1.A Case of Flexural Darier' s Disease.

Dariers disease is a domirantly inherited skin disease that is characterized by keratotic papules predominantly on the sebornieic areas, palmar pits, and a nail dystrophy. The diagnosis is difficult in the small group with flexural involvement because the features of flexural Dariers disease overlap clinically and histolopica.lly with those of Hailey-Hailey disease. The diagnosis of Dariers disease may be confirmed if the patient has the pathognomonic nail dystrophy. We presents a case of flexured. Dariers disease in a 65 year old woman. She has suffered from skin lesions which have some resemblance to those of Hailey-Hailey disease. We could diagnose her skin lesions as Dariers disease because she had the eharacteristic nail dystrophy.

2.Benign Neonatal Hemagiomatosis with Unusually Persistent Lesions and Conjunctival Hemangioma.

Most neonatal cutaneous hemangiomatosis are complicated by visceral involvement and. are associated with a high mortality rate in the first month of life. Some neonates with multiple cutaneous hemangiomas, however, may follow a benign course of spontaneous resolution without visceral involvement. Such cases are called benign neonatal hemangiomatosis (BNH). BNH is characterized by a lack of mucosal or symptomatic visceral involvement. rapid spontaneous regression of cutaneous hemangimas, and an excellent prognosis. We report herein a case of BNH which is atypical compared to previously reported BNFI cases. Our patient had a more prolonged course than those of usual cases with BNH and showed a conjunctival involvement.

3.Lower Extremity Rotational Profile in Students of Elementary School.

We studied 600 normal lower extremities of Korean students of elementary school in order to establish normal values for the rotational profile and to define any differences between ethnic groups. Rotational profile examined at this study included medial and lateral rotation of the hip and thighfoot angle. The mean value of medial rotation of the hip was 48 degrees with a range of 35 to 64 degrees in male subjects, and 50 degrees with a range of 35 to 60 degrees in female subjects. The mean value of lateral rotation of the hip was 57 degrees with a range of 43 to 75 degrees, and 53 degrees with a range of 40 to 72 degrees in female subjects. Unlike medial rotation, lateral rotation of the hip was greater in male than in female subjects by a mean difference of 4 degrees(T-test, p<0.05). The mean value of thigh-foot angle was 10 degrees with a range of -2 to 23 degrees in male subjects, and 12 degrees with a range of 0 to 21 degrees in female subjects. The total range of hip rotation was slightly greater in Korean than in Caucasians or in Chinese, but, to conclude this, more large study by the same methods will be needed in other areas of Korea.

4.The Clinical Study of Pyogenic Arthritis: Pathogeny and Complication

After developement of antimicrobial chemotherapy, morbidity and mortality from pyogenic arthritis has been reduced dramatically, but still this disease has remained as a serious and life threatening infectious disease of childhood or late sequelae in surviving patients. For the period of 6 years from January 1978 to December 1983, seventy nine patients, eigthy one cases who were treated as pyogenic arthritis at Severance Hospital and Yong Dong Hospital were studied retrospectively and the results are summerized as follows. 1. We thought that the main pathogeny of septic arthritis of knee joint are trauma and direct invasion of microorganism, because the frequency of pyogenic arthritis in knee joint are proportional to the age and history of accupuncture therapy and trauma are common. 2. In thirty three cases (40.7%) underlying causes were found, composed of infectious focus in eleven cases(13.6%) correspondent to hematologic transmission, osteomyelitis in seven cases(8.7%), trauma and accupuncture in six cases(7.4%) and eight cases(9.9%) corresponding to direct invasion. 3. In forty five cases(55.6%) microorganism are identified, among which coagulase positive staphylococci are in forty cases(88.9%) . 4. Tc-99m-MDP(Medronate) whole body bone scan were taken at twelve patients and positive findings were eleven cases. 5. In thirty eight cases(46.9%) the complications were appeared: Osteomyelitis of tibia in ankle joint and periarticular or subarticular bone defects in knee joints were common. 6. There were complications in every cases that the duration was more than eleven days, no exceptional.

5.A Case of Unilateral Nevoid Telangiectasia Associated With Acanthosis Nigricans.

We report a case of unilateral nevoid telangiectasia associated with acanthosis nigricans in a 20-year old male patient. Unilateral nevoid telangiectasia may be congenital or acquired. The unilateral and dermatomal distribution suggests that distribution of target vessels is fixed and that they are sensitive to estrogen. Estrogens are mainly produced by the ovaries, but they are also produced by peripheral cirornatization of androgen. Because a major site of this conversion is adipose tissue, estrogen can increase in an obese person. Obesity also reverses the metabolism of estrogen and increases the serum insulin level. Therefore it seems that obesity in our patient induces the development of unilateral nevoid telangiectasia and acanthosis nigricans.

6.Self-care, Family Support and Depression in Elderly Patients with Diabetes Mellitus.

