A clinical evaluation was made with particular attention to surgical management on 156 patients with renal calculi who had been admitted to the Department of Urology, Kyungpook National University Hospital during 1O years periods from January, 1976 to December, l985. The results were as follows: 1. The calculus was unilateral in 144 patients (right in 73 and left in 71) and bilateral in l2.57 patients had staghorn calculi, 62 either single or multiple pelvic calculi, 20 either single or multiple calyceal calculi, and 17 multiple pelvic and calyceal calculi. 2. The treatment modality was surgical intervention in l39 patients(143 operations) and conservative management in 17.143 surgical interventions were nephrectomy in 17 and conservative surgery in 126. Conservative surgery was simple pyelolithotomy in 14, extended pyelolithotomy in 59, extended pyelolithotomy with parenchymal extension in 21, radial nephrolithotomy in 4, anatrophic nephrolithotomy in l4, partial nephrectomy in l3, and extended pyelolithotomy with ureterocalycostomy in 1. 41 operations with nephrotomy were done using in situ hypothermia. 3. 60 surgical interventions for pelvic calculi were simple pyelolithotomy in 14, extended pyelolithotomy in 40, extended pyelolithotomy with parenchymal extension in 2, and nephrectomy in 4.l9 surgical interventions for calyceal calculi were extended pyelolithotomy in 4, extended pyelolithotomy with parenchymal extension in 4, radial nephrolithotomy in 3, partial nephrectomy in 6, and nephrectomy in 2. 14 surgical interventions for pelvic and calyceal calculi were extended pyelolithotomy in 1, extended pyelolithotomy with parenchymal extension in 3, radial nephrolithotomy in l, anatrophic nephrolithotomy in 2, and partial nephrectomy in 5. 50 surgical interventions for stag horn calculi were extended pyelolithotomy in 14, extended pyelolithotomy with parenchymal extension in 2, anatrophic nephrolithotomy in 14, partial nephrectomy in 5, and nephrectomy in 6. 4. Except for 15 cases with remnant calculi and 1 with nephrectomy, no mortality nor significant morbidity was found 126 cases with renal calculi treated by conservative surgery. Cases found to have remnant calculi after surgical procedures were 1 out of 14 pyelolithotomies, 6 out of 59 extended pyelolithotomies, 4 out of 21 extended pyelolithotomy with parenchymal 2 extension, out of 14 anatrophic nephrolithotomies, and 1 out of 4 radial nephrolithotomies.

As the development of the percutaneous renal surgery, Ureteropelvic junction obstruction can be treated with endourologic technique instead of open surgery. Precutaneous endopyelotomy has been used mainly for the treatment of secondary ureteropelvic junction obstruction. But it was also successful for treatment of primary ureteropelvic junction obstruction. Recently, we performed endopyelotomy for 3 cases of ureteropelvic junction obstruction a 39 years-old male who had renal pelvic stones combined with ureteropelvic junction, a 32 years-old male with ureteropelvic junction obstruction and a 23 years-old male who had horseshoe kidney combined with ureteropelvic junction obstruction and good results were obtained.

The incidence of duodenal diverticulum found incidentally during upper gastrointestinal roentgenographicexamination varies between 2% and 5%. The majority of cases occur along the medial aspect of the second portion of the duodenum, within 2.5cm of the ampulla of Vater. The majority of duodenal diverticula are asymptomatic, but insome cases, complications such as diverticulitis, hemorrhage, perforation, and fistula formation occur in thethird and fourth portions of the duodenum. We describe a case of giant diverticulum of the transverse duodenum,revealed by UGI and angiography, massive gastrointestinal bleeding in a 80-year-old patient.

A clinical survey has been undertaken of 50 patients with urethral stricture who had been treated with intermittent dilatation in the Department of Urology, Kyungpook National University Hospital during 8 years periods from January, 1977 to December l984. These patients had history of urethral stricture for 1-15 years periods prior to attending this hospital and had been attended with intermittent dilatation for l-6 years. The interval between dilatation was a range from 2 days to 2 years. Sixty-two percent had infected urine but the apparent decreasing of the incidence of infected complications following dilatation may be attributed to the use of prophylactic chemotherapeutic agents. Associated diseases and complications related to instrumentation were noticed in 20 cases of these patients but half of this was healed during intermittent dilatation and use of chemotherapeutic agents. In the follow up of 50 patients with intermittent dilatation, 37 cases remained trouble free whereas 13 cases had repeat complications or required another management.

Replacement lipomatosis of the kidney is a rare disease. It is characterized by fatty replacement of destroyed or atrophic renal parenchyma which associated with inflammatory or noninflammatory renal disease. Herein we reported a case of replacement lipomatosis of the kidney with renal tuberculosis and calculus.

PURPOSE: To assess the suitability of multidetector row CT angiogram (MDCTA) as the sole preoperative imaging for infrainguinal arterial surgery. METHOD: From March 2002 to September 2003, 75 patients (24 claudicants, 41 limb-threatening ischemia) were studied with MDCTA preoperatively. We compared the surgical inflow and outflow site changes between preoperative planning based on MDCTA and the results of final operation. MDCTA was interpreted by the same vascular surgeon, and arterial segments from the renal artery to foot were reviewed. Surgery plans were formulated based on arterial anatomic and hemodynamic characteristics. Additional diagnostic value and test related complications were also studied. RESULT: Twenty- one patients had conventional angiogram after MDCTA scan - 9 for interventional treatment at inflow site and 12 patients for complement of MDCTA, although the operation plan was not changed. The agreement between preoperative plan based on MDCTA and final operation was 100% even in critical limb ischemia. In 11 patients tortuous calcified iliac artery was ambiguous in routine image but it could be solved using the other specific functional option of MDCTA. There were no serious complications related to the test. CONCLUSION: These findings suggest that MDCTA is an adequate preoperative imaging study of infrainguinal arterial surgery and that it may be substituted for conventional angiogram without any serious complication. The particular functional options of MDCTA help resolve its defect.
