2.A Clinical and Statistical Study in 78 Cases of Ovarian Cancer.

Clinical and pathological studies were carried out on 78 patients of ovarian cancer from January 1985 to March 1991. The results are as follows : l. Among 78 patients of ovarian cancer, the age group from 51 to 60 years old was most common which was 18 cases(23.1%). 2. When classified according to the cell type, epithelial cell origin cancers were 59 cases (75.6%), ovarian cancers from germ cell origin 9 cases(11.5%), sex cord stromal cancers 3 cases(3.8%), and metastatic cancers 7 cases(8.9%) respectively. 3. When classified according to the FIGO staging, stage I were 34 cases(43.5%), stage II 8 cases(10.3%), stage III 23 cases(29.5%) and stage IV 13 cases(16,7%), respectively. 4. Preoperative mean CA 125 value of stage I ovarian cancer was 130.2+/-57.3U/ml, stage II 74.2+/-144.4U/ml, stage III 376.9+/-296.8U/ml and stage IV 433.9+/-148.2U/ml respectively. and Also preoperative mean CEA value of stage I ovarian cancer was 3.4+/-3.2ng/ ml, stage II 6.9+/-4.3ng/ml, stage III 4.7+/-4.4ng/ml and stage IV 4.6+/-6.3ng/ml respectively. 5. When classified according to the physical examination, the most common finding was palpable mass which were 38 cases(43.7%), the second, abdominal pain 30 cases(38.4%) and the third, abdominal distension 21 cases(26.9%) 6. When classified according to the operation, ipsilateral oophorectomy was 15 cases(25.5%), ipsilateral oophorectomy and contralateral ovary wedge resection 6 cases(10.2%), total abdominal hysterectomy 17 cases(28.8%), total abdominal hysterctomy and omentectomy 14 cases(23.7%), total abdominal hysterectomy and omentectomy and debulking tumor resetion 4 cases(6.7%) and biopsy only 3 cases(5.1%) respetctively. 7. When calssifide according to the postoterative endometrial finding, atrophic pattern was 28 cases(65.7%), proliferative pattern 10 cases(28.6%) and secretory pattern 2 cases(5.7%) respectively.

Congenital tuberculosis is an infection that is established in the fetus by hematogenous spread or by the aspiration or inhalation of infected amniotic fluid either before or during labor. The diagnosis can be confirmed only if both the tuberculous nature of the lesion and the antenatal origin of the infection can be proved. The authors analysed roentgen findings of two cases of pulmonary tuberculosis, thought to be congenital; 1. Diffuse distribution of nodular densities and some confluent densities were seen in the entire lungs in the inital film.The pulmonary markings were decreased in some degree. 2. Air bronchogram was noted. 3. Hepatosplenomegaly was associated.

4.Avascular Necrosis of the Femoral Head in a Patient with Poliomyelitis Treated by THA with a Large-diameter Metal Head - A Case Report -.

Total hip arthroplasty (THA) can be a good treatment option for avascular necrosis (AVN) of the femoral head. However, because dislocation can frequently occur after surgery, THA is a concern in patients with a neuromuscular disease, such as cerebral palsy or poliomyelitis. In patients with poliomyelitis, only one case of AVN of the femoral head in the affected limb has been reported in the English literature. Here, the authors report on a case of AVN of the femoral head in a patient with poliomyelitis, who was treated with a large diameter femoral head metal-on-metal THA using a modified minimally invasive-2-incision technique.

5.The Relationship among Pain, Body Image and Satisfaction in Hand-injured Workers.

PURPOSE: The purpose of this study was to explore relationship among pain, body image and satisfaction in hand-injured workers. METHODS: Using a sample of 96 out-patient workers with hand injuries, pain, body image and satisfaction were measured by VAS and Modified Jung Keum-hee's Tennesee Self Concept Scale. Repeated measured ANOVA and a Bonferroni correction for multiple statistical testing during post hoc analyses were conducted to evaluate relationship among pain, body image and satisfaction in hand-injured workers. RESULT: Functional satisfaction(r=.46, p<.001) and appearance satisfaction(r=.55, p<.001) significantly increased by treatment satisfaction. Functional satisfaction(r=.35, p= .001) and appearance satisfaction(r=.31, p= .002) increased by body image. But treatment satisfaction(r=-.20, p=.049), functional satisfaction (r=-.34, p=.001) and appearance satisfaction (r=-.39, p<.001) significantly decreased by pain. CONCLUSION: Poor pain management was related to bad body image and treatment satisfaction. Therefore, interventions targeting at pain management are needed for hand-injured workers.

6.Endoscopic retrograde cholangiographic findings in choledocholithiasis

Although ultrasonography replaced many invasive studies in biliary tract diseases, direct cholangiography does still play an important role in the diagnosis and management of choledocholithiasis. Endoscopoic retrograde cholangiography (ERC) is regarded as the best method in evaluation of exact extent of the disease and its frequent complication, cholangitis. Authors analysed 56 cases of choledocolithiasis diagnosed by ERC and compared these with ERC in 18 cases of normal, 22 cases of cholecystitis, 15 cases of clonorchiasis and 9 cases of parenchymal diseases of liver. The results are as follows; 1. ERC findings of choledocholithiasis are filling defects by stoneor stones, dilation of common hepatic as well as common bile ducts and findings of cholangitis. 2. ERC findings of cholangitis are dilatation of larger intrahepatic biliary radicles and acute peripheral tapering, decrease of arborization, increased or right angle branching pattern, straightening and rigidity as well as irregular narrowing of intrahepatic biliary trees. This findings are observed in majority of choledocholithiasis. 3. Over9mm in diameter at intraprancreatic portion of common bile duct was regarded as abnormal, with 95% sensitivity,85% specificity and 91% diagnostic accuracy by decision matrix analysis. 4. In the presence of dilatation of CBD and findings of cholangitis in ERC, one should consider choledocholithiasis in spite of absence of stone defect.

