1.The Effects of Dogbi(ST35) & Sulan Moxibustion on Knee Joint Pain, Range of Motion and Discomfort during ADL in the Aged.

PURPOSE: The purpose of this research was to test the effects of ST35 & Sulan moxibustion on knee joint pain, range of motion, and discomfort during Activies of Daily Living (ADL) in elderly with knee joint pain. METHOD: A nonequivalent control group pre-post test research design was used. Thirty four elderly who had knee joint pain were studied. Of them, sixteen were in the experimental group and eighteen the control group. The instruments used for this study were NRS, goniometer, and a modified ADL questionnaire developed by Lee. RESULT: The pain scores of right & left knee joint after moxibustion were significantly different between the experimental group and the control group (Right: F=26.27, p=0.000, Left: F=20.77, p=0.000). Right and left knee ROM scores were significantly different between the experimental group and the control group after moxibustion (Right: F=10.74, p=0.003, Left: F=9.239, p=0.005). Discomfort during ADL scores were significantly different between the experimental group and the control group after moxibustion (F= 32.31, p=0.000). CONCLUSION: It is necessary that nurses provide the elderly with knee joint pain with moxibustion to reduce joint pain and to increase knee ROM as an alternative therapy.

2.Histochemistry on the Effects of Sulfur Dioxide on Glycoconjugates of Rat Skin.

This experiment was performed the effect of the glycoconjugates of the skin in the Sprague-Dawley male rats exposed to SO2 for 1, 3 and 6 hours with various concentrations (10, 25, 50, 100 and 200 ppm). To investigate the change of glycoconjugates of skin, seven biotinylated lectins (DBA, SBA, PNA, BSL-1, sWGA, UEA-1, Con A) were applied with ABC method. The epidermis of experiment rats was more or less different according to the concentration of SO2, comparing control group, and beta-N-acetyl-D-galactosamine, alpha-L-fucose, alpha-D-mannose and alpha-D-glucose of the epidermis tend to be increased according to exposed SO2 time, but alpha-D-galactose was tend to be decreased. Galactose-beta-1, 3-Nacetyl- D-galactosamine was tend to more or less increase in horny layer of the epidermis, but decrease in granular and spinous layers. As contrasted control group, beta-N-acetyl-D-glucosamine in the dermis of experiment rats was decreased, but alpha- D-mannose and alpha-D-glucose were increased. In the hair follicle of the experiment rats, beta-N-acetyl-D-galactosamine and galactose-beta-1, 3-N-acetyl-Dgalactosamine tend to be decreased according to exposed SO2 time though more or less differ from the portions of the hair follicle, and beta-N-acetyl-D-glucosamine and alpha-L-fucose were found noticeably to increase comaring control group. According to exposed SO2 time, the decrease of alpha-D-galactose tend to be little more pronounced in the outer root sheath of the upper portion of hair follicle. In contrast, the same sheath of the upper portion in above follicle was increased relatively. alpha-D-mannose and alpha-D-glucose were increased in the root sheath of hair follicle and hair cortex, but decreased in the hair medulla. In the experiment rats, the beta-N-acetyl-D-galactosamine tend to be decreased gradually according to exposed SO2 time, but the decrease of galactose-beta-1, 3-N-acetyl-D-galactosamine were showed significantly as contrasted control group. beta-N-acetyl-D-glucosamine, alpha-D-mannose and alpha-D-glucose were increased gradually according to exposed SO2 time, but alpha-L-fucose was increased remarkably. The effect on lectin binding pattern of the glycoconjugates in the rat skin according to the concentrations and exposed time of SO2 was noted that the alternations of lectin binding pattern tend to be a little more pronounced in low concentration of exposed SO2 for a long time than in high concentration of exposed SO2 for a short time. The alternations of lectin binding pattern were appeared almost similar in concentrations of 50 ppm SO2 and above.

3.The impact of malnutrition on survival in patients with gynecologic cancer undergoing chemotherapy.

