1.Roentgenological findings of histiocytic medullary reticulosis in the lung

Histocytic medullary reticulosis, known as malignant histiocytosis, is a disorder characterized by a systemicproliferation of morphologically abnormal histiocytes with prominent erythrophagocytosis. Fever, wasting, lymphdenopathy, hepatosplenomegaly and pancytopenia are the main clinical pictures. Prognosis is very poor andinvariably die within several months. Authors present radiological findings of pulmonary involvement in 5 cases of histologically proven histiocytic medullary reticulosis. The findings are progressive interstitial, alveolar ormixed infiltrations, small amount of pleural effusion, hilar and/or mediastineal lymph node enlargement and cardiomegaly. Histiocytic medullary reticulosis should be borne in mind in differential diagnosis of any patientswho has unexplanable above radiological findings.

2.Stages and Processes of Smoking Cessation among High School Students in Rural Community.

PURPOSE: The purpose of this study was to describe stage distribution of current and former smokers and to analyze significant variables that used by adolescents at each stage using the Transtheoretical Model. METHODS: A cross-sectional study was performed with 145 students attended to five high schools in one rural area. The survey instruments included stages and processes of change, decisional balance, self-efficacy, and knowledge related to smoking. RESULTS: Among the participants, 47.6% were in the precontemplation and contemplation stages, 20.7% were in the preparation stage, and the other 31.7% were in the action and maintenance stages. Analysis of variance tests revealed that behavioral processes, pros and cons of smoking, and self-efficacy were significantly associated with the stages of smoking cessation, while among 10 processes of change, self-reevaluation, self-liberation, and reinforcement management were significantly different by the stages. CONCLUSIONS: The study results suggest that smoking cessation programs should emphasize behavioral strategies, self-efficacy, and decisional balance to help students adopt and maintain smoking cessation

3.Study on Korean Daily Walking Activity <1>.

Since there is no report of daily walking activity of Korean, authors studied on normal adult daliy walking activity in korean of various kinds of occupation, personal life and patients. Total 134 cases were studied and in which there were 10 patients, 7 retired elderlies, 4 house wives, 48 employees of compaines, 30 nurses, 4 laboratory technicians, 2 pharmacists, 7 house staff doctors, 12 chief doctors, 4 cases of hicking and 6 cases of 18 holes golf players. The daily walking steps were measured with pedometer in 167 times in total and following results were, obtained. 1. Bed rest patients ; less than 1000 steps/day 2. Retired elderly ; 2,724(2,150-3,500) steps/day 3. House wife ; 5,437(3,030-7,920) steps/day 4. High ranking, old clerk and chief doctor ; 5,000-6,000 steps/day 5. 5.Interpretation of DNA Histogram in Flow Cytometry: A Comparative Study of DNA Ploidy in Fresh and Paraffin-embedded Tissues of Colorectal Adenocarcinomas.

As flow cytometric analysis using paraffin-embedded tissue was developed by Hedley et al in 1983, retrospective study with large amount of archival material was possible. Many literatures reported that the result of paraffin embedded tissue was compatible with that of fresh tissue. We compared the DNA histograms of 26 cases of colorectal adenocarcinoma in which the analysis was done in both fresh and paraffin-embedded tissues. Aneuploidy in fresh and paraffin-embed-ded tissues was 73.0% and 50.0%, respectively. The concordance rate of fresh and paraffin-em-bedded tissues was 76.8% and six interpreters were agreed in 73.0% of the cases. Because flow cytometric DNA analysis using fresh tissues can detect more aneuploid population than in paraffin-embedded tissue, the former is strongly recommeded in DNA ploidy study. Also careful observation using standard criteria may improve the interpretation of DNA histogram.

6.Anesthetic Experience with 14,337 Cases - Trends of Anesthesia .

To evaluate historical trends, anesthetic experiences of 14, 337 in the total performed at the Korea University Hospital from Jan. 1966 through Dec. 1977 were analyzed statistically according to age, sex, department, methods, physical status, duration of anesthesia, induction agents, main agents and anesthetic techniques. The results were as follows: 1) General anesthesia has been used as almost the main method of anesthesia. 2) Patients were about half in the 21~40 group of age. 3) Male patients were 2. 2 times more than females. 4) The most largest department for surgery was of patients in general surgery. 5) According to the ASA, classification of physical status, patients in class 1 were 43. 5%, and elective surgery to emergency ratio was 1. 3 to 1. 6) Patients in the 1~ 2 hour duration group were 41. 7%. 7) Thiopentone has mainly been used for intravenous induction. 8) Ether had mainly been used for a main anesthetic agent until 1970, but balanced anesthesia has been used with increasing favor since that time. 9) In almost all cases, a semiclosed circle absorption system has been employed and non- rebreathing system has recently been used with increasing favor for pediatric anesthesia.

