Abscence of right superior vena cava(SVC) in visceroatrial situs solitus is a rare(0.07% to 0.13%) congenital cardiovascular malformation, and little is known about the type and frequency of additional heart defects and arrhythmias. We reviewed previous publications and report the case of 8 month old male infant with absent right superior vena cava and persistent left superior vena cava and large secundum atrial septal defect, mild pulmonary valvular stenosis.

3.Computed tomography of calcification of the basal ganglia

Calcification so of the basal ganglia are rarely found at routine autopsies and in skull radiographs. CT is superior to the plain skull radiographs in detecting intracranial attenuation differences and may be stated to bethe method of choice in the diagnosis of intracranial calcifications. Of 5985 brain CT scans performed in KyungHee Univeristy Hospital during past 3 years, 36 cases were found to have high attenuation lesions suggesting within basal ganglia. 1. The incidence of basal ganglia calcification on CT scan was about 0.6%. 2. Of these 36 cases, 34 cases were bilateral and the remainder was unilateral. 3. The plain skull films of 23 cases showed visible calcification of basal ganglia in 3 cases (13%). 4. No specific metabolic disease was noted in the cases.

4.A Clinical and Statistical Study in 78 Cases of Ovarian Cancer.

Clinical and pathological studies were carried out on 78 patients of ovarian cancer from January 1985 to March 1991. The results are as follows : l. Among 78 patients of ovarian cancer, the age group from 51 to 60 years old was most common which was 18 cases(23.1%). 2. When classified according to the cell type, epithelial cell origin cancers were 59 cases (75.6%), ovarian cancers from germ cell origin 9 cases(11.5%), sex cord stromal cancers 3 cases(3.8%), and metastatic cancers 7 cases(8.9%) respectively. 3. When classified according to the FIGO staging, stage I were 34 cases(43.5%), stage II 8 cases(10.3%), stage III 23 cases(29.5%) and stage IV 13 cases(16,7%), respectively. 4. Preoperative mean CA 125 value of stage I ovarian cancer was 130.2+/-57.3U/ml, stage II 74.2+/-144.4U/ml, stage III 376.9+/-296.8U/ml and stage IV 433.9+/-148.2U/ml respectively. and Also preoperative mean CEA value of stage I ovarian cancer was 3.4+/-3.2ng/ ml, stage II 6.9+/-4.3ng/ml, stage III 4.7+/-4.4ng/ml and stage IV 4.6+/-6.3ng/ml respectively. 5. When classified according to the physical examination, the most common finding was palpable mass which were 38 cases(43.7%), the second, abdominal pain 30 cases(38.4%) and the third, abdominal distension 21 cases(26.9%) 6. When classified according to the operation, ipsilateral oophorectomy was 15 cases(25.5%), ipsilateral oophorectomy and contralateral ovary wedge resection 6 cases(10.2%), total abdominal hysterectomy 17 cases(28.8%), total abdominal hysterctomy and omentectomy 14 cases(23.7%), total abdominal hysterectomy and omentectomy and debulking tumor resetion 4 cases(6.7%) and biopsy only 3 cases(5.1%) respetctively. 7. When calssifide according to the postoterative endometrial finding, atrophic pattern was 28 cases(65.7%), proliferative pattern 10 cases(28.6%) and secretory pattern 2 cases(5.7%) respectively.

5.Influence of Droperidol on Postoperative Nausea and Vomiting during Patient-Controlled Analgesia with Fentanyl.

INTRODUCTION: Patient-controlled analgesia (PCA) has become an important means for postoperative analgesia with parenteral opioid, but postoperative nausea and vomiting (PONV) remains a major problem using a PCA system. The present study was designed to assess the antiemetic effectiveness of droperidol in patients using an intravenous PCA during the first 24 hours after surgery. METHODS: For the postoperative analgesia, 80 patients underwent orthopedic surgery were randomly allocated to receive PCA with either a mixture of 3.75 mg droperidol and fentanyl 2500 microgram after initial bolus of 1.25 mg droperidol as the FD group, or a mixture of saline and fentanyl 2500 microgram as the FS group at the end of surgery. RESULTS: The incidence of PONV showed 52.5% in FS group and 15% in FD group respectively (P<0.05). The incidence of PONV was 2~3 times more in women (76% in FS group & 25% in FD group) than men (35% in FS group & 9% in FD group). CONCLUSION: The addition of droperidol to fentanyl in a PCA system reduces the PONV during the first 24 hours after surgery.

