2.A Clinical Study of Vascularized Osteocutaneous Fibular Transfer

Recently, there were many cases which was associated with bone defect in the limb and skin defect. We had performed vascularized osteocutaneous fibular transfer with microsurgical technique and experienced early bone union and simutsneous external wound healing. So we, authers, reviewed 12 cases of vascularized osteocutaneous fibular transfer which were performed to the extensive bone defect of the tibia and associated soft tissue injury. And the following remarkable results were obtained. 1. With the uncontrolled bone infection, the vascularized osteocutaneous fibular transfer was able to be performed and there was no non-union. 2. During the follow up period, the grafted fibula had been hypertrophied. 3. In the cases which the fibula were grafted to the defect after the segmental and partial excision of the tibia, in general, the partial weight bearing was able to be started from 3 months to 5 months after the operation, In the cases with segmental and complete excision of the tibia, the partial weight bearing was able to be started from 5 months to 7 month after the operation. 4. In adult, free fibula can be obtained and grafted as 20cm as long and the fibula was a good donor for extensive long bone defect as a compact bone; the skin flap with fibula can be obtained and grafted as 17 × 9cm as large in our experience. 5. In the vascularized osteocutaneous fibula transfer, the skin flap could be confirmed wheather the vascular anastomosis is patent or not as a monitor.

4.Treatment of Hyperpigmented Disease with 4-Isopropylcatechol.

Disfiguring hyperpigmentation of the exposed areas may have impartant and lasting effects on the emotional well-being of the patient. And many attempts to treat areas of hyperpigmentation have been earried out, but all of them are not very successful. Recently 4-isopropylcatechol has been known to inhibit melanin pigmentation with a selective action on melanocytes either destroying or inactivating the melanocytes experimentally. There is no report on its clinical trial till now. In this study, 1% 4-isopropylcatechol cream was topieally applied to the hyperpigmented lesions of 29 patieots of melasma, freckle, and cafe-au-lait spot. The bleaching effect on meiasmas was very satisfactory in all patients but coantact dermatltis in 3 patients and reticular hyperpigmentation with areas of depigmented spots in one patient were developed as the side effects of the drug. Freckle and cafe-au-Iait spots were not bleached.

5.A Study about Changes in the Lumbosacral Region and Wrist of the Adolescent Weightilifters

The weightlifting is a sort of the sport, which has objects to promote the physical condition and to develop the fundamental strength in the adolescent period, but thete been no interests from the view points of sports injuries for weightlifting. So the authors have studied the changes in the lumbosacral region and wrist of the adolescent weightlifters and the results obtained are as follows; 1. The increase in the lumbosacral angle has a statistically significant relationship with the low back pain in the adolescent weightlifters. 2. The ulnar variance shows statistically significant increase than the control group and its increase seems to be a causative factor in developing wrist pain among the adolescent weightlifters.

7.Laboratory Information System using One Combined Request Slip: Specimen Requisition and Order Input System.

Usually LIS(Laboratory Information System) depend on OCS(Order Communication System) of HIS(Hospital Information System). So, both systems are constructed simultaneously. But it needs high cost and has potential of failure. So, we designed one combined laboratory request slip and isolated LIS(sever: Power Mac 7200, eight clients: LC 475, connected by Ethernet) operating with own order input system before HIS construction. we analyzed the efficiency during early 9 weeks (from Aug. 17 to Oct. 17 in 1995) and later 4 weeks (March in 1996). Daily input amount of request changed stably in the later period(maximum: 552. mean: 410.0) than the early period(maximum: 604. mean: 341.6). Peak time was between 9 a.m. and 10 a.m.(maximum: 127. slip/hour). In the later period, 64.2% the requests for in-patients were completed until 10 a.m. and the request for out-patients distributed more evenly. Majority of problems occurred, were delayed(25.5% in the early, 61.1% in the later period) or missed specimen(31.9% in the early, 22.9% in the later). Total number (26,309) of used requests was less than half of the calculated number(57,621) of previous laboratory slips.

9.A Case of Subsequent Papillary Carcinoma of the Thyroid gland and Hashimoto's Thyroiditis

The association of thyroid carcinoma and Hashimotos thyroiditis in same thyroid gland is controversial. Incidence of carcinoma who has Hashimotos thyroiditis has been reported from 0.5 to 22.5 per cent by Crile and by Hirabayashi et al. The reason that there are such great diffarences in the reported incidences of carcinoma in Hashimotos disease is the result of the way the material is reported. The carcinomas of the thyroid which occur in association with Hashirnotos thyroiditis are predominently papillary tumors of lower grade malignancy. Thyroid carcinoma need not be feared in patimts with Hashimotos thymiditis, if one examines the ghmd catefully. When patients with Hashimotos disease are treated with thyroxine, there is little or no tendency for Hashimotos disease propess to clinieally detectable carcinoma of the thymid, and the microcarcinoma does not appear. In this case, single thyroid nodule was detected in Hashiimotos disease patient who was treated with thyroxine. There was no significant volume change of thyroid nodule despite of TSH suppression therapy during six months. Therefore we perforrned FNABC twice, the results were highly suspicious thyroid malignancy and subtotoal thyroidectomy was performed. The final pathologic result was microscopic papillary carcinoma with background Hashlmotos thyroiditis. In conclusion, we experienced a case of subsequent microscopic papillary carcinoma of the thyroid in patient with Hashimotos thyroiditis who was TSH suppression therapy with thyroxine.

10.The effect of denture cleansers on the bond strength and the surface hardness of reline resin to denture base resin.

STATEMENT OF PROBLEM: Removable partial denture and complete denture often require denture base relines to improve the fittness against tissue-bearing mucosa because of the gradual change in edentulous ridge contour and resorption of underlyng bony structure. Self-curing hard reline resins offers the immediate and relatively inexpensive means to be recondition the surface of denture base directly. However weak bond between denture base resin and reline material can harbor bacteria, promote staining, or result in complete separation of the two materials. PURPOSE: The purpose of this study was to evaluate the effect of denture cleansers on bond strength and surface hardness of reline resin to denture base resin Denture base resin beams(60.0x15.0x3.0 mm) were made with Lucitone 199. MATERIAL AND METHODS: 10mm section was removed from the center of each specimen. The samples were replaced in the molds and the space of 10mm sections were packed with Tokuso Rebase reline material. The specimens were immersed in denture cleansers(Polident, Cleadent) and were evaluated after 1week, 2weeks, and 4weeks. The bond strength and surface hardness of self-curing hard reline materials to heat-curing denture base resin were measured using an UTM(universal testing machine). RESULTS AND CONCLUSION: 1) There was no significant difference of usage, kind, and denture cleaner by application time on the bonding strength of self-curing hard reline resin to denture base resin. 2) There was no significant difference of usage, kind, and denture cleaner by application time on the surface hardness, but the surface hardness showed decreasing tendency, as the time of immersion was extended 3) The failure modes of the specimens was initially adhesive failure and finally cohesive failure of self-curing hard reline resin.
