1.Serial femoral arteriography in Buerger's disease

Femoral arteriography has assumed in recent years a greater clinical and surgical significance, especiallysince the advent of arterial grafting for occlusive arterial disease. Evaluation of the site and extent ofocclusion, the state of distal arterial tree and degree of collateral circulation can best be obtained byserialographic studies. Authors analyzed 28 cases of clinically and radiologically diagnosed Buerger's disease inGURO hospital radiology, College of medicine, Korea university, during last 6 months from March to August, 1984.The results are as follows; 1. The age distribution were between 20 and 50 years old, and most commonly involvedage group was 20-29. 2. The most frequent finding was the obstruction of peripheral artery with or withougcollateral vessel and almost all patient had occlusion more than 1 segmental branch. 3. The most frequentlyinvolved arteries in Buerger's disease were trifurcation area below the knee joint, anterior and posterior tibialartery and peroneal artery. Peroneal artery was less commonly involved than anterior or posterior tibial artery.

4.Analysis of Heavy Metals in 201TlTlCl Injection Using Polarography.

PURPOSE: Thallous-201 chloride produced at Korea Cancer Center Hospital(KCCH) is used in detecting cardiovascular disease and cancer. Thallium impurity can cause emesis, catharsis and nausea, so the presence of thallium and other metal impurities should be determined. According to USP and KP, their amounts must be less than 2 ppm in thallium and 5 ppm in total. In this study, the detection method of trace amounts of metal impurities in [201Tl]TlCl injection with polarography was optimized without environmental contamination. MATERALS AND METHODS: For the detection of metal impurities, Osteryoung Square Wave Stripping Voltammetry method was used in Bio-Analytical System (BAS) 50W polarograph. The voltammetry was composed of Dropping Mercury Electrode (DME) as a working electrode, Ag/AgCl as a reference electrode and Pt wire as a counter electrode. Square wave stripping method, which makes use of formation and deformation of amalgam, was adopted to determine the metal impurities, and pH 7 phosphate buffer was used as supporting electrolyte. RESULTS: T1, Cu and Pb in thallous-201 chloride solution were detected by scanning from 300 mV to -800 mV. Calibration curves were made by using TlNO3, CuSO4 and Pb(NO3)2 as standard solutions. Tl was confirmed at -450 mV peak potential and Cu at -50 mV. Less than 2 ppm of Tl and Cu was detected and Pb was not detected in KCCH-produced thallous-201 chloride injection. CONCLUSION: Detection limit of thallium and copper is approximately 50 ppb with this method. As a result of this experiment, thallium and other metal impurities in thallous-201 chloride injection, produced at Korea Cancer Center Hospital, are in the regulation of USP and KP. Polarograph could be applied for the determination of metal impurities in the quality control of radiopharmaceuticals conveniently without environmental contamination.

5.Prevention of Autologous Vein Graft Atherosclerosis Thru Cell Cycle Inhibition.

PURPOSE: Autologous vein graft is the most commonly used conduit for surgical revascularization of the small and medium sized vessels of the coronary and lower extremity circulations. But, 30~50% of these grafts succumb eventually to occlusive failure, often with recurrent morbidity that necessitates reoperation. The cause of failure is the vein graft atherosclerosis from the neointimal hyperplasia, which is the arterial remodeling process through adaptive thickening of the vein wall in response to the increased wall stress. We try to use cis-platin, DNA binding cell cycle specific inhibitory agent, against neointimal hyperplasia in experimental model. METHOD: 5 korean dogs had a autologous vein graft interposition at femoral or carotid arteries using saphenous or jugular veins bilaterally. One side of pairs is treated with cis-platin for one hour (study), while the other is not (conrol). After 4 weeks, we explanted 6 pairs of vein graft (3 jugular veins and 3 saphenous veins), and measured intimal thickness, intimal area and luminal area using videomorphometry, and analysed intimal ratio and luminal ratio. RESULT: Treatment with cis-platin reduced intimal thickness by 44% (P=0.0118) and intimal area by 50% (P=0.0124). Intimal ratio was decreased in the study group when compared to the control group by 0.18 versus 0.66 (P=0.001). The luminal area of the treated grafts was significantly larger than controls, having increased by 823% (P=0.0307), and luminal ratio having increased by 0.82 versus 0.34 (P=0.001). CONCLUSION: These results suggest that cis-platin reduced the development of neointimal hyperplasia in experimental vein grafts. In order to prove the obtained results to a higher degree, an accumulation of further cases is necessary.

