1.Misread Partial Airway Obstruction as a Muscle Paralysis of Parathion Poisoning: A Case Report.

A case of parathion poisoning has been reported; in which the symptoms of hypoxia resulted from partial airway obstruction, however this was mistaken for parathion poisoning, and the correct diagnosis was proved by removal of hard blood clot in the bronchial bifurcation. Conclusions were as follows: Adquate administration of PAM and atropine presented impressive response of symptoms. 2. Careful and frequent removal of secretion might be the most important problem especially in this poisoning. 3. Massive administration of antibiotics could prevent pulmonary complications. 4. Pulmonary edema, the most common complication, was not present.

2.Air in Vagina: Significance in the Staging of Uterine Cervical Carcinoma.

PURPOSE: To evaluate tlne significance of vaginal air seen on CT scan in preoperative staging of uterine cervical carcinoma. METHODS AND MATERIALS: A comparison was made between CT findings of vaginal air and true vaginal involvement status in 85 patients with uterine cervical carcinoma. CT findings were analyzed in terms of the presence or absence of vaginal air, number of CT slices in which vaginal air was seen, shape of vaginal air, and relation of vaginal airto cervical mass. RESULTS: Vaginal air was present in 35 patients and was absent in 50. Although the mere presence of vaginal air or multiplicity of CT slices showing vaginal air did not signify the presence of vaginal involvement, vaginal air with irregular margin or vaginal air adjacent to uterine cervical mass was suggestive of vaginal involvement. CONCELUSION: These observation of vaginal air in interpreting CT may be helpful in the preoperative staging of uterine cervical carcinoma.

3.Changes in PaO2 and PaCO2 with Modified Flagg Can System with Ether .

PaO2 and PaCO2 values were studied in 8 surgical patients with a modified Flagg can system anesthesia. Arterial blood gases were analyzed with a Radiometer before every induction, after the first 30 minutes of anesthesia and in the immediate post-anesthetic period. The following results were obtained: 1) The values of PaO2 and PaCO2 in all cases during anesthesia did not show any remarkable change. 2) In cases, with no necessity of muscle relaxation or assisted and controled respiration, especially in children, the modified Flagg can system anesthesia seemed to be safe, simple and economical.

4.The Effects of Sexual Rehabilitation Education on the Knowledge of Sexual Activities and Satisfaction of Sexual Activities of Spinal Cord Injury Clients.

The purpose of this study is to identify the effects of sexual rehabilitation education on the knowledge of sexual activities and satisfaction of sexual activities of spinal cord injury clients by the method of Nonequivalent control group pretest-posttest group. The study period was between Jan. 6th and Apr. 11th, 1999. The experimental group was 21 spinal cord injury clients from one general hospital, and the control group was 21 spinal cord injury clients from three University hospital. Sexual rehabilitation education was done with manuals and video-tapes. Education time was 90 minutes a week for 4 weeks. The study tool was 20 questionnaires about sexual knowledge, designed Song, Chung Sook(1990) and 10 questionnaires from Derogatis's Derogatis Sexual Function Inventory for satisfaction of sexual activities(1979) translated by Jang, Soon Boke(1989). Analysis of data was done by x2 -test, t-test, repeated measures ANOVA and simple main effect with SAS/PC. The results are as follows. The 1st. hypothesis, "The group educated about the sexual rehabilitation has more sexual activities knowledge than those of the uneducated group." is significant statistically(F=12.06, p=0.001). The 2nd. hypotheses, "The group educated about the sexual rehabilitation has more satisfaction of sexual activities than those of the uneducated group." is significant statistically(F=5.04, p=0.030). According to the results, sexual rehabilitation education helps spinal cord injury clients to gain knowledge of sexual activities and satisfaction of sexual activities.

5.Peripheral Nerve Paralysis following General Anesthesia - 3 cases.

3 cases of ulnar nerve paralysis following general anesthesia are reported. In all of 3 cases, there were no anatomical abnormalities and any neurologic disease, and no traumatic history before operation. And there were no specific findings in laboratory and radiologic examination. It is suggested in the absence of any causative factors other than malposition. In two cases, the paralysis disappeared without specific therapy in 4 to 5 weeks, but in one case, the paralysis was more severe and the improvement was not remarkable with physiotherapy for 3 months.

6.Identification of Viral Pathogens for Lower Respiratory Tract Infection in Children at Seoul During Autumn and Winter Seasons of the Year of 2008-2009.

PURPOSE: The Purposes of this study are to identify the circulating etiologic viruses of acute lower respiratory tract infection in children and to understand the relation with clinical diagnosis. METHODS: We obtained a total of 418 nasopharyngeal aspirates from children admitted for their acute lower respiratory tract infections at three tertiary hospitals in Seoul from September 2008 to March 2009. We performed multiplex RT-PCR to identify 14 etiologic viruses and analyzed their emerging patterns and clinical features. RESULTS: Average age of patients was 16.4 months old and the ratio of male to female was 1.36. Viruses were detected in 56.2% of a total of 418 samples. Respiratory syncytial virus (35%) was the most frequently detected and followed by human rhinovirus (22%), human bocavirus, adenovirus, human metapneumovirus, parainfluenza virus, influenza virus and human coronavirus. Co-infection reached 21.9% of positive patients. CONCLUSION: When we manage the patients with acute lower respiratory infectious diseases, we should remind the role of various viral pathogens, which might be circulating by seasons and by local areas.

8.Coagulation Status Changes during Progressive Hemodilution.

BACKGROUND: Hemodilution reduces the concentration of clotting factors in blood and this may induce some degree of impairment of coagulation. But there are some evidence that hemodilution may induce a hypercoagulable state. The goal of this study is to investigate the changes of coagulation status during progressive hemodilution. METHODS: Whole Blood was diluted by 10% increment to 80% with 0.9% normal saline. At each 10% dilution blood coagulation status was analysed using thrombelastography (TEG) and was compared with that of an undiluted control specimen obtained concurrently from the same patients. RESULTS: Hemodilutions up to 40% decrease r and K times and increase values of MA and angle. Hemodilutions more than 70% increase r and K times and decrease values of MA and angle. CONCLUSIONS: Hemodilutions up to 40% increase coagulability and hemodilutions more than 70% decrease coagulability of whole blood in vitro.

9.Percutaneous Transluminal angioplasty of membranous obstruction of IVC: a case report

Percutaneous tansluminal angioplasty(PTA) was done in 38-year-old female patient with membranous obstructionof IVC in the department of Radiology, Seoul National University Hospital. After PTA, pressure gradient between right atrium and IVC was relieved from 16mmHg to 6mmHg. The collateral circulation through hemiazygos vein andreflux into hepatic and right renal vein were disappeared. PTA is suggested to be an effective method in thetreatment of membranous obstruction of IVC.

10.Clinical Observations on Effortil and Other Pressor Drugs .

To 100 patients who underwent various operations under general anesthesia, Effortil and methoxamine were given in order to assess and to compare their effects on blood pressure and pulse rate. The drugs were administered intravenously in 6 to 10 mg doses when the systolic pressure fell to 80 mmHg. or lower during the operation. The results obtained are as follows: 1. Excellent pressor effect was obtained with Effortil in 84% of 50 patients while in 72% with methoxamine. 2. Both drugs were equally effectve in elevating both systolic and diastolic pressures. 3. The effect of Effortil on pulse rate was much less than that of methoxamine which moderately reduced the pulse rate, especially in the early phase. 4. No untoward effects of either Effortil or methoxamine were observed in this study.
