5.Osteochondritis Dissecans

Osteochondritis dissecans occurs most commonly in knee joint and often in elbow, ankle, shoulder, hip than in other joint. In osteochondiritis dissecans, an area of subchondral bone undergoes avascular necrosis and degenerative change ususlly occurs in cartilage overlying it. During the course of disease, unless interrupted by treatment or spontaneous healing, the necrotic bone and cartilage seperates from adjacent bone and cartilage and together becomes loose body. To promote healing, the proper treatment is necessary before degenerative change developes. The authors reviewed 11 cases of osteochondritis dissecans which recieved treatment at Severance hospital from January 1972 to December 1981. The results were obtained as follows: 1. The incidence of osteochondritis dissecans in male was a little more frequent than that of female and 50% were observed in the age of 16 to 30 years. 2. The most common symptom and sign was pain and limitation of motion, instability senastion, tenderness, snapping sound in order of frequency. 3. The most common area involved was the knee in 9 cases, 5 cases involved the medial femoral condyle, 4 cases involved the lateral femoral condyle. The ankle and elbow was 1 case for each. 4. Eight(72.7%) of 11 cases had history of trauma. 5. Among 9 cases of knee inlovement, 5 (55.6%) cases were associated with rupture of meniscus. 6. The methods of treatment were as follows. Conservative treatment was performed in one case, but failed. So, all 11 cases recieved operative treatment and obtained satisfactory results in 72.8%.

6.The Emergence of the North Texas Korean American Nurses Society and its Contributions to Korean Immigrant Societies in the U.S..

PURPOSE: To describe the emergence of the North Texas Korean American Nurses Society (NTKANS) and to examine its sociocultural contributions to Korean immigrant societies in the U.S. for the last half century. METHODS: The study used retrospective historical analysis to explore the first North Texas Korean immigrant nurses' footsteps. Using Christy's historical research methodology, this study explored themes found in the NTKANS Minutes, the directories, and newspapers, and compared them with historical nursing contexts found in documents, immigrant nurse's pictures, and letters. Interviews with twenty first immigrant Korean nurses, the members, were also used as main data. RESULTS: Since its emergence in 1969, the NTKANS have contributed to local Korean societies through community medical services, financial supports to local Korean associations, local publications, and opening Korean school. In addition, the society has contributed to the other Korean immigrant nursing societies in the U.S. CONCLUSION: The sociocultural contributions NTKANS had made to Korean immigrant societies were not possible without its members' enormous efforts, personal struggles, and altruistic dedications. The trials and tribulations these immigrant nurses have overcome and the achievements they made for last half century would greatly inspire students and nurses in Korea who may seek international leadership and scholarship.

7.THE EFFECT OF SCREW HOLE SEALING METHOD ON ABUTMENT SCREW LOOSENING IN DENTAL IMPLANT.

One of the most common problems of implant prosthesis is the screw loosening of abutment screws. This brings on discomfort on mastication, inflammation in the peri-implant tissue due to poor oral hygiene and fracture of prosthesis or loss of osseointegration. To prevent screw loosening, appropriate implantation to direct the occlusal force to the long axis of the implant, accurate design of the superstructure, decrease of the occlusal table, and adequate torque on the abutment screw are necessary. In this study the screw loosening torque was evaluated in implants with dimples or flutes in the internal surface of abutment screw holes. The abutments were fastened with slot type and hexagonal type abutment screws and were sealed with vinyl poly siloxane impression and bite registration material respectively. The screw loosening torque was evaluated after 1,800 and 12, 600 times loading under a loading machine. The results were as follows. 1. The flute form group showed significantly higher loosening torque compared to the dimple form group and the group with no inner surface treatment (p<0.05). 2. There was no statistical difference in loosening torque according to the sealing materials. 3. The loosening torque according to the types of abutment screw showed no significant difference. 4. The loosening torque was significantly higher after 1800 times loading compared to 12600 times loading(p<0.05). From the above results, it is thought that formation of a flute in the internal surface of the screw hole decreases the chance of screw loosening, but the sealing materials and types of abutment screw did not show significant difference in prevention of screw loosening.

9.Small Cell Osteosarcoma Similar to Ewing's Sarcoma in Histologic Findings and MIC2 Expression: A case report.

Small cell osteosarcoma is a rare form of osteosarcoma and the histological differential diagnosis from other small round cell tumors (SRCTs) is difficult. The immunohistochemical stain for MIC2 has been considered an useful diagnostic marker for Ewing's sarcoma and primitive neuroectodermal tumors but recently, other SRCTs such as malignant lymphoma and embryonal rhabdomyosarcoma also showed positive reaction. Therefore, the usefulness of MIC2 must still be proven. We experienced a case of small cell osteosarcoma of the mandible in a 25-year-old man. Histologically, the tumor consisted of small round cells that resembled those of Ewing's sarcoma. Immunohistochemically, the tumor cells expressed diffuse strong positive reaction for MIC2 gene products. However, the scanty foci of lacy osteoid material between the tumor cells seemed to be diagnostic of osteosarcoma. The histologic and immunohistochemical findings of this case suggest close relationship between small cell osteosarcoma and Ewing's sarcoma.