7.Changes in Pain, Mood and Posttrauma Risk Level of Patients with Acute Hand Microsurgery.

PURPOSE: The purposes of this study were to explore changes in pain, mood and the level of posttrauma risk for 2 weeks after acute hand microsurgery and determine predictors of pain 2-week after microsurgery. METHOD: Using a sample of 84 patients with hand microsurgery, pain, mood, and posttrauma risk were measured by the Brief Pain Inventory-Korea, the modified Profile of mood states, and the Posttrauma risk checklist at 1-day, 1-week and 2-week post-microsurgery. Repeated measures ANOVA and Multiple regression analysis were conducted to evaluate changes in pain, mood, and posttrauma risk over time after the surgery and determine predictors of pain 2-week after the surgery. RESULTS: Pain significantly decreased (F=63.22, p<.001), mood significantly improved (F=41.04, p<.001) 2 weeks after microsurgery and interestingly, posttrauma risk significantly decreased from baseline to 1-week microsurgery but increased at 2-week after microsurgery (F=24.66, p<.001). Approximately 57% of the variance of pain 2-week post-microsurgery was explained by pain at 1-week post-microsurgery, mood and the numbers of injured fingers. CONCLUSION: The findings suggest the pain control for 1week after acute surgery being critical and posttrauma rehabilitation of injured finger being necessary. Developing nursing interventions is urgently needed to help individuals with impending hand microsurgery.

8.The Effects of Sexual Rehabilitation Education on the Knowledge of Sexual Activities and Satisfaction of Sexual Activities of Spinal Cord Injury Clients.

The purpose of this study is to identify the effects of sexual rehabilitation education on the knowledge of sexual activities and satisfaction of sexual activities of spinal cord injury clients by the method of Nonequivalent control group pretest-posttest group. The study period was between Jan. 6th and Apr. 11th, 1999. The experimental group was 21 spinal cord injury clients from one general hospital, and the control group was 21 spinal cord injury clients from three University hospital. Sexual rehabilitation education was done with manuals and video-tapes. Education time was 90 minutes a week for 4 weeks. The study tool was 20 questionnaires about sexual knowledge, designed Song, Chung Sook(1990) and 10 questionnaires from Derogatis's Derogatis Sexual Function Inventory for satisfaction of sexual activities(1979) translated by Jang, Soon Boke(1989). Analysis of data was done by x2 -test, t-test, repeated measures ANOVA and simple main effect with SAS/PC. The results are as follows. The 1st. hypothesis, "The group educated about the sexual rehabilitation has more sexual activities knowledge than those of the uneducated group." is significant statistically(F=12.06, p=0.001). The 2nd. hypotheses, "The group educated about the sexual rehabilitation has more satisfaction of sexual activities than those of the uneducated group." is significant statistically(F=5.04, p=0.030). According to the results, sexual rehabilitation education helps spinal cord injury clients to gain knowledge of sexual activities and satisfaction of sexual activities.

9.Size of Pancreas in Non-insulin-dependent Diabetes Mellitus: A Study Based on CT.

PURPOSE: To evaluate changes of pancreatic size with aging in control subjects and in non-insulin-dependent diabetic patients. MATERIALS AND METHODS: Two groups of non-insulin-dependent diabetic patients were examined; one had been treated with an oral hypoglycemic agent(n=59), and the other with insulin(n=56). The CT findings of 175 patients without clinical evidence of pancreatic disease were included as a normal control. RESULTS: In control subjects, pancreatic size and age correlated. The pancreas was smaller in non-insulin-dependent diabetics than incontrol subjects and smaller in insulin- treated non-insulin-dependent diabetics than in non-insulin treated patients. CONCLUSION: The pancreas was smaller in non-insulin-dependent diabetic patients than in control subjects within the same age range.

10.Angioleiomyoma: A Clinicopathological Study of 59 Cases.

BACKGROUND: Angioleiomyoma of the skin is a rare smooth muscle tumor and may develop anywhere in the body. Comprehensive clinicopathological analysis of angioleiomyomas developed in Korean has not been reported in domestic dermatological journals. OBJECTIVE: The purpose of this study was to evaluate the clinical and pathological characteristics of angioleiomyoma and the relationship between the tumor and the nerve with immunohistochemical stains. METHODS: Fifty nine cases of angioleiomyoma were reappraised from the files of the pathology of Seoul National University, Green Cross Reference Laboratory Center and Korea Veterans Hospital. Brief clinical histories of all cases were examined and microscopic findings in 35 cases were classified according to subtypes. Paraffin blocks were recut and stained in 16 cases for desmin, smooth muscle actin and S-100 protein. RESULTS AND CONCLUSION: 1. Male to female ratio was 1:1.27. 2. The age of the patients was mostly over 20 years and the mean age was 51 years in men and 40 years in women. 3. Tumors occurred most frequently in the lower extremity(38 cases), and followed by upper extremity(18 cases), head(2 cases), and trunk(1 case). Women had predominance of the legs in location site and men showed equal distribution in the lower and upper extremities.
