1.A Study on the Lived Experiences of the Family Caregivers in Senile Dementia.

The purpose of this study was to explore and describe the lived experience of the family caregivers with the demented elderly. For investigate purposes of this study, data collection was done from May 9th to October 16th 1997, by means of in-depth interviews with 6 individuals in caregiving families. The research question was "What are the lived experiences of being a family caregiver with the demented elderly? "All interviews were tape-recorded and transcribed for the analysis using Colaizzi's method. The main results of this study were as follows : 1) Family caregivers ascribed the cause of the dementia to 'environmental change' and the 'introverted personality of the elderly'. 2) Family caregivers let the demented elderly alone initially and then they restrained the elderly who was in a fit of dementia. 3) The coping response of family caregivers varied. The coping reponses were (1) having their moods go up and down, (2) enduring and praying, (3) avoiding the elderly, (4) hoping for release from responsibility, (5) enduring their conditions(obligations), (6) accepting the elderly, (7) taking an objective view, and (8) taking safety measures. 4) The long ordeal of coping with a demented elderly person resulted in the (1) loss of physical and psychological well-being, (2) ethical conflicts, (3) family conflicts, (4) become desperate, (5) rejection of the aging process, (6) sympathy and understanding for the elderly, (7) awareness that the support system is important and (8) hope of sharing their responsibility. The results of this study may help nurses to understand the lived experiences of the family caregivers with the demented elderly better, in order to provide more basic data for the development of educational programs for dementia family caregivers. It may help to make the coping process easier and more successful for the family members of the demented elderly.

2.A Cytogenetic Survey of the Mentally Retarded Children.

OBJECTIVE: A cytogenetic survey of the mentally retarded children in Seoul City Welfare Center for the Mentally Retarded and St Peter school has been undertaken. METHODS: The chromosome analysis was carried out in 92 males and in 66 females as a total of 158 cases. RESULTS: Abnormal karyotypes were observed in 22.2% of the total cases (35/158). Autosome and sex chromosome anomaly were observed in 20.3%(32/158) and 1.9%(3/158) respectively. Of 35 cases of anomaly, 91.4%(32/35) was autosome anomaly and 8.6%(3/35), sex chromosome anomaly. CONCLUSION: Down syndrome karyotype was the most frequent anomaly among autosome anomalies which was 62.5%(20/32) and other autosome anomalies were observed in 37.5%(12/32).

4.In Vitro Response of Uterine Endometrial Cancer Cell Lines to the Antiestrogen Tamoxifen.

Medroxyprogesterone acetate(MPA) is one of the most commonly used hormonal agents for the treatment of advanced or recurrent endometrial adenocarcinoma. However, the progesterone receptor content of endometrial carcinoma varies directly to the degree of differentiation and inversely with stage of the tumor. Thus one would predict that MPA therapy would be less effective in advanced and poorly differentiated tumors. In addition, MPA has been shown to reduce progesterone receptor content of both normal and malignant endometrial cells, which could result in loss of hormone responsiveness. Tamoxifen, which is often used in breast cancer therapy, has also been used in the treatment of patients with advanced and recurrent endometrial carcinoma. Tamoxifen is known to have some estrogenic effects at low concentration and one of these effects is induction of progesterone receptor both in normal and malignant endometrium. This property has focused interest on sequential or simultaneous use of tamoxifen and MPA in the therapy of endometrial carcinoma. The growth inhibitory effects of MPA and tamoxifen were tested on six longestablished endometrial carinoma cell line(HEC-1-A, HEC-1-B, RL 95-2, AN3CA, KLE) and on SCHE-1, a new endometrial carcinoma cell line established in our laboratory. MPA and tamoxifen were used in growth experiments either alone, simultaneously or sequentially. The MCF-7 breast cancer cell line was used as a control. Only 20% reduction in cell number was achieved after 10 days of exposure to the drug, even with the highest MPA concentration tested(10micronm) in endometrial carcinoma cell lines. But in MCF-7 cells, 60% reduction in cell number was achieved with the same concentration of MPA(10um). Ten days of feeding with 5micronm tamoxifen produced a 96% reduction in cell number in MCF-7, a 91% reduction in HEC-1-A, a 88% reduction in HEC-1-B, a 98% reduction in AN3CA and a 71% reduction in KLE cultures. In SCHE-1 cultures a 83% reduction in cell growth was seen and no viable cells remainde in RL 95-2 cultures after 10 days of feeding with a 5uM tamoxifen. In AN3CA cultures, simultaneous exposure to 5um tamoxifen and 5um MPA resulted in partial reversal of the tamoxifen-induced growth inhibition. In RL 95-2, HEC-1-A and HEC-1-B cultures, simultaneous use of these drugs had the same effect as tamoxifen alone, whereas in KLE and SCHE-1 cultures a slight additive growth effect was observed. All six endometrial carcinoma cell lines resumed logarithmic growth when medium containing tamoxifen of logarithmic growth under these conditions was slower than that in the other endometrial carcinoma cultures. Our results show that MPA does not have growth inhibitory effects in these endometrial carcinoma cell cultures, whereas tamoxifen has been shown to have potent endometrial carcinoma cells. These findings are of special importance since patients who are most likely to need adjuvant therapy for advanced or recurrent endometrial carcinoma are those with estrogen receptor and progesterone receptor negative tumors.

