1.Experience of reorganizing life in married immigrant women with chronic disease; With three Asian countries at the center

Purpose:
The purposes of this study was to describe the lived experience structures of married immigrant women with chronic diseases in reorganizing their lives in a variety of situations and contexts.
Methods:
This study applied grounded theory approach, and the participants were 15 married immigrant women with chronic disease. Data were collected through individual in-depth interviews and then analyzed by the method in Strauss and Corbin (1998).
Results:
The extracted data were organized 41 concepts, 21 subcategories, and 8 categories. The central phenomenon was determined to be “Life turned into a chronic disease.” The causal conditions were “Undergoing physical and mental stress from marriage”, “Lack of information on chronic diseases.” The extracted contexts was “The demand for health resources.” action and interaction strategies, “Finding problems and solutions.” The intervention conditions influencing the strategies was “Establishing a support system.” The result of such action was “To settle down in a way of life that suits your conditions.”
Conclusion
By providing a comprehensive and integrative understanding of how married immigrant women with chronic disease reorganize their lives, the study is expected to contribute to the development of social systems and national policies.
2.Experiences of Precocious Puberty in Primary School Girls with Hormone Therapeutics

PURPOSE: The purpose of this qualitative study was to identify the nature of precocious puberty and to explore what it means in primary schools.METHODS: The participants of this phenomenological study were nine primary school girls who were diagnosed with precocious puberty and experienced hormone therapeutics, applying a convenience sampling method. Data were collected from July 2017 to January 2018 through individual in-depth interviews of the participants, including gestures, facial expressions and nonverbal means. The data analysis followed the method of Giorgi.RESULTS: The study identified 37 concepts, 12 clusters, and five themes from experiences of precocious puberty. The essential five themes were as follows: ‘ashamed and concealing experience’, ‘there is no therapeutics option’, ‘difficulties in the process of therapeutics’, ‘difficulties in daily life’, and ‘ambivalence toward therapeutics’.CONCLUSION: The findings of this study indicate physical, psychological and social difficulties faced by girls with precocious puberty. Based on this results of the study, it is necessary to develop nursing intervention programs focusing on healthy growth and development for children with precocious puberty.

3.Nursing Students' Perceptions on Diet and as Environmental Factors Related to Cancer Risk Factors.

The purpose of this study was to identify how students majoring in nursing perceive cause of cancers and the effects of diet for preventing cancers. Data for the study were collected by 651 nursing students, who were registered in the second and third year in three technical colleges and third and fourth year in two universities. The research instruments included items on general characteristics of subjects, items about the degree of perception of the frequency of cancer onset and items on the perception of mortality, risk factors, preventive diets, knowledge, and high risk factor for cancer in specific body areas. The findings of this study are as follows : 1. Almost all subjects(92.8%) reported that the frequency of cancer onset increases and that it is 93.9% for people over 40. Degree of perception about cancer mortality was low at 33.0%. 2. As far as the perception of risk factors for cancer onset was concerned, smoking, stress, heredity, family history, and alcohol were rated high, over 80.0%. Risk factor in clouding, virus, hormones, pesticides were rated as low. 3. As to the perception of risk factor for body area as associated with diet salted and scorched food were rated at 44.5% for stomach cancer, alcohol, 50.4% for liver cancer, smoking, 72.8% for lung cancer, pregnancy times, 25.3%, and marriage age, 23.0% for uterine cancer, and no delivery experience, 40% for breast cancer. 4. The knowledge score for cancer was between 12 and 36, with a mean score of 26.75(SD=4.13). There was a statistically significant difference between experience in caring for cancer patients during clinical practice and knowledge score(t=3.09, p=.002).

4.Development and Evaluation of Community-based Respite Program for Family Caregivers of Elders with Dementia.

PURPOSE: Purposes of this study were to develop a community-based respite program for family caregivers and to test the effects of the program. METHODS: Focus group interviews were performed to extract meaning of respite care for family caregivers (13 participants) and a survey was done to identify respite needs of family caregivers (157 participants). The community-based respite program for family caregivers was developed based on results of the focus group interview and survey. The program was used with 41 participants (19 experimental and 22 control). Independent t-test and Mann-Whitney U-test were used to test differences between control and experimental groups for respite needs, burden of caregivers, subjective wellbeing, social support, fatigue and functional status of elders with dementia. RESULTS: There were statistical differences in caregiver burden, subjective wellbeing, and social support after the program, but, none for respite needs, fatigue and functional status of elders with dementia. CONCLUSION: The results indicate that a respite program can be useful to decrease burden of caregivers and increase subjective wellbeing and perceived social support of family caregivers in community settings. Further intervention research is needed to increase the functional status of elders with dementia and decrease fatigue in caregivers.

5.Ultrasound-guided central cluster approach for the supraclavicular brachial plexus block: a case series.

There are many different approaches to ultrasound-guided supraclavicular brachial plexus block (US-SCBPB), and each has a different success rate and complications. The most commonly performed US-SCBPB is the corner pocket approach in which the needle is advanced very close to the subclavian artery and pleura. Therefore, it may be associated with a risk of subclavian artery puncture or pneumothorax. We advanced the needle into the central part of the neural cluster after penetrating the sheath of the brachial plexus in US-SCBPB. We refer to this new method as the "central cluster approach." In this approach, the needle does not have to advance close to the subclavian artery or pleura. The aim of this study was to evaluate the clinical outcomes of the central cluster approach in US-SCBPB.

