Squamous cell carcinoma has occasionally been encountered in orthopedic practice as a complication of chronic osteomyelitis, burn scar and other lesions. The possible occurrence of this secondary change impose difficulty in the diagnosis and management of chronic orthopedic diseases. During 11 years from 1966 to 1976, 16 cases of squamous cell carcinoma were confirmed by histopathological examination and the results of clinical observation on them are summarized here. l. Of 16 patients, men were 15 and woman one. Their average age was 51.3 years 2. The primary lesions in which this malignancy developed were osteomyelitis (5 cases), and scar (5 cases), and scar after extensive laceration (2 cases). In one case no primary lesion was present. 3. Average duration of symptomatology of the primary lesion was 35.4 years in osteomyelitis and 31.2 yeary in burnscar. 4. Location of the lesion was in lower extremity in 15 cases, eight of which were in the leg. 5. Methods of treatment undertaken were; amputation (10 cases), chemotherapy (2 cases), chemotherapy and irradation (1 case). In three no treatment was done because of refusal. 6. The follow-up study was possible in five of them. Of four cases amputated, one was well alive for two years after ablation with no evidence of metastasis, the others two showed spread, to inguinal nodes in one and ribs in the other, after two years and the fourth died of unknown disease after two years. The fifth died, after initial remission with chemotherpy and irradation, of lung cancer after two years.

OBJECTIVE: This study was performed to find out mitochondrial deoxyribonucleic acid mutations in preeclampsia because Mendelian models fail to explain all the patterns of inheritance in preeclampsia. METHODS: Ten preeclampsia patients and two of their related family members who have the obstetric history of preeclampsia were studied. The mitochondrial transfer ribonucleic acidleu[UUR] gene was amplified using polymerase chain reaction, cut by a restriction endonuclease (Apa , and also sequenced to see the whole gene. RESULTS: There were neither the known mutation at Nucleotide 3243 nor other mutations on the mitochondrial transfer ribonucleic acidleu[UUR] gene in these objects. CONCLUSION: It seems that the known mutation of mitochondrial transfer ribonucleic acidleu[UUR] gene is not so frequently detected in preeclampsia of South Korean, But it could not be concluded how many South Korean women with preeclampsia have the mutation.

BACKGROUND: The regional fat distribution is known to be related with some cardiovasular risk factors independently to total body fat amount. Regional fat distribution pattern can be assessed by waist to-hip ratio(WHR). WHR distribution patterns depend on age, sex, race and show regional difference. But, our own unique data are inadequate, so we tried to investigate distribution patterns, cut off points and related factors of WHR in middle aged men and women among Korean. METHODS: The study population were middle aged men and women above 40 years old who visited our hospital for routine check up from April to July, 1996. We measured height, weight, blood pressure, waist circumference, hip circumference, fasting blood sugar and total cholesterol in 1,000 persons. We gathered data about diet habits, exercise, smoking and drinking status by standard questionnaire. We analized above data by using SAS for Window 6.10. RESULTS: The study population numbers of men and women were 494 and 499, and average ages of men and women were 49.3 and 50.5 years respectively. For men, the average WHRs as a whole were 0.895, and 0.893, 0.893 and 0.911 in their forties, fifties and above sixties respectively. For women, the average WHRs as a whole were 0.827, and 0.816, 0.837 and 0.850 in their forties, fifties and above sixties respectively. The percentage above WHR 1.0, 0.95, and 0.90 in men were 2.4%, 11.7% and 46% and the percentage above WHR 0.90, 0.85, and 0.80 in women were 8.2%, 32.9% and 68.7%. Adjusting by age, body mass index, WHR was significantly correlated with weight, systolic blood pressure, diastolic blood pressure in men only. For men, WHR was associated with alcohol drinking positively and with exercise negatively. For women, WHR was associated with alcohol drinking and smoking status positively. CONCLUSIONS: The WHR had a tendency to increase according to age. The 90th percentile points in men and women were 0.957 and 0.894 respectively. WHR was related with several cardiovascular risk factors and several life styles.

PURPOSE: The purpose of this study is to evaluate the gadolinium-enhanced MR imaging findings of cerebral venous angiomas, and to compare those findings with those of cerebral angiography. MATERIALS AND METHODS: We reviewed the findings of gadolinium-enhanced MR images of 11 cases with angiographically proved cerebral venous angiomas. We obtained Tl-and T2-weighted images with gadoliniumenhanced Tl-weighted images at a 0.5 T MR imager with a spin-echo technique. We analyzed the location and signal intensity of the lesion, pattern of contrast enhancement, the distribution of draining veins and medullary veins before and after contrast enhancement, and then compared with the findings of cerebral angiography. RESULTS: The characteristic central tubular signal void structure showed low signal intensity on Tl-weighted images and high signal intensity on T2-weighted images in all cases. After gadolinium enhancement, we could find a characteristic enhancement of all medullary and central veins in 7 cases. CONCLUSION: Gadolinium-enhanced MR image was useful in the diagnosis of venous angiomas especially in the detection of medullary veins. The invasive cerebral angiography could be replaced by this technique in this disease.

Onychomycosis caused by the genus aspergillus is extremely rare in Korea and only one case due to A. sydowi has been reported. We experienced two casea of onychomycosis infected with A. repens, which were healthy adults. The first case was a 29 year-old male who developed thickening and dark discoloratian of 1st, and, 3rd and 5th toe-nails of left foot from about 5 months on. The second case was a 27 year-old male who developed thickening and yellow-whitish discoloration of the nail of right thumb from about 3 rnonths on. In both cases, direct examination of the affected nails showed many thick bluish hyphae and spore chains, and A. repens was isolated. Our cases were first reported through the Korean literature.