6.Primary Intrapericardial Lipoma Simulating Pericardial Effusion -Report of A Case-.

Primary pericardial tumors are rare than those originating within the myocardium or endocardium and, moreover, primary benign pericardial tumors are much rare in occurrence. Fine(1986) was able to collect only 43 cases of lipoma. Most cases are diagnosed only at autopsy. One fourth of the cardiac lipoma arise subepicardially, where they suggest the presence of pericardial effusion with compression or displacement of the heart. A case of primary intrapericardial lipoma simulating pericardial effusion is presented. This tumor originated in left anterior aspect of pericardium. The tumor was measured 30x30x5cm in size and 2,200mg in weight. Total excision of the mass was accomplished by posterior lateral thoracotomy incision, resulting in complete cure. This is the first case of primary benign lipoma of the pericardium in literatures in our knowledge.

7.A Study about promoting Health Lifestyles and Relating Variables on the Life-cycle of Women.

Promoting women's health lifestyles are important due to their connection to family health. The purpose of this study was to analyse women's health lifestyles(HLP) and their effects on women's life-cycle, so in order to develop a program in a women's health care center. The subjects included were 1080 women over 18 years old living in Wonju city, and were selected by stratified and purposive sampling. The data were collected through a questionnaire and interview. The Cronbach alpha, %, mean, ANOVA, Pearson's correlation, and regression in SPSS PC Win. package was used to analyze the data. The sample was sepernted into three groups premarital group 20.2%(premarital women between 18 and 40 years old), delivery and children rearing group 49.9%(marital women between 18 and 40 years old), over middle agedelderly group 29.9%(women over 41 years old). Significant difference were found in the HPL according to group. Also, relating variables, such as self-efficacy, family functions, health locus of control and gender role perception that were considered relating variables to HPL significantly differed among the tree groups. HLP significantly correlated with self-efficacy, family functions, HLOC and gender role perception in all participants and at all groups. The regression analysis of HPL was interpreted 40.6% with relating variables, self-efficacy, health attention, family functions, and internal locus of control, health perception, power other locus of control and chance locus of control in all participant. Self-efficacy, family functions, health attention were considered important variables in premarital group, self-efficacy, family functions, internal locus of control, health attention, health perception and power of control were important in delivery-rearing group. Self-efficacy, health attention, internal locus of control, family functions and health perception were important in middle aged-elderly group. As a result, we found the differences HPL scores and relating variables according to life-cycle groups. Therefore, we should prepare health promoting education programs for women according to women's life cycles. Also we suggest that women's health care centers based on communities was needed for proper management of women's health.

9.Two Cases of Chronic Otitis Media caused by Aspergillus terreus.

Fungi have been recognized as a significant cause of external otitis and it may be the primary pathogen or be part of a mixed infection. In the immunocompromised host, fungus is capable of producing infection in inner ear or middle ear. Otomycoses are most frequently caused by Aspergillus spp. and Candida sap. There are few reports that Aspergillus species other than A. fumigatus, A. niger and f. flavus have caused chronic otitis media. We report two cases of chronic otitis media caused by Aspergillus ferrous in Korea. One case is a 7-year-old girl who had recurrent serous otorrhea and otalgia for 4 years, was reattended otolaryngology clinics with otorrhea of 3 days durations and another is a 6-year-old girl who had serous otorrhea for 2 months and 3 day fever, was attended otolaryngology clinics with them. Microscopic appearance and colony morphology from ear discharge cultures revealed A. ferrous. The infection responded well to topical ketoconazole therapy. This report should help to raise medical personnel's awareness of such human opportunistic fungal ear infections.