8.Diagnosis of Central Venous Catheter-Related Sepsis Using Differential Quantitative Blood Cultures.

BACKGROUND: When central venous catheter (CVC) related sepsis is suspected based on clinical symptoms, removal of catheter is both diagnostic and therapeutic, but this approach leads to wastage of many sterile lines. Therefore, a reliable method to diagnose or exclude CVC sepsis without catheter removal is desirable. We performed differential quantitative blood cultures for the diagnosis of CVC related sepsis, in catheterized patients who had previous positive blood cultures. METHODS: Differential quantitative blood cultures were performed by collecting the blood specimens simultaneously via catheter and peripheral vein in 1.5 mL Isolator tubes (Wampole, USA). Sixty-three samples from 61 catheterized patients were taken and the colony counts from catheter blood samples were compared with those from peripheral samples. RESULTS: In 17 samples (27%), the colony counts from catheter blood samples were 5-fold higher than those from peripheral samples (the C/P ratio, > OR =5), suggesting CVC related sepsis; in 7 samples the C/P ratio was below 5, suggesting that sepsis was not CVC related. Of 35 samples (56%) in which no organisms were cultured, 2 samples were diagnosed as CVC related sepsis by the catheter tip culture. In 19 cases with proven CVC related sepsis, Candida spp. (n=8) and Gram-negative rods (n=7) were the predominant causative organisms and 16 cases (84%) were improved after catheter removal. CONCLUSIONS: This data show that quantitative blood culture method using Isolator may be very useful for diagnosing CVC related sepsis, especially in patients with positive blood cultures.

9.Statistical Estimate Formula Using Prognostic Factors for Predicting Hearing Recovery in Idiopathic Sudden Sensorineural Hearing Loss.

Sudden deafness is not rare but its pathophysiology has not been fully determined and treatment is controversial at present. About concerning factors for the recovery of sudden deafness, various studies have been done, but there are still many unknown things. The probability of recovery from sudden deafness depends on many factors. To evaluate hearing recovery and prognostic factors we studied 65 patients treated with bed rest, steroid, vasodilator, plasma expander, vitamines from March 1988 to October 1995 at our department. Data were statistically examined and we present a model for calculating the probability of patient recovery based on the combined effects of risk factors determined for these variables. Interrelationships among the prognostic factors and their relative importance in predicting hearing recovery are discussed.

10.Biodistribution of 3-<131I>iodo-O-mythyl-L-alpha-methyltyrosine in Tumor Bearing Rats: A Comparison Study with L-3-<131I>iodo-alpha-methyltyrosine.

PURPOSE: The aim of this sutdy was to evaluate the feasibility of 3-[131I]Iodo-O-methyl-L-a-methyltyrosine ([131I]OMINT) as an agent for tumor image. MATERIALS AND METHODS: After synthesis of 4-O-methyl-L-a-methyltyrosine (OMAMT), OMAMT was labeled with 131I using Iodogen method. In viro cellular uptake study was performed using 9 L gliosarcoma cells at various time points upto 4 hr. The biodistribution (five rats implanted with the 9 L gliosarcoma cells per group) was evaluated at 30 min, 2 hr, 24 hr after iv injection of 3.7 MBq [131I]OMIMT or L-3-[131I]iodo-a-methyltyrosine ([131I]IMT). Gamma camera images were obtained at 30min, 2 hr, and 24 hr. RESULTS: [131I]OMINT uptake was 3.3 times and 2.5 times higher than [131I]IMT uptake at 30 min and 60 min, respectively and same after 2 hr in in vitro sutdy using 9L gliosarcoma cells. Maximum accumulation in tumor occurred at 30 min for both [131IOMINT and [131I]IMT in tumor bearing rats. The tumor uptake of [131I]OMINT was significantly higher than that of [131I]IMT in tumor bearing rats. The tumor uptake of [131I]OMIMT was significantly higher than that of [131I]IMT at early time point studied (3.74 +/- 0.48 vs 0.38 +/- 0.17% ID/g at 30 min and 2.40 +/- 0.17 vs 0.24 +/- 0.03% ID/g at 2 hr, respectively, p<0.01). However, the tumor uptake of both radiolabels were not significantly different at 24 hr (0.04 +/- 0.01 vs 0.05 +/- 0.01% ID/g). Tumor was visualized as early as at 30 min in gamma camera images. CONCLUSION: These data suggested that [131I]OMIMT might be a useful tumor imaging agent and has more advantage for the tumor imaging compared to [131I]IMT.
