1.Occlusal analysis using T-scan system after open reduction of mandibular fracture patients.

During ideal physiologic occlusion in the intercuspal position, all molars are occluded simultaneously and each occlusal contact evenly transmits similar pressure. The purpose of this study is to analyze the occlusion in the mandibular bone fracture patients treated by open reduction. This patients were selected according to the following criteria: 1) complete dentition without any missing teeth, 2) relatively normal occlusion without original TMJ disorder. Occlusal analysis using T-scan system were performed in 22 patients and 20 normal subjects. Our results were followed: 1. The number of occlusal contacts and the magnitude or occlusal force were increased with time. (p<0.05). 2. In the unilateral mandibular fracture patients, occlusal force and occlusal contacts were increased with time on the both sides. 3. There was significant difference in the number of occlusal contacts and occlusal force between postoperative 1 month and normal group(P<0.05), but there was no significant difference in the number of occlusal contacts and occlusal force between postoperative 6 month and normal group(p>0.05).

2.The Metabolic Acidosis and Clinical Outcome in Acute Infantile Diarrhea.

To determine the types of metabolic a cidosis using anion gap in acute infantile diarrhea and to correlate it with clinical outcome, we examined 103 infants admitted with acute diarrhea. The serum electrolytes (sodium, potassium, chloride, phosphorus), creatinine, CO2 content and anion gap were measued on first admission day. They were classified group A with normal anion gap (8~16mEq/L) and group B with increased anion gap (>16mEq/L). The results were as follows. 1) The number of group A with normal anion gap (11.6+/-3.3mEq/L) was 62 and the number of group B with incresed anion gap(21.1+/-5.5mEq/L) was 38. 2) The duration of diarrhea was significantly prolonged in group B (9.0+/-2.5 days), compared with group A (5.9+/-1.1 days)(p<0.001). 3) The duration of admission was significantly prolonged in group B(5.6+/-2.2 days), compared with group A (4.+/-01.4days)(p<0.001). 4) Infants in group B, compared with group A, were significantly more severe dehydrated (p.

3.Clinical and Hypotensive Experiences with Diltiazem Hydrochloride(Herben(R))in Essential Hypertension.

30 patients with essential hypertension were treated with diltiazem (Herben(R))significantly decreased both systolic and diastolic blood pressuer and heart rate. Mean decrease in systolic & diastolic blood pressuer were 38.0+/-4,5mmHg and 16.3+/-2.3 mmHg respectively and mean drop in heart rate was 17+/-2 beats/mm. The side effects of diltiazem (Herben(R)) were mild headache, fatigue and pruritus, but those were insignificant. Diliazem may be of benefit to hypertensive patients, and particularly useful when hypertension occurs in association with coronary artery disease.

6.Real-time ultrasonographic findings of gallbladder empyema

Although the ultrasonography has been regarded as a main procedure in the diagnosis of gallbladder diseases, no many papers concerning the ultrasonographic findings of gallbladder empyema appeared in the literature. Twenty-four cases of surgically proven gallbladder empyema were studied by ultrasonography in our hospital during last 15 months. The results were as follows; 1. The size of gallbladder was enlarged in 18 cases. 2. The thicknessof gallbladder wall was more than 3mm in 16 cases. 3. The echogenicity of gallbladder wall was diminished in all cases. the inner margin of gallblaldder wall was also ill-defined in all cases. These findings are considered to be important in the diagnosis of gallbladder empyema. 4. The internal echogenicity of gallbladder (diffuse or localized) was positive in 16 cases and negative in the remainders. The echo genility was thought to beproportional to the amount of pus in gallbladder. 5. The 7 cases presented periocholecystic sonolucent area hadperforation of gallbladder and pericholecystic abscess. 6. Gallstone was found in 17 cases.

8.Real time ultrasonography in obstructive jaundice

Ultrasonography is a predominantly accurate, relatively simple unique diagnsotic method of obstructive jaundice. The ultrasonographic findings of obstructive jaundice are dilated intra- and extrahepatic duct with intraluminal hyperreflective echo or mass in and/or around the bile duct. The superiority of high resolution real time ultrasonography for the diagnosis of obstructive jaundice is based on the easy detectability of extra-and intrahepatic bile ducts by its multiple sectional images in a short time, the flexibility of probe and the small crystal size. Author evaluated real time sonographic findings 46 obstructive jaundice patients confirmed by surgery or radiographical examinations. The results were: 1. Diameter of extrahepatic duct in obstructive jaundice were varied from normal to 4.0 cm, mostly 8 to 10 mm in diameter (26%). Degree of dilatation of biliary duct appeared more prominent in cancer patients than any other causes of obstruction. 2. The site of obstruction was detected in85% (39/46) and its common site was common bile duct in 63% (29/46). 3. The diagnostic accuracy of choledocholithiasis and cancer was 82% (22/27) and 44% (4/9), respectively. Diagnostic accuracy of the real time ultrasonography in obstructive jaundice was over all 75% (34/46).

9.Diagnostic value of Tc-99m sulfur colloid imaging and comparative evaluation with ultrasonography in liver disease

A Tc-99m sulfur colloid hepatic scintigraphy is often the first imaging modality empolyed in the evaluation ofthe patient with suspected liver disease,since the hepatic scintigraphy is not only highly sensitive, low expenseand easy of performance but also provides both structural and functional information of the liver. The authoranlayzed the scintigraphic findings in 304 patient proven various hepatic disease and 58 normal liver, and alsoretrospectively analyzed the result of hepatic scintigraphy and ultlrasonography of the liver in 117 patients. Theresults were as follows; 1. The overall sensitivity and specificity of hepatic scintigraphy in the liver diseaseis 91% and 67%, respectively. 2. On the evaluation of the diffuse parenchymal parenchymal disease of liver, thescintigiraphy was found to be highly sensitive (88%) and also specific image patterns were found in cirrhosis. 3.The hepatic scintigraphy was highly sensitive (92%) in the detection of the focal lesions of liver. 4. Theultrasonography was capable of differentiating solid and cystic masses which were detected on scintigraphy, whilescintigraphy was more sensitive in detection of hepatocellular disease. 5. Tc-99m sulfur colloid imaging reminedthe preferred inital screening method in patients with suspected liver disease, while ultrasonography should bedone for those patients with prior suspicious findings.

10.Real-time ultrasonographic findings of biliary ascariasis

Although the ultlrasonography has been regarded as a main procedure in the diagnosis of bile duct andgallbladder diseases, a few reports concerning the ultrasonographic findings of biliary ascariasis appeared in theliterature. Seven cases of real-time ultrasonographic diagnosis of biliary ascariasis were made in our hospitalduring last 15 months. In six, the diagnosis was confirmed by surgery or radiographic examination. The resultswere as follows; 1. The characteristic long, thick, highly reflective echo without distal acoustic shadowing inthe extra and/or in tra-hepatic ducts were seen in all six proven cases. 2. The echogenic, nonshadowing images ofthe worms were seen as single stripe in five cases(containing a central longitudinal, anechoic tube, suggestingits digestive tract in three cases ) or multiple stripes in one case. But so-called coiled appearance or amorphousfragments were not seen in our cases. 3. Associated findings were as follows; mild to moderate dilatation of theextrahepatic duct in all six duct in 1 case, intrahepatic stone in 1 case, contracted gallbladder containingsingle stone in 1 case and evidence of cholecystitis in 4 cases. 4. One false positive diagnosis of ascariasis inthe gallbladder is considered due to reverberation artefact. No false negative was seen.
