2.Intravenous Infusion of Clonidine Potentiates Postoperative Analgesia Induced by Fentanyl.

BACKGROUND: Clonidine, an alpha 2adrenergic agonist, has nonopiate antinociceptive properties which might be an alternative for postoperative analgesia free of undesirable effects from opioid. The aim of this study was to evaluate the postoperative analgesic effects of intravenous (IV) infusion of clonidine. METHODS: Seventy two healthy patients who undergoing cesarean section or gynecological surgery under general anesthesia were randomly divided into three groups as follows; the patients of group l received fentanyl (100 g bolus + 0.5ug/kg/hr) alone, the patients of group II received clonidine (100ug bolus + 10ug/kg/hr) and same dose of fentanyl, and the patients of group III received clonidine (200 g bolus + 20ug/kg/hr) and same dose of fentanyl. Pain score using a visual analogue scale (VAS), sedation score and any side effects were evaluated at 1, 6, 24 and 48 hours after starting the infusion. RESULT: Patients in group II and group III were more pain relieved during 48 hours and more sedated compared to group I at 1 hr, but there were no significant difference of side effects. CONCLUSION: Continuous IV infusion of clonidine potentiates postoperative analgesia induced by fentanyl.

3.A Study on the Recognition and Performance of Nursing Students' Hand Washing.

BACKGROUND: The purpose of this study is to inquire into the recognition and performance of nursing students' hand washing and propose basic data for upright recognition and performance as reserve nurses. METHODS: A survey was carried out from September 1 to September 14, 2008 by first distributing questionnaires to 550 nursing collage students attending three-year-course or four-year-course colleges in Daegu. Of all the questionnaires later collected, only the copies received from the students, who were in their last year of college(416 copies), were used for analysis. RESULTS: The recognition and performance of their hand washing averaged 3.49 and 4.08 points, respectively, showing that the level of recognition was higher than that of performance. In addition, the four-year-course nursing college students showed higher recognition level compared to the three-year-course nursing college students. However, the result was vice versa for the performance level. CONCLUSION: It is necessary for nursing students, as reserve nurses, to receive an education to improve the recognition and performing level of hand washing.

4.A Study on the Cognitive Coping Strategies by Job Stress Level of Call Center Workers.

PURPOSE: This study aimed to investigate the job stress level of call center workers and figure out its relation with cognitive emotion regulation strategies as a cognitive coping strategy. METHODS: Data collection was conducted on 985 call center workers, working for K companies in Korea. ANOVA, t-test, Scheffe's test and multiple regression have been conducted for the data analysis using SAS 9.0 software. RESULTS: The total job stress score of subjects ranges Q25~49 (the lower 50%) whereas the score of job demand and job control ranges Q50~74 (the upper 50%), and its sectional job stress level is high. Since job stress is the major factor affecting cognitive emotion regulation strategies, it is shown that when the job stress score gets higher, the score of positive cognitive emotion regulation strategies gets decreased but the score of negative cognitive emotion regulation strategies, increased. CONCLUSION: Based on the results of this study, it is necessary to develop and apply an effective stress easing method for call center workers which reduces negative cognitive emotion regulation strategies and increases positive ones.

6.The Effect of Rotating Shift Work(2 days intervals) on the changes of 17-OHCS and Free Cortisol.

In this study, 20 students in C university were selected by control group and 14 shift work nurses working in C university hospital by test group It took part in case of day shift(8AM-4PM), evening shift(4PM-12MN), and night shift(12MN-8AM) to know a urinary 17-OHCS and free cortisol changes which works in a rapidly rotating shift work system. The working team were an rest period of a day and an oberservation of an week. It obtained an urine specimen before and after work shift in 2nd day. Test group and control group of shift work compared to change of levels of urinary 17-OHCS and free cortisol of urine collected from nurses of day shift, evening shift, and night shift. The data was analyzed by t-test, paired t-test. The results are as follows. 1. Compare 8AM with 4PM in day shift. Control group increased 4.1mg at 8AM, 4.2mg at 4PM in a case of 17-OHCS(p=.84) and also test group increased a little 3.5mg at start time of work, 3.6mg at stop time(p=.97). In a case of free cortisol control group decreased 3.8microgram at 8AM, 2.4microgram at 4PM(p=.12) and test group decreased 8.3microgramat start time of work, 3.2microgram at stop time(p=.22). 2. Compare 4PM with 12MN in evening shift. Control group decreased 4.2mg at 4PM, 2.9mg at 12MN in a case 17-OHCS(p=.54), but test group increased 1.7mgat start time of work, 3.4mg at stop time(p=.07). In a case of free cortisol control group decreased 2.4microgram at 4PM, 1.9microgram AT 12MN(p=.23) and also test group decreased a little 2.6microgram at start time of work, 1microgram at stop time(p=.43). 3. Compared 12MN with 8AM in night shift. Control group increased 3.9mg at 12MN, 4.1mg at 8AM in a case of 17-OHCS(p=.79) and also test group increased 6.3mg at start time of work, 8.4mg at stop time(p=.16). In a case of free cortisol control group increased 1.9microgram at 12MN, 3.8microgram at 8AM(p=.08) and test group increased 4.4microgram at start time of work, 11.6microgram at stop time(p=.04). As a result of this study reveals that health level of nurses is decreased in a rotating shift work(2 days internal). As mentioned above, it was confirmed that the rotating shift work had influence on the health of the nurse. For it, I present the objective base data to measure the level healthy in order to extend the understanding of the physical aspects of the nurses. Moreover, it is considered that one can make use of it as an objective base data with a view to the rational management for the nursing administration.