PURPOSE: Malnutrition is a major concern in patients with gynecologic cancer receiving chemotherapy. The aim of this study was to evaluate the prognostic significance of malnutrition in patients with gynecologic cancer undergoing chemotherapy. METHODS: A prospective, observational study was conducted on a total of 99 subjects who were treated at a tertiary hospital in Korea. Data regarding demographic, clinical, nutritional, and psychological characteristics at baseline and survival were obtained. RESULTS: Performance status, nutritional status, depression, and annual income were significantly different between survivors and non-survivors. Multivariate Cox modeling after adjusting for other factors showed that a malnourished status in patients with gynecologic cancer undergoing chemotherapy was a significant and independent negative influencing factor for survival. CONCLUSION: These findings provide evidence that adequate nutritional assessment and intervention may assist in improving survival in patients with gynecologic cancer undergoing chemotherapy.

6.A Case of Pycnodysostosis.

A case of pycnodysostosis in 12 years old girl was presented with a brief review ofliterature. This patient was admitted to our pediatric dept. of N.P.H. with chief complaints of loss of appetite and respiratory difficulty. She was characterized by a peculiar face with anterior bossing, exophthalmos, opened anterior fontanel and coronal suture and dwarfism. So bone series was performed and showed character of pycnodysostosis such as opened anterior fontanel and coronal suture, receding mandibular angle, inoreased bone density with narrowing bone marrow and acrcosteolytica on distal phalanx. But she died on the 25 th hospital day due to chronic renal failure.

7.Study on Correlations among Polysomnogram Indices for Obstructive Sleep Apnea-Hypopnea Patients.

BACKGROUND AND OBJECTIVES: The polysomnography is essential for diagnosis of obstructive sleep apnea-hypopnea (OSAH) and provides important objective information. But, in fact, there are some difficulties for comprehensive interpretation of multiple indices from the polysomnogram. This study was designed to find out which polysomnogram indices were significant to respiratory distress indices (RDI) and apnea index (AI) on statistical correlation and which other indices should be considered together for proper management plan apart from RDI and AI (respiratory index). SUBJECTS AND METHOD: We evaluated 40 patients whose RDI were over 5 as the result of overnight polysomnography before any management from February 2003 to September 2003 at Eulji medical center. By multiple linear regression analysis, we studied the relationship of RDI and AI as dependent variables to ST, O2 desaturation events (O2DE), lowest O2 saturation (LoO2), slow wave sleep portion (S3&S4), arousal index (ArI), body-mass index (BMI) and age. We also studied the relationship of ST as an dependent variable to O2DE, LoO2, S3&S4, ArI, BMI, and age. RESULTS: 1) There were significant correlations between RDI and O2DE, ArI as was true between AI and O2DE, ArI (p<0.05). And there was no significant difference in the degree of the correlation between RDI and AI. 2) Other variables such as ST, LoO2, S3&S4, BMI and age were not significant. 3) ST had no significant correlation with respiratory index and any other variables (p<0.05). CONCLUSION: Because the respiratory index may not the entire physical status during sleep for OSAH patients, multiple indices (such as LoO2, BMI, ST, S3&S4, age) should be considered together apart from the respiratory index for proper management plan.

