1.Blood Oxygen Content in the Various Chambers of the Right Side of the Heart in Patients without Intracardiac Shunts.

It would seem logical that a left to right shunt may be localized to that chamber in which an increase in oxygen content beyond the normal variation is first seen, while this is generally true, there are certain limitations to such direct diagnostic interpretation. The interpretation of an observed difference in oxygen content is influenced by the result of extremely poor mixing or of a change in respiratory, circulatory or metabolic steady states. This is one of the major challenges to proper interpretation. The purpose of this study was to analyze the blood oxygen content in various chambers of the right side of the heart in patients without shunt and to study the difference in oxygen content between the various chambers. The right heart catheterization was performed in 19 cardiac patients without shunt in Cardiac Laboratory in Hospital. 1. The blood oxygen content was 12.36+/-2.23 volume per cent (mean+/-standard error) in superior vena vava, 12.59+/-2.23 volume per cent in inferior vena cava, 12.59+/-2.23 volume per cent in mid-right atrium, 12.92+/-1.90 volume per cent in mid-right ventricle and 12.26+/-2.13 volume per cent in main pulmonary artery. 2. The mean difference of oxygen content was 0.16 volume per cent between superior vena cava and right atrium, 0.08 volume per cent between right atrium and right ventricle, and 0.06 volume per cent between right ventricle and main pulmonary artery.
2.Rupture of The Adductor Longus Muscle of The Thigh: 5 Cases Report

Rupture of the adductor longus muscle of the thigh is a rare injury. It is produced by several causes, but indirect force is much more frequent than other causes. Wide abduction of the legs may produce rupture of the adductor longus muscle. Palpable mass appears at the upper third of the thigh and become more prominent during contraction of the adductor muscles. We have had the opportunity to examine 5 patients with ruptured adductor longus muscles of the thigh and report them in this paper.

3.An Evaluation Study on the Cardiovascular Risk Factors of the Staff in a University.

To get basic data for the development of health care toward the staff of a university, an evaluation study of the risk factors for the cardiovascular disease utilizing data from questionnaire and health examination performed in 1994 for 1233 staff of Chosun University and its affiliated Hospital. The results are as follows: 1. The mean values of systolic blood pressure was 122.0+/-14.7mmHg for the male and 111.8+/-12. lmmHg for the female. The mean values of diastolic blood pressure was 82. 5+/-12.0mmHg for the male and 73.7+/-10.0mmHg for the female. The mean values of serum total cholesterol was 185.6+/-33.4mg/dl for the male and 173.5+/-32.6mg/dl for the female. The mean values of body mass index(BMI) was 23. l+/-2.4kg/m2 for the male and 21.5+/-2.3kg/m2 for the female. The smoking rate of the male was 47. 6% and that of the female was 0.4%. 2. The systolic and diastolic blood pressure levels increased with age in both sexes. Under 55 years of age the mean blood pressures of the male were higher than that of the female, but the levels of both sexes at 55-64 year old group did not show any big difference. The mean values of serum total cholesterol and BMI also increased with age in both sexes. Under 45 years of age, the serum cholesterol and BMI levels of the male were higher than that of the female, but on the contrary, levels of the male were lower than that of the female at 55-64 year old groups. The mean cholestrol and BMI levels of both sexes at 45-54 year old group was similar. As the age increases, the rate of exsmoker increased and the rate of current smoker decreased among the male. 3. The additive cardiovascular risk scores were calculated considering the values of blood pressure, serum total cholesterol level and the smoking status. The distribution curve of risk scores for the male showed 39. 1% under 4 points and 60.9% above 4 points with the peak at 4 points. 61.7% of the female showed that the risk scores were under 4 points and the rest was more than 4 points with maximum 9 points. As a whole the risk scores of the female were lower than that of the male. 4. By the analysis of association between the risk score of cardiovascular disease and the subjective symptom under the stratification of age and sex, the prevalence of exertional dyspnea was significantly higher at 35-44 year male group and prevalence of dizziness was significantly higher at 45-64 year male group as the risk score of cardiovascular disease increased. Among 25 34 year and 45-64 year female group persons with lower risk score of cardiovascular disease showed higher prevalence of dizziness than persosns with higher risk score.

4.Metronidazole in amoebiasis 1. The use of metronidazole on the treatment of hepatic and pulmonary amoebiasis.

Total 9 cases of extraintestinal amoebiasis, amebic liver abscess and amebic lung abscess, were treated with metronidazole. The diagnosis and progress check were done by (1) stool examination, (2) size and pain of liver, (3) hematological examination, (4) urinanalysis, (5) X-ray or liver scanning (6) examination of aspirated pus from liver and (7) immobilization test of E. histolytica. Results are summarized as follows: Amebic hepatitis: Metronidazole was given to 3 cases with daily dose of 0.9-1.2 gm for 3-7 days. Clinical symptoms were completely restored by the treatment. Amebic lung abscess: One case was treated with metronidazole for 3 days with the daily dose 1.2 gm. X-ray findings showed remarkable improvement by the treatment. An additional administration of the drug for 6 days resulted complete resolution of the pneumonic changes. Amebic liver abscess: Four cases were treated with metronidazole. The daily doses were 0.4-2.4 gm and continued for 2-10 days. Aspiration of the abscess for 1-10 times or continuous drainage of pus were performed during the course supplementarily. All cases were recovered to normal by the combining treatment. No remarkable side effect was noticed by the drug administration, and no relapse was experienced up to date.

10.Radiological evaluation of pulmonary metastasis

158 cases of pulmonary metastasis having satisfactory histological proof of the primary malignant tumor, the frequency and pattern of which were analysed by conventional radiologically. The results are as follows: 1. The frequency of pulomonary metastasis according to the primary sites was 24%(38/158) hepatobiliary system,19.6%(31/158) gastrointestinal tract, 18.9%(30/158) uterine cervix and 12%(13/158) head and neck. 2. Pulmonary metastatic pattern showed nodular type 74% (117/158), granular type 10.1%(16/158), streaky type 9.5%(15/158),hilar enlargement 3.8%(6/158), patchy type 1.9% (3/158) and others 0.6%(1/158). Of all these type, nodular type was predominant. 3. The nodular type of pulmonary metastasis was classified into less than 10 nodules56.4%(66/17), 10-50 nodules 25.6%(30/117), more than 50, 17.9%(21/117), less than 1cm 12.8%(15/117) and more than5cm in 5.2%(1/117) of the cases. 4. The size of nodule was 1-3cm 36.6%(43/117), 2-3cm 20.5%(24/117), less than 1cm12.8%(15/117) and more than 5cm in 5.2%(1/117) of the cases. 5. Most cases of pulmonary metastasis involved bothlung fields 69.2%(110/58). Others were right lung field only 17.7% (28/158), left lung only 12.7%(20/158). 6. Theages between 40 and 70 represented 79.8%(126/158) of the cases and the raio of male and female was about 1.5:1.