8.Osseointegration with Ceramic Coated Implant.

PURPOSE: This study was designed to clarify the osseointegration of the titanium screw coated with CMP, in regard to the time schedule, through the characteristic of early osseointegration. MATERIALS AND METHODS: Mechanical, radiological and histomophometric measurements were performed in 28 rabbit tibial proximal metaphyseal cortical bone screws 6, 12, 26 and 52 weeks after surgery for the in vivo comparison of the osseointegration of titanium screws (3.75 mm diameter, 5 mm length) with different surface treatments: CMP coating group, with the sol-gel method (experimental group) and uncoated group (control group). RESULTS: 1. Radiology: There were no differences between the two groups without a radiolucent line or in regard to the time schedule. 2. Histology: There were no differences between the two groups without a fibrous tissue intervening surface or in regard to the time schedule. 3. Torque test: The test results for the CMP coated group were 1.5 times higher than those for the uncoated group, which was statistically meaningful, but there was no difference in regard to the time schedule. CONCLUSION: CMP coating is an option to increase the osseointegration of the titanium screw.

9.Anesthetic experiences with two cases of congenital esophageal fistula atresia with tracheo-esophageal fistula .

The authors have experienced two cases of general anesthesia for patients of congenital esophageal atresia with tracheo-esophageal fistula. Of great importance in anesthetic management is preventing pulmonary complications before or during operation as well as post-operation. Survival rates mostly depend upon the early diagnosis and abscence of pulmonary complications which are the most common cause of high death rates. Meticulous care of the patients to keep on airway, clearing of bronchial secreations, maintaining normal body temperature and humidity are very important in management of anesthesia. To avoid gastric distension, careful inflation of the lungs during opened chest has been recommended. Also intermittent positive pressure ventilation during the repair may be required.

10.Clinical Analysis of Puffer Fish Poisoning.

BACKGROUND: Puffer fish can be the source of lethal flood poisoning in humans. Tetrodotoxin(TTX) poisonings are not infrequency seen in Korea, but there are few clinical reports. So we reviewed the patients of TTX poisoning and analyzed the clinical characteristics of patients. METHODS: A retrospective study was performed of 40 patients who visited Chung Ang Gil Hospital from Jan. 1, 1995 to May. 31, 1998 with a diagnosis of TTX poisoning by a review of patients' medical records and telephone inquiries, The diagnosis of TTX poisoning was made by causal links between consumption of puffer fish and the development of typical symptoms of tetrodotoxication. The clinical severity of the patients in this study was classified according to the classification of Fukuda. RESULTS: Mean age of the patients was 40 years. The highest incidence was in the 4th decade in 21 patients(52.5%). The ratio of male to female was 3.44:1, Seasonal distribution excluding cases in 1998 was 12 patients in spring, 6 in summer, 5 in autumn and 12 in winter. The mean interval between consumption and symptom onset was 137 minutes. Common initial symptoms were circumoral numbness(32 patients) and paresthesia of extremities(24 patients). Various symptoms were developed after ingestion of puffer flesh such as neuromuscular(39 patients), cardiovascular/pulmonary (23 patients) and gastrointestinal (16 patients) system. Mean recovery time from the onset of symptom was 22.00 hours. All were treated with symptomatic and supportive measures and recovered completely without sequelae. In 2 cases ventilatory supports were applied for 18.5 hours and 31.5 hours respectively.