6.Evaluation of Diagnostic Efficiency of Troponin T Measurement in Acute Myocardial Infarction and Ischemic Heart Disease.

BACKGROUND: Troponin T(TnT), a 37 kDa polypeptide subunit of contractile protein, is part of the troponin complect in striated muscle, where it binds to tropomyosin. TnT is not usually found in circulating blood, but increase in serum by cytoplasmic damage. Because the amino acid sequence is unique to cardiac muscle, one can immunologically differentiate skeletal muscle and cardiac protein isoforms. METHODS: We evaluated serum cardiac TnT (ELISA, Troponin-T, Boehringer Mannheim, Germany) versos CK-MB mass (IMX CK-MB, Abbott Laboratories, USA) in 46 cases (173 samples) of acute myocardial infarction, 28 cases (94 samples) of ansi na pectoris, 23 cases (62 samples) of other cardiac disease, and 32 cases of non-cardiac disease from September 1994-June 1996. RESULTS: TnT was detected in serum(cutoff value 0.20 ng/ml) within 6 hours after onset of chest pain, slightly earlier than CK-MB mass (cutoff value 5.0 ng/ml). Sensitivity of TnT (81%) is not statistically different from CK-MB mass (84%) within 24 hour of cutest pain but more sensitive after 24-72 hours of symptom, Specificity (79.5%) and positive predictive value (70.7%) of TnT were superior to that of CK-MB mass within 24 fours of cutest pain, and persistent to 6 days, which was longer than that of Cl4-MB mass. More patients show increment of TnT in unstable angina pectoris(40.0%) than stable angina pectoris(15.4%), No difference in detection rate of TnT between angina pectoris(28.6%) and other cardiac disease patients(34.8%). Only 3.1 % of non-cardiac disease patients show TnT increment. CONCLUSIONS: We concluded that TnT Is not detected In most of non-cardiac disease patients, and is an early and later marker with very wide diagnostic time win-dow. Also, TnT can be used as a valuable masker for ischemic myocardial damage in any underlying causes.

10.The Effects of Sexual Rehabilitation Education on the Knowledge of Sexual Activities and Satisfaction of Sexual Activities of Spinal Cord Injury Clients.

The purpose of this study is to identify the effects of sexual rehabilitation education on the knowledge of sexual activities and satisfaction of sexual activities of spinal cord injury clients by the method of Nonequivalent control group pretest-posttest group. The study period was between Jan. 6th and Apr. 11th, 1999. The experimental group was 21 spinal cord injury clients from one general hospital, and the control group was 21 spinal cord injury clients from three University hospital. Sexual rehabilitation education was done with manuals and video-tapes. Education time was 90 minutes a week for 4 weeks. The study tool was 20 questionnaires about sexual knowledge, designed Song, Chung Sook(1990) and 10 questionnaires from Derogatis's Derogatis Sexual Function Inventory for satisfaction of sexual activities(1979) translated by Jang, Soon Boke(1989). Analysis of data was done by x2 -test, t-test, repeated measures ANOVA and simple main effect with SAS/PC. The results are as follows. The 1st. hypothesis, "The group educated about the sexual rehabilitation has more sexual activities knowledge than those of the uneducated group." is significant statistically(F=12.06, p=0.001). The 2nd. hypotheses, "The group educated about the sexual rehabilitation has more satisfaction of sexual activities than those of the uneducated group." is significant statistically(F=5.04, p=0.030). According to the results, sexual rehabilitation education helps spinal cord injury clients to gain knowledge of sexual activities and satisfaction of sexual activities.