6.The Link between Health-related Physical Fitness Level and Cardiovascular Disease-related Risk Factors.

PURPOSE: The purpose of this study was to identify the link between health-related physical fitness level and cardiovascular disease-related risk factors in adult male workers. METHODS: We tested cardiovascular disease- related risk factors (waist circumference, SBP, DPB, fasting glucose, TC, HDL-C, TG, LDL-C) and health-related physical fitness (VO2max, grip, Sit-up, Flexibility, Body fat) and divided health-related physical fitness level of the subjects into 3 groups - A (very good, n=56), B (good, n=59), and C (below-average, n=57) according to the criterion of the Health and Fitness counseling guidelines of KOSHA. The statistical techniques such as standard deviation, one-way ANOVA and multiple regression (p<.05) were used. RESULTS: There were significant differences between group C and group B & A (p<.001) in waist circumference, DBP, Fasting glucose, HDL-C, TG, LDL-C. In TC/HDL-C, TG/HDL-C, LDL-C/HDL-C, Group C was higher than group B and A. CONCLUSION: On the basis of these results, we identified that improvement of health-related physical fitness level positively effects on the decrease of cardiovascular disease-related risk factors.

Intradermal injections of indigo carmine for sentinel node mapping are considered safe and no report of an adverse reaction has been published. The authors described two cases of profound hypotension in women that underwent breast-conserving surgery after an intradermal injection of indigo carmine into the periareolar area for sentinel node mapping.

Sarcoidosis is a chronic multisystemic disorder of unknown cause characterized by presence of noncaseating epithelioid granuloma in affected organ It is relatively common In western countries, but it is uncommon in East Asia including Korea. The lung is the most frequently affected organ. Usual radiologic manifestation of thoracic sarcoidosis art readily recognized, but there are extremely diverse manifestation Sarcoidosis rarely causes large pulmonary nodules, and cavitation in such lesion is very rare. We experienced one case of Sarcoidosis with Cavitary nodule of the lung and report it with brief review of the literature.

9.Development of a PDA based Personal Asthma Management System.

OBJECTIVE: The purpose of this study was to self-management asthma including symptom, medication, and to prevent the exacerbation of the condition using the Personal Asthma Management System(PAMS) which was developed based on Personal digital Assistant(PDA). METHODS: A development tool of the PAMS was EVC(Microsoft Embadded Visual C++). The system contents referred to workshop report of the Global Initiative for Asthma, on-line lecture of the Korean Society of allergology and Korea Asthma allergy Foundation, asthma-related web-sites. RESULTS: The PAMS was implemented with four main menus: personal information, symptom, medication management, and self care. The patients input their daily condition, and this data will be used in the successful prevention and management of asthma exacerbation. This real-time nature of PAMS is its primary advantage over other retrospective computing system for reducing the effects of forgetting, so this information will be useful in classifying of asthma severity. CONCLUSION: The PAMS can be contributed to aid self-management of asthma and effective treatment by attending physician.

10.Plasma Atrial Natriuretic Peptide Concentration after Birth in Low Birth Weight Neonates.

PURPOSE: Atrial natriuretic peptide(ANP) is a hormone with diuretic and natriuretic properties that is released by the atrial stretch and plays an important role in sodium and volume homeostasis. We studied plasma ANP concentration and the influence of ANP on sodium balance after birth in low birth weight neonates on the basis of 34 wks gestational age when the nephrogenesis was completed. METHODS: Twenty low birth weight neonates without congenital heart disease, respiratory insufficiency, renal disease or sepsis born between June 1997 and December 1997 at Korea University Guro Hospital were enrolled in this study. Blood sampling for ANP was done at 6 hr, 12 hr, 24 hr, 3 days, 4 days and 10 days after birth. FENa was calculated by blood and urine electrolyte and creatinine. We analyzed the correlation of plasma ANP concentration and fractional Na excretion rate(FENa). RESULTS: Plasma ANP concentration of low birth weight neonates less than 34 wks was 63.67+/-12.94pg/ml at 6 hr after birth and peaked at 24 hr(110.67+/-6.34pg/ml). Thereafter, it gradually decreased and reached 42.43+/-21.89pg/ml at 10 days(P<0.05). Plasma ANP concentration of low birth weight neonates more than 34 wks was 25.50+/-8.22pg/ml at 6 hr after birth and peaked at 12hours(152.67+/-39.93pg/ml). Thereafter, it gradually decreased and reached 42.78+/-17.67pg/ml at 10 days(P<0.001). And plasma ANP concentration did not correlate significantly with FENa in low birth weight neonates less than 34 wks(r=0.02, P=0.09), but there was good correlation between plasma ANP and FENa in low birth weight neonates more than 34 wks(r=0.6, P<0.001). CONCLUSION: From the above results, it is concluded that ANP influences renal Na excretion after 34-week gestational age when the development of distal tubule, containing ANP receptors, is rapid and contributes to body fluid and Na homeostasis.