8.A Case of Spontaneous Perforation of the Right Intrahepatic Duct - An ERCP Diagnosis.

Spontaneous perforation of bile duct in adults is very rare, with less than 30 cases described in the literature to date. We repoit a case of a 65-year-old man who presented with severe colicky abdominal pain and fever, just like symptoms of peritonitis. ERCP provided a preoperative noninvasive confirmation of the diagnosis of the rupture of right intrahepatic duct. The patient was performed T-tube choledochotomy and drainage of retroperitoneal bile collection. The etiolgy, diagnosis, and treatment of spontaneous perforation of bile duct is discussed.

9.Hyperinfection Syndrome with Strongyloides Stercoralis: Report of a case.

Strongyloides stercoralis is a nematodes and is prevalent in the tropical regions. In Korea, 6 cases have been reported in which the parasites were identified in feces. We report a case of hyperinfection syndrome with Strongyloides stercoralis confirmed in the tissue sections. A 52-year-old woman was admitted to the hospital because of generalized edema and weakness. She received steroid therapy for 30 years because of rheumatoid arthritis. One year ago, generalized edema and dyspnea were developed and she was diagnosed as minimal change nephrotic syndrome for which she received cyclosporin. On admission, she complained of generalized weakness and edema, multiple arthralgia and abdominal discomfort with nausea. The white-cell count was 14,600 without eosinophilia. A stool specimen was negative for occult blood, ova or parasites. Right lower quadrant abdominal pain and tenderness were developed, and the exploratory laparotomy was done under the impression of the acute appendicitis. An appendectomy with a biopsy of mesenteric lymph nodes was performed. From the 5th day after operation, the patient began to complain of the abdominal pain and constipation. Under the impression of adhesion bad formation, the laparotomy was done and disclosed that the proximal ileum was markedly dilated. On jejunotomy, a great amount of formed stool and barium was noticed in the proximal portion of small bowel. The jejunal tissue was biopsied. Grossly, the appendix was unremarkable. Microscopically, the inflammatory reaction was nearly absent, but in serosa, minute granulomas were found which contain a part of the parasite. Same feature was noted in the mesenteric lymph node. The biopsied jejunum contains numerous adult female, filariform and rhabdidiform larvae and eggs in the mucosa, submucosa and muscle wall. Their morphology was compatible with Strongyloides stercoralis. The patient died 5 days later in the state of multiple organ failure.

10.Improved Reporting Rate of Notifiable Communicable Diseases through a Disease Code-Based Surveillance and In-House Electronic Reporting System.

Background: Reporting rate of notifiable communicable diseases has been assumed to be low in Korea. To encourage timely reporting of notifiable communicable diseases and to improve the reporting process by the Infection Control Unit, a disease code-based electronic surveillance and in-house electronic reporting system was developed and implemented in February 2003. We evaluated the reporting rates of notifiable communicable diseases before and after the implementation of the electronic reporting- system. METHODS: The electronic reporting system, which was integrated to the computerized physician order entry system, was designed to monitor KCD-4 codes at the time of every order entry. Whenever the system recognized the predetermined codes of notifiable communicable diseases, it generated an alert box to recommend reporting: and then a physician could fill up an electronic report form, which was collected and faxed to the local public health authority by the Infection Control Unit. Reporting rate of notifiable communicable diseases in 2002, when the conventional paper-based reporting system was used, was assumed comparing the number of cases reported actually and that of cases identified to be reported by reviewing the medical records and electronic database. RESULTS: From March to July 2003, 181 cases with tuberculosis and 14 with other communicable diseases were reported whereas 106 and 9 cases were reported respectively for the same 5-month period in 2002. Reporting rate using the conventional reporting system in 2002 was assumed to be 61% and 39% respectively. Conclusions: Using a disease code-based electronic reporting system, reporting of notifiable communicable diseases could be improved compared with conventional paper-based reporting system.