PURPOSE: This study was done to investigate the degree of self-care, family support and depression, and relationship among these variables for elder with diabetes mellitus. METHOD: Participants were 202 diabetic patients, 65 years or over, living in Seoul, Korea. Data were collected using the self-care tool for diabetic patients by Kim (1996), the family support tool for diabetic patients by Park (1984), and Korea\' s BDI scale by Lee (1995). RESULTS: Of the patients, 43.1% showed HbA1c levels than higher 7%. The highest mean score was for self-care for medication compliance, and the lowest for blood glucose testing compliance. Factors affecting self-care were employment, education, HbA1c level, diabetic self-care education and complications. Factors affecting family support were living with family, diabetic self-care education, hospitalization and complications. Factors affecting depression were gender, living with family and complications. All of these factors were significant. Patients experiencing depression were 16.8% of patients. There was a significant positive correlation between self-care and family support, and significant negative correlations between self-care and depression, and family support and depression. CONCLUSION: For more effective management of diabetes mellitus in elders, improvement in self-care compliance, and family support are needed

7.Serum hormone and testicular histologic changes in spinal cord injured men.

A quantitative assessment of the testicular biopsy, and serum hormone levels( testosterone, prolactin, and follicle-stimulating and luteinizing hormones) were studied in 10 spinal cord injured men and comparison of these parameters to normal controls were performed. The mean number of Sertoli cells, mature spermatids, tubular diameter and tubular wall thickness were determined in both groups and analyzed. In the spinal cord injury group the mean number of mature spermatids per tubule was significantly lower and the mean number of Sertoli cells per tubule was significantly higher than in controls(p<0.05). Serum testosterone and luteinizing hormone values were significantly higher in the spinal cord injury group than in controls(p<0.05). These significant differences of the quantitative parameters of the testicular biopsy and hormonal values that may contribute to the reproductive dysfunction.

8.The Efficacy of Allylestrenol in the Treatment of Benign Prostatic Hyperplasia.

Benign Prostatic Hyperplasia is one of the most important desease with a high frequency in urology. There is no doubt that the main stream of treatment is transurethral resection. of the prostate, however, marked improvement in drug therapy has been observed in recent years. Allylestrenol is a synthetic gestagen which was known to act directly on the prostate to exhibit its effect of reducing the size of the prostate by inhibiting the selective uptake of serum testosterone, inhibition of metabolism of testosterone to 5a-DHT (dihydrotestosterone) reductase, inhibition of 5a-DHT receptor binding. Our present study was carried out on 108 patients with benign prostatic hyperplasia to evaluate the efficacy and safety of antiandrogen therapy with allylestrenol. Allylestrenol was administered in a dose of 25 mg twice a day in the morning and evening after meal for 12 weeks to the patients and its efficacy was evaluated with peak urinary flow rate, Madsen Symptom Score at 4, 12 weeks after treatment and prostate volume at 12 weeks after treatment. Of these patients 46 completed the study and only 2 patients withdrew from the study owing to side effects. However,these side effects were not serious. At baseline (mean +SD) the mean peak urinary flow rate was 11.0+/-3.7 ml/sec, mean symptom score 10.2+4.0 and mean ultrasonic assessment of prostatic volume 36.8 +/-9.6 gm. At 4 weeks after treatment(mean + SD) mean peak urinary flow rate was 12+/-3.5 ml/sec, mean symptom score 7.7+/-4.2 (p > .0.05, p < 0.05). At 12 weeks after treatment (mean +SD) mean peak urinary flow rate was 12.8+4.7 ml/sec, mean symptom score 7.0+4.2 and mean ultrasonic assessment of prostatic volume 36.6+/-9.3 gm (p < 0.05, p < 0.05, p > 0.05).

9.Life Experience of Maternal Absence to the Adolescent Boys Living with Father.

The purpose of this study was to understand life experience of mother's abscence to the adolescent boys. Nine participants(7th-9th grades) were selected and interviewed intensively. Data were analyzed according to the methods of Colaizzi. Significant statements were extracted from data. From those significant statements, 37 themes, 15 clusters of themes and 5 categories were emerged. Fifteen cluster of themes formed 5 categories : Feeling loneliness, longing for mother and suppressing the painful feelings, sense of distance between toward father and friend, being pessimistic about the situation they face, and feeling envy. In this study the author described unique life experience of adolescent boys who are living with single father.

10.A Case of Juvenile Xanthogranuloma Associated with Juvenile Chronic Myeloid Leukemia and Neurofibromatosis.

Juvenile xanthogranuloma(JXG) is a a benign histiocytic proliferative disorder most frequently seen in infants. The usual course is self-limited, but it may be a manifestation of juvenile chronic myeloid leukemia(JCML). In addition to JXG, some patient with JCML may have multiple cafe-au-lait spots and a family history of neurofibromatosis(NF). The association of JXG and cafe-au-lait spots in patients with JCML is of interest in light of description of otherwise healthy children with JXG who also had cafe-au-lait spots. We report herein a case of JXG in 18-month-old boy who also had multiple cafe-au-alit spots, which predated the JCML.
