2.Congenital Dislocation of Hip in Twin Babies: A case report

Congenital dislocation of the hip has been known as one of the most common congenital anomalies in the orthopedic field since the time of Hippocrates (460-357 B.C.). The occurence of C.D.H. in twin babies is extremely rare thus few if any papers have been reported. During the period of January 1971 through June 1976 the authors have treated a total of 49 cases as inpatients. Two of these were twin babies. In both cases the hip was surgically reduced and followed by open reduction and femoral derotation osteotomy. Clinically and radiologically the results were good during the duration of follow-up seven months after operations.

3.A study on the Compliance and Educational Demand of Renal Transplantation Patient.

This study was started for the purpose of providing the basic data for continous managment of kidney transplantation patients after discharge. This study was conducted on 180 patients who received renal transplants at three hospital ( B, M. P) pusan, korea.. The data collection was done for june 1. to August 31. 2002. General characteristics, renal transplantaton characteristics. physical characteristics, the level of compliance and the degree of educational demand were done by the number and percentage. the mean. standard deviation. The level of compliance and educational demand followed by the characteristics of general and kidney transplantation were analyzed by t-test and ANOVA. The result were as fallows: 1. Man was higher than woman such as 60.0%, Mean age was 42.5 years old. Average total duration of after operation was 5.5 years. 2. Cases of systolic blood pressure over 140mmHg were 10.0%, cases of diastolic blood pressure over 90mmHg were 22.8% and obesity factor in BMI was 15.6%. The person who daily water intake amount is 5000cc was 0,6%, the case that daily urine output is below 1000cc was 8.9%. and the case that urine output is zero was 0,6%. 3. The mean score of compliance was 77.47 point. The score in medication part was highest such as 4.67 point, that in stress situation was lowest such as 3,50 point. 4. The average score of educational demand was 154.02 point. and physical state part was 4.36 points highest. activation part was 3.48 points lowest. As a role of nurse Confirmation of compliance is very important encourage to make good through regular hospital visitation. point out the noncompliance part and then increase compliance of renal transplantation patient As well there will be maintain the normal kidney function to satisfy educational demand through continous education.

4.CT menifestations of cervical tuberculous lymphadenitis.

Cervical tuberculous lymphadenitis is a commonly encountered disease, expecially in adults, Differentiation from other lymphadenopathy and benign conditions such as cystic neck masses is important. CT findings of tuberculous lymphadenopathy in the abdomen and thorax are reported in many literatures. But there are only a few articles concerning cervical tuberculous lymphadenopathy. The authors retrospectively analyzed CT findings of 33 cases with cervical tuberculous lymphadenitis regarding distribution, contour, enhancing pattern, changes of adjacent fascial plane, and dermal and subcutaneous manifestations. We concluded that the presence of conglomerated nodal masses with central lucency, thick irregular rim of contrast enhancement and inner nodularity, varying degree of homogenous enhancement in smaller nodes, dermal and subcutaneous manifestations of inflammation such as thickening of overlying skin, engorgement of the lymphatic and thickening of adjacent muscles, and diffusely effaced fascial plane are suggestive of tuberculous lymphadenitis. However, some CT patterns of tuberculous adenitis may be seen in other disease; for example, enhancement can occur in hyperplastic nodes, vascular metastasis(thyroid, melanoma, and hypernephroma), lymphoma, granulomatous disease, and Castleman's disease.

5.Aneurysms of the superficial temporal artery.

Aneurysm of the superficial temporal artery is rare. We reviewed seven cases of aneurysm of the superficial temporal artery, which were confirmed surgically and angiographically. The results were as follows: The most common site of aneurysm was left superficial temporal artery, major feeding artery was anterior branch of superficial temporal artery, mean diameter was 7.8mm×12.6mm, and all aneurysms showed lobulated margin with intraluminal filling defects.

8.A Clinical Review of Intussusception of Infancy and Childhood in Emergency Department.

