2.The intrahepatic biliary Cyst in Biliary Atresia after Kasai Operation.

A 6(1/2)-year-old girl developed recurrent cholangitis following hepatic portoenterostomy for biliary atresia. Computed tomogram showed an ovoid cyst (4.5 x 4.0 cm in size) in the left hepatic lobe and another tubular dilatation (2.0 x 0.8 cm in size) in the right hepatic lobe. Percutaneous transhepatic cholangio-drainage (PTCD) with cystogram showed an ovoid cyst in the left hepatic lobe (Tsuchida type A), measuring 6.6 x 5.0 cmin size. She became afebrile and anicteric with aid of PTCD and parenteral antibiotics. However she continued to drain 45-150 cc of bile per day via the tube for over 2 weeks. Then she successfully underwent intrahepatic cystojejunostomy with guidance of intraoperative ultrasonography. This case illustrates relapsing cholangitis caused by Tsuchida type A intrahepatic cyst, which was successfully managed with PTCD followed by internal drainage procedure.

3.Recent 9-year Experience for Biliary Atresia with Introduction of a New Ultrasonographic Diagnosis.

This paper includes our 9-year experience of 34 infants with biliary atresia with introduction of a new non-invasive diagnostic method, that is, ultrasonographic "triangular cord" (TC) sign. TC sign was defined as visualization of a triangular or a band-like echogenicity just cranial to the portal vein. Ultrasonographic TC sign seemed to be a simple, non-invasive, time-saving and useful tool in the diagnosis of biliary atresia, representing 84% sensitivity. Active bile excretion was restored in 90% of the patients who were treated between 31-60days, 78% of those between 61-90 days, and 33% of those being 91days or older. The incidence of postoperative cholangitis was 36%, and construction of antireflux valve in the Roux-en -Y loop did not affect the incidence of postoperative cholangitis (P=0.18). As for the surgical outcome, of 34 infants with biliary atresia, 23 (68%) are alive for 2-102 months period, and 12 of them are alive for more than 5 years . Five-year estimate survival by Kaplan-Meier method was 66 %.

Primary segmental volvulus (PSV) of the ileum in neonate occurs rarely but shows an aggressive clinical course. Thus, early diagnosis is important to prevent necrosis and perforation of the intestine. We report a case of PSV of the ileum in a 2-day-old female neonate whose clinical features and radiologic findings appeared to be meconium plug syndrome.

9.Differential Expression of Transforming Growth Factor-α and Epidermal Growth Factor in Developing Mouse Embryo.

The distribution of transforming growth factor-α (TGF-α) and epidermal growth factor (EGF) in developing mouse embryos of gestational age 8 to 15 days was immunohistochemically (ABC method) studied to investigate the differential expression of these growth factors. Paraffin embedded sections were immunostained with antibodies for TGF-α and EGF. Staining of TGF-α was observed in several organs derived from endoderm, mesoderm and ectoderm in 9-day-old mouse embryos, such as in the heart, optic pit, head mesenchyme, neural tube and primitive gut, and the staining became more intense in 10 to 15-day-old mouse embryos. The staining of EGF was seen in the heart and primitive gut derived from mesoderm and ectoderm respectively, in 9-day-old mouse embryos, but it was observed in other organs as well in 10 to 15-day-old embryos although the intensity was weaker. In the development of heart, immunoreactivity for TGF-α was more intense than EGF, which suggests more active involvement of TGF-α. In the lung, TGF-α staining was observed both in the bronchus and lung bud, whereas EGF staining was seen only the bronchus. In the nervous system, TGF-α was expressed more extensively and more intensively than EGF. In the developing skeletal system, TGF-α staining was stronger and the expression was observed at earlier stage compared with EGF. These results indicate that the activity of TGF-α is more potent than EGF in the development of mouse embryo in general, especially, in the development of mouse heart, nervous system, mesenchyme and skeletal system.

10.Health-Related Behaviors and Subjective Symptoms Associated with Smoking of Freshmen in a University.

This study was to examine the association of the health-related behaviors and subjective symptoms with smoking. Data were collected by questionnaire survey during regular health examination from Mar. 23 to Mar. 31, 1992 for 1,615 male freshmen of a university in Kwangju City. The results obtained were as follows; 1. Among the freshmen, 26.9% reported that they were smokers. 69.4% for smokers started smoking for the recent 4 years, and 63.4% for smokers smoked 10 cigarettes or more a day. 2. Meal regularity, meat eating, use of coffee or tea and alcohol drinking were positively associated with the status, the duration and the amount of smoking while the vegetable preference was negatively associated with the status and the duration of smoking. 3. There was no evidence of familial aggregation in smoking status except that of siblings. 4. Respiratory symptoms like cough or phlegm, dyspnea were positively associated with the status, the duration, and the amount of smoking. General symptoms like chest pain, fatigue, back pain, facial edema, and weight loss were positively associated with the duration and the amount of smoking. Other symptoms like headache, dizziness, and myalgia were not associated with smoking. 5. In multivariate' logistic regression analysis, cough or phlegm, dyspnea, chest pain, facial edema, and back pain were related to smoking status.
