2.Survey for Hygiene Behavior on Healthcare Personnel by Hygiene Inventory 23.

BACKGROUND: The purpose of this study was to survey hygiene behavior of healthcare personnel according to the Hygiene Inventory 23 (HI23) and to use the results in education and research for promoting hygiene behaviors. METHODS: We sampled a total of 400 people with 50 from each job category. The sample was obtained through convenient sampling among 8,200 employees working at a tertiary-care hospital with 2,600 beds in Seoul. The HI23 consisted of 5 subscales and 8 questions on general hygiene, 3 on household hygiene, 3 on food-related hygiene, 5 on hand hygiene methods, and 4 on personal hygiene, resulting in a total of 23 questions. The researcher also added 14 questions on the general characteristics of the subjects. The self-administered questionnaires were distributed on October 2009 and analyzed blindly. RESULTS: The average total score for hygiene behavior was 3.04+/-0.44 (maximum of 4). In the subcategory of hygiene behavior, the mean score was 3.20+/-4.24 for general hygiene, 3.07+/-0.68 for household hygiene, 3.07+/-0.68 for food-related hygiene, 2.91+/-0.69 for hand hygiene methods, and 2.97+/-0.48 for personal hygiene. When factors affecting the level of hygiene behavior were identified through multivariate regression analysis, independent factors were found to be gender (female), job category (food-related job), experience in hand hygiene education, and experience in teaching hand hygiene or general hygiene. CONCLUSION: Programs and education adapted to the characteristics of each job category in hospitals are needed to promote hygiene behaviors such as general hygiene, hand hygiene methods, household hygiene, food-related hygiene, and personal hygiene.

6.Placental Site Trophoblastic Tumor(PSTT) with Vaginal Metastasis: A case report.

PSTT has been established as a separate entity distinct from choriocarcinoma since 1983, because of its different morphological features, biologic behavior and response to chemotherapy. Most cases of PSTT have indolent clinical courses, but there have been several reports showing malignant behavior resulted in death of the patient. We report a case of PSTT showing vaginal metastatis. In biopsy specimen, it was difficult to make differential diagnosis from squamous cell carcinoma of uterine cervix due to uniform and mononuclear cytologic characteristics. Immunohistochemical stains for hPL and hCG were not typical for either PSTT or choriocarcinoma. We suggest that poorly differentiated PSTT may synthesize the lesser amount of hPL than typical PSTT and it may behave more aggressively.

8.Measurement of Serum sIL-2R, sCD8 and TNF-alpha Levels in Patients with Myelodysplastic Syndrome and Acute Myeloid Leukemia.

BACKGROUND: Soluble IL-2R, soluble CD8 and TNF-alpha are elevated in sera of some patients with hematological malignancies, and a marked elevation of these cytokines could be used to assess disease activity and prognosis in this malignancy group. METHODS: The serum levels of sIL-2R, sCD8 and TNF-alpha were assessed in 28 patients with myelodysplastic syndrome (MDS) and 32 patients with acute myeloid leukemia (AML), and 39 cases of healthy control subjects to define clinical usefulness as prognostic markers by sandwich enzyme immunoassay. RESULTS: In MDS patients, serum sIL-2R levels were significantly higher as compared with controls, and a more pronounced increase of serum sIL-2R levels was found in patients with RAEB RAEB-t and CMML as compared with RA and RARS. Serum sCD8 levels were higher as compared with controls, but not related with FAB classification. In patients with leukemic conversion. sCD8 levels tended to be higher as compared with patients with non-conversion. The sIL-2R levels of AML patients were significantly higher than controls, and a significant correlation was detected between the levels of sIL-2R and WBC counts. Higher sIL-2R levels( >2000 U/ml) tended to affect both complete remission rate and survival. Serum sCD8 levels were higher than controls, but not related to FAB classification. No differences of serum TNF-alpha levels were detected as compared with healthy controls. CONCLUSIONS: From these results, this study indicates that serum sIL-2R and sCD8 are significantly increased in some patients with MDS and AML, and increased levels of serum sIL-2R and sCD8 may be useful for predicting prognosis of these patients.

10.Emphysematous Pyelonephritis in Diabetic Nephropathy A report of two cases.

Diabetic nephropathy is characterized by one or a combination of the following lesions: (1) glomerular involvement with three distinctive patterns: diffuse glomerulosclerosis, nodular glomerulosclerosis, and exudative lesions; (2) arteriolo sclerosis; (3) urinary tract bacterial infection with pyelonephritis and sometimes emphysematous pyelonephritis. Emphysematous pyelonephritis is an uncommon life-threatening and acute suppurative infection of the kidney, and usually occurs in diabetic female patients. It is characterized by the production of intraparenchymal gas. Glucose fermentation has been considered the main cause of the gas formation. We presented two illustrative nephrectomy cases of emphysematous pyelonephritis in addition to the typical pathologic features of diabetic nephropathy.
