderly. The instruments for this study were Personal Resource Questionnaire-part II, Geriatric Depression Scale (Short Form), and Instrumental Support Activities Checklist. Data were gathered by means of structured personal interviews. The internal consistency of the instruments was relatively good. RESULT: The participants' perceived social support was moderately high and instrumental support needs was low. Depression was mild and moderate relatively. The correlation analysis showed that the perceived social support was negatively related to depression and negatively related with instrumental support needs. CONCLUSION: The findings suggested that more perceived social support, less their depressive symptom in elderly women. It also revealed that the age, marital status, and socioeconomic status of elderly women also are important indicator of the elderly depression. Therefore nursing assessment and the design of intervention that incorporate the determinants supported in this study, such as perceived social support of the elderly women are needed in adopting and maintain health promoting lifestyle for the mental health and effective functioning in later life in women.

9.DICS Behavior Pattern and Medication Errors by Nurses.

PURPOSE: Human factor is one of the major causes of medication errors. The purpose of this study was to identify nurses' perception and experience of medication errors, examine the relationship of Dominance, Influence, Steadiness, Conscientiousness (DISC) behavior patterns and medication errors by nurses. METHODS: A descriptive survey design with a convenience sampling was used. Data collection was done using self-report questionnaires answered by 308 nurses from one university hospital and two general hospitals. RESULTS: The most frequent DISC behavioral style of nurses was influence style (41.9%), followed by steadiness style (23.7%), conscientiousness style (20.4%), and dominance style (14.0%). Differences in the perception and experience level of medication errors by nurses' behavioral pattern were not statistically significant. However, nurses with conscientiousness style had the lowest scores for in experience of medication errors and the highest scores for perception of medication errors. CONCLUSION: The results of this study show that identification of the behavior pattern of nurses and application of this education program can prevent medication errors by nurses in hospitals.

10.A Case of Multiple Xanthogranuloma in an Adult.

We report a case of multiple xanthogranuloma occurring ori the scalp in a 33 years old wornan. Multiple yellowish papules were developed on her scalp 3 months ago. Histopathologic findings showed massive granulomatous infiltrations of foamy histiocytes Touton giant cells in the dermis, and these cells were positive for lipid stain. This case was considered to be multiple xanthogranuloma in an adult, and treated by sur- gical excision.