BACKGROUND AND OBJECTIVES: Standard unfractionated heparin (UFH) has long been used to prevent death and myocardial infarction in patients with acute coronary syndrome and acute occlusion undergoing percutaneous revascularization. However, UFH binds to several plasma proteins, platelets, and endothelial cells producing a highly variable anticoagulant response. In contrast, Low molecular weight heparin (LMWH) exhibits less protein binding and provides more predictable anticoagulant response with reduced need for patient monitoring and dosage adjustment. The purpose of this study was to assess the anti-Xa activities of LMWH in Korean patients with acute coronary syndrome after recommended dose for caucasians and to determine an optimal method of administration of LMWH. MATERIALS AND METHODS: Twenty five patients with acute coronary syndrome were enrolled and allocated to five separate groups (5 patients in each group) by types according to molecular weight (LMWH (A): (molecular weight of 4500 daltons, LMWH (B): molecular weight of 6400 daltons) and methods of administration (Group 1A and 1B: Subcutaneous and subcutaneous injections (SC-SC), Group 2: Intravenous and subcutaneous injections (IV-SC), Group 3A and 3B: Intravenous, subcutaneous and subcutaneous injections (IV-SC-SC). Five groups were as follows: Group 1A: LMWH (A) 1 mg/kg SC every 12 hours, Group 1B: LMWH (B) 100 IU/kg SC every 12 hours, Group 2: LMWH (A) 1 mg/kg IV bolus and 1 mg/kg SC 12 hours later, Group 3A: LMWH (A) 0.5 mg/kg IV bolus, 3 hours later 1 mg/kg SC every 12 hours, Group 3B: LMWH (B) 50 IU/kg IV bolus, 3 hours later 100 IU/kg SC every 12 hours. Anti-Xa activity was measured by amidolytic assay method (Rotachrome, Stago, France) in 555 samples from 25 patients. All the data of anti-Xa activity in each group were plotted along the sequential time and mean values of them were analyzed by Wilcoxon signed rank test. RESULTS: 1)The anti-Xa activity (mean 0.6216+/-0.238 IU/mL) of LMWH (A) was greater than that of LMWH (B)(mean 0.2587+/-0.1709 IU/mL) in the conventional SC-SC method (p<0.001). 2) The anti-Xa activity of LMWH (A) (mean 0.6203+/-0.2383 IU/mL) was also greater than that of LMWH (B)(mean 0.468+/-0.2428 IU/mL) in the IV-SC-SC method (p<0.001). 3) More rapid and effective anti-Xa activities were achieved by IV-SC-SC method compared with conventional SC-SC method. CONCLUSION: This study suggests that immediate achievement and optimum maintenance of anticoagulant activity can be accomplished by IV-SC-SC method rather than conventional SC-SC method in patients of acute coronary syndrome.

Nephrocalcinosis is described as the deposition of calcium crystals in the renal parenchyma that result from prolonged states of hypercalcemia. Recently recognized is the deposition of calcium phosphate in the renal tubular injury by using sodium phosphate cathartics commonly used for the colonic cleansing. This phenomenon was termed phosphate nephropathy. Acute renal failure secondary to phosphate nephropathy has been increasingly recognized as a complication after the administration of sodium phosphate solution for colonoscopy. We report a case of acute phosphate nephropathy following oral sodium phosphate solution to cleanse the bowel for colonoscopy. A renal biopsy showed diffuse tubular calcium deposition.

10.Colchicine-derived compound CT20126 promotes skin allograft survival by regulating the balance of Th1 and Th2 cytokine production.

Colchicine has been shown to regulate the expression of inflammatory gene, but this compound possesses much weaker anti-inflammatory activity. In this study, we synthesized a new colchicine derivative CT20126 and examined its immunomodulatory property. CT20126 was found to have immunosuppressive effects by inhibiting lymphocyte proliferation without cytotoxicity and effectively inhibit the transcriptional expression of the inflammatory genes, iNOS, TNF-alpha, and IL-1beta, in macrophages stimulated by LPS. This effect was nearly comparable to that of cyclosporine A. This compound also significantly suppressed the production of nitric oxide and Th1-related pro-inflammatory cytokines, IL-1beta, TNF-alpha, and IL-2, with minimal suppression of Th2-related anti-inflammatory cytokines IL-4 and IL-10 in the sponge matrix allograft model. Moreover, administration of CT20126 prolonged the survival of allograft skins from BALB/c mice (H-2d) to the dorsum of C57BL/6 (H-2b) mice. The in vivo immune suppressive effects of CT20126 were similar to that of cyclosporine A. These results indicate that this compound may have potential therapeutic value for transplantation rejection and other inflammatory diseases.
