1.Spinal extradural angiolipoma: a case report.

Spinal extradural angiolipoma is a rare benign tumor containing vascular and mature adipose elements. The most common location of this tumor is the thoracic level with a tendency to involve several spinal segments. Almost all spinal extradural angiolipomas develop on the posterior aspect of the dura. The authors report a case of surgically proven spinal extradural angiolipoma involving the thoracic level and describe its CT findings.

2.A Review Study on Health Impacts of Agent Orange.

In order to understand epidemiologic characteristics of dioxins related health impacts, 107 diox in-related literatures published up to May, 1993 were thoroughly reviewed. The results obtained are as follows: 1. Review studies on dioxins can be classified into two types : one is studies concentrated on one specific subject related to dioxins such as the effects on skin, birth outcomes, metabolic machanisms in the human body, major sources of exposure to the general population, immunotoxicity, association between dioxins and cancers, and research methods on effects of dioxins. The other is review studies dealing with overall aspects of dioxins, among which the report published by Academy of Science of United Stateds of America in 1993 is the most recent one. 2. Judging by consistency of results and biological plausibility, there were three groups of diseases ; diseases having causal relationship, having possibilities of causal relationship, and having no causal relationship for the time being. 1) The diseases showing causal relationship were lung cancer, Hodgkin 'disease, non-Hodgkin 'lymphoma, soft tissue sarcoma, chlordane, and polyneuropathy 2) the diseases showing possibilities of causal relationship were stomach cancer, lymphatic and haematopoietic cancer, multiple myeloma, skin cancer, diseases of digestive system, and CNS deformity among children born to the people exposed. 3) The diseases showing no causal relationship were oral and laryngeat cancer, esophageal cancer, prostate cancer, kidney cancer, bladder cancer, brain and CNS cancer, diseases of circulatory and respiratory systems. There were numerous diseases of digestive system, which showed difficulty in establishing definite causal relationship owing to the lack of consistency among researchers. 3. Even though papers dealt with dioxinrelated mortality and morbidity studies showed high mortality rate from accident, biological plausibility explaining causal relationship between them should be sought further. 4. There was a high prevalence of hearing loss among Vietnam veterans, and it seemed rather related to the explosive noise in combat than dioxin itself. 5. The current TCDD levels of Vietnam veterans were almost the same as those of the general population.

3.Gd-DTPA Eenhanced IVIRI of the Cerebral Venous Angiomas: Cornparision with Cerebral Angiography.

PURPOSE: The purpose of this study is to evaluate the gadolinium-enhanced MR imaging findings of cerebral venous angiomas, and to compare those findings with those of cerebral angiography. MATERIALS AND METHODS: We reviewed the findings of gadolinium-enhanced MR images of 11 cases with angiographically proved cerebral venous angiomas. We obtained Tl-and T2-weighted images with gadoliniumenhanced Tl-weighted images at a 0.5 T MR imager with a spin-echo technique. We analyzed the location and signal intensity of the lesion, pattern of contrast enhancement, the distribution of draining veins and medullary veins before and after contrast enhancement, and then compared with the findings of cerebral angiography. RESULTS: The characteristic central tubular signal void structure showed low signal intensity on Tl-weighted images and high signal intensity on T2-weighted images in all cases. After gadolinium enhancement, we could find a characteristic enhancement of all medullary and central veins in 7 cases. CONCLUSION: Gadolinium-enhanced MR image was useful in the diagnosis of venous angiomas especially in the detection of medullary veins. The invasive cerebral angiography could be replaced by this technique in this disease.

7.A Case of Angioedema Probably Induced by Captopril.

Angioedema is a disorder characterized by well-demarcated nonpitting edema involving the tongue, floor of the mouth, larynx, lips, and face. The incidence of angiotensin converting enzyme(ACE) inhibitor related angioedema has been reported to be about 0.1% to 0.2%, and the time of onset is usually during the first week of therapy. These ACE inhibitors include captopril, enalapril, and lisinopril. A 53-year old man with an 8 month history of hypertension previously controlled with atenolol, was presented to the dermatologic department with angioedema of the face and tongue. He had begun therapy with captopril one day before this episode. Even though he was treated with epinephrine and methylprednisolone sodium succinate, the edema gradually progressed and finally dyspnea developed. He was urgently intubated and treated with steroids and pheniramine maleate in the intensive care unit. The edema resolved after 24 hours.

9.Gd-DTPA Enhanced MRI of the Brain Infarction: Correlation between Onset of Infarction and Enhancing Patterns.

PURPOSE: To evaluate the correlation between onset of brain infarction and Gd-DTPA enhancing patterns on MRI. MATERIALS AND METHODS: We reviewed MRI of 58 lesions in 45 patients with clinically documented brain infarction retrospectively. Axial, coronal and sagittal T1WI (TR/TE 450-520/20), T2WI (TR/TE 2190/90) and Gd-DTPA enhanced T1WI were performed with a 0.5T superconductive MR system. We analyzed Gd-enhancing patterns lhat were divided into intravascular, meningeal, and parenchymal enhancement. Parenchymal pattern was subdivided into mottled, partial ring like and dense enhancement. RESULTS: lntravascular enhancement was seen at 1-10 days in 30(53%) of 58 infarctions. Meningeal enhancement (13%) was noted at 1--6 days. Parenchymal enhancement (50%) was seen at 2--28 days and subdividing patterns are as follows:The mottled enhancement pattern was seen earlier at 2-8 days and partial ring like or dense enhancement patterns at 5-28 days. CONCLUSION: After reviewing Gd-enhanced MRI of infarction, the intravascular and meningeal enhancement patterns were earlier than parenchymal enhancement. Among parenchymal patterns, the mottled pattern was seen earlier than partial ring like or dense patterns. In conclusion, Gd-enhancing patterns of brain infarction are useful in estimating the age of infarction including acute infarction.

10.Glomerular Basement Membrane Thickness in Minimal Change Disease.

The thickness of the glomerular basement membrane may vary not only in glomerular disease, but also in normal persons according to age and sex. But there has been no data on the normal thickness of the basement membrane in Korea. This study was designed to determine the glomerular basement membrane thickness as a reference value according to age and sex, in 50 cases of minimal change disease obtained from patients aged 2~67 years. Measurement of glomerular basement membrane was made on electron micrograph using an image analyzer. The thickness of each case was estimated by the arithmetic and harmonic mean methods. The mean thickness of the glomerular basement membrane was 291.9 47.9 nm by harmonic mean method and 284.2 43.7 nm by arithmetic mean method. And the harmonic mean thickness of the glomerular basement membrane according to age was 249.1 32.5 nm (1~5 years), 256.6 45.3 nm (6~10 years), 279.2 57.9 nm (11~15 years), 303.2 43.8 nm (16~20 years), 335.3 37.5 nm (21~30 years), and 291.1 22.5 nm (over 30 years), respectively. There was a trend that the thickness of glomerular basement membranes increased with the age till 30 years of age. There was no significant sex-related difference. In conclusion, the mean glomerular basement membrane thickness is comparable to the data from western people and shows a trend of increasing thickness according to the age.