Intussusception is a condition of invagination of the proximal segment of bowel into the more distal receiving bowel. Intussusception is one of the common pediatric and surgical emergencies of infancy and childhood in Korea. Intussusception is the most common cause of acquired intestinal obstruction in infancy and childhood, and it needs emergency treatment. 1) On the age and sex distribution, the age under 15 month was the most common(70.6%), and the peak incidence was 3-12 months of age(60.1%). In sex distribution, male to female ratio was 1.25:1. 2) The seasonal distribution was summer(27.4%), spring, autumn and winter in order of frequency. 3) As to the etiology of Intussusception treated by surgery, idiopathic was the most common(75%). 4) The cardinal symptoms and signs were vomiting(72.5%), bloody or bloody mucoid stool(57.5%), abdominal pain(49.7%), abdominal distention(20.9%), abdominal mass(19.6%) and others in order of frequency. 5) On the hematologic finding, 60.8% revealed leukocytosis above 10,000/mm3. 6) As to the duration from the onset of the symptom to admission, 84.2% was within 24 hours, 9.8% was from 24 hours to 48 hours, and 5% was over 48 hours. 7) The common anatomical types were ileocolic(73.2%), and ileocecal(17.0%). 8) The success rate of air reduction was 84.3%. 9) In the method of operation, manual reduction was performed in 91.7% and intestinal resection in 8.3%. 10) The recurrence rate after air reduction was 12.4% and after operative manual reduction was 8.3%.

9.A Case of Extrapulmonary Paragonimiasis Involiving Liver and Cecum.

We report a caae of hepatic and intestinal infestation with paragonimus wewstermani complicating abscess formation. The pathway by which the paragonimus westermani reaehea liver and intestine is not well understood. However, there was possiblity that it may become lodged in other organs that the lung because of polonged larval migrations through the diaphragm or into various sites in the abdomen. The patient, 52-year-old female gave a history of having reyeatedly consumed raw crabs. An x-ray film of the chest showed clear lunga Skin test for paragonimus-westermani was positive. Abdominal ultrasonogram showed enlargement of the liver with multiple hypoechoic lesiona ERCP revealed multiple ie lesions in the right lobe of the liver. Abdaminal CT showed multifocal abscess cavities with slight rim enhancements. Above meetioned diagnostic procedures suggested liver abscess or hepatoma. Then, an exploratory laparotomy was done for a definite diagnosis, Frozen biopsy, from liver and surgically exe mass from ileocecal region revealed extrpulmonary paragnomiasis involving liver and cecum. Thus, the patient was treated with prasiquantel. Three months later, abdominal ultrasonogram demonstrated slight enlargement of the liver but no evidence of abnormal mass like lesions.

10.A Study of Lumbar Spinal Canal Using Computerized Axial Tomogram in Korean Adults

Appreciation of the normal variation of anterior and posterior diameter of the lumbar spinal canal under computerized axial tomogram may throw very important value on the diagnosis and treatment in lumbar spinal problems. The authers have measured the diameter of the lumbar spinal canal using EMI scanner CT 5005. The data was first based on the study of two cadaver lumbar spine and then the study of 268 normal Korean adults male 185, and female 83. The results were summerized as follows. 1. The diameter of the osseous segment was more narrow than articular segment in normal lumbar spinal canal, 2. The shape of lumbar spinal canal was oval in upper lumbar. but became triangular going down to the lower lumbar. There was no difference of diameter in sex. 4. The average diameter of anterior and posterior spinal canal was; L1 16.53± 0.18, L2 16.23± 0.215, L3 15.42± 0.26, L4 15.57± 0.43 and l% 17.7± 1.63mm respectively. 5. The average diameter of transverse spinal canal was; L1 23.31± 0.18, L2 23.76± 0.28, L3 24.78± 0.345, L4 26.07± 0.605 and L5 29.31± 1.19mm respectively. 6. It was more accurate to check out the pathology of the lumbar spinal canal using coinputerized axial tomogram than other method previously used.
