1.Leukonychia Striata Caused by Trichophyton Rubrum.

The authors observed 5 cases with leukonychia striata caused by Trichophyton rubrum infection. They were 4 males and 1 female in the 28-32 years age group. In 4 of the 5 cases the lesions disappeared in 1-3 months with treatment-avulsion of the involved nails followed by oral griseofulvin 500mg/day, one case did not have follow-up observation.

2.A study on the Compliance and Educational Demand of Renal Transplantation Patient.

This study was started for the purpose of providing the basic data for continous managment of kidney transplantation patients after discharge. This study was conducted on 180 patients who received renal transplants at three hospital ( B, M. P) pusan, korea.. The data collection was done for june 1. to August 31. 2002. General characteristics, renal transplantaton characteristics. physical characteristics, the level of compliance and the degree of educational demand were done by the number and percentage. the mean. standard deviation. The level of compliance and educational demand followed by the characteristics of general and kidney transplantation were analyzed by t-test and ANOVA. The result were as fallows: 1. Man was higher than woman such as 60.0%, Mean age was 42.5 years old. Average total duration of after operation was 5.5 years. 2. Cases of systolic blood pressure over 140mmHg were 10.0%, cases of diastolic blood pressure over 90mmHg were 22.8% and obesity factor in BMI was 15.6%. The person who daily water intake amount is 5000cc was 0,6%, the case that daily urine output is below 1000cc was 8.9%. and the case that urine output is zero was 0,6%. 3. The mean score of compliance was 77.47 point. The score in medication part was highest such as 4.67 point, that in stress situation was lowest such as 3,50 point. 4. The average score of educational demand was 154.02 point. and physical state part was 4.36 points highest. activation part was 3.48 points lowest. As a role of nurse Confirmation of compliance is very important encourage to make good through regular hospital visitation. point out the noncompliance part and then increase compliance of renal transplantation patient As well there will be maintain the normal kidney function to satisfy educational demand through continous education.

3.Self Rated Health and Mortality in Elderly Kangwha Cohort, 8-year follow up.

The effect of self rated health on all cause mortality was. examined in a community residents sample of 2, 848 men and 3, 534 women aged 55 years or over who has been followed for eight years, 1985-1993. This longitudinal study, conducted in Kangwha County, Keanggido, began in 1985 with baseline examinations of physical and life style characteristics. The results indicate that the self-rated health has a profound and independent effect on all cause mortality during the study period. Cox's hazard model estimates that persons with 'poor' self-rated health has 1.7 times (CI 1.2-2. 6) higher risk of all cause mortality than persons with 'excellent' self-rated health and persons with 'worst' one has 4.2 (CI 2.6-6.2) in male. Estimates for women are similar to those of men. Further researchs to investigate the effects on cause specfic mortality and effects to reveal the work mechanism of self-rated health are highly expected.

7.Maximum Detrusor Pressure Measurement by Eyeball Urodynamic Study in the Diagnosis and Postoperative Follow-up in BPH.

It is known that the diagnostic accuracy is about 75% by conventional method without pressure/ flow study in BPH. But we cannot apply the pressure/flow study to the every patient in the diagnosis of BPH due to high cost of equipment and invasiveness of the study. Generally, in compensatory phase with obstruction, we can diagnose obstruction easily because maximum urine flow rate is decreased and maximum intravesical pressure is high. But in decompensatory phase with obstruction we must perform invasive pressure/flow study to differentiate obstruction from the patient with decreased detrusor contractility without obstruction because urine flow rate is decreased and maximum intravesical pressure is low in both cases. We diagnosed obstruction if the maximum flow rate is lower than 15 ml/sec and the maximum intravesical pressure is higher than 50 cm H2O by eyeball urodynamic study and if URA is higher than 29 cm H2O by invasive pressure/flow study in the patient whose maximum flow rate is lower than 15 cc/sec with less than 50 cm H2O of maximum intravesical pressure by eyeball urodynamic study. We diagnosed 141 BPH patients in which 116 (82.3%) patients by eyeball urodynamic study and 25 (17.7%) patients by pressure/flow study and treated them by open prostatectomy or TURP. Symptoms were improved in 120 (83%) patients within 3 months. Ten (8.0%) out of 21 (17%) patients whose symptoms were not improved after 3 months with higher than 15 ml/sec of maximum flow rate were diagnosed as increased detrusor contractility. Eyeball urodynamic study was performed in the other 11 (8.9%) patients with lower than 15 ml/sec of maximum flow rate. Six (4.8%) of them were diagnosed as decreased detrusor contractility due to lower than 50 cm H2O of maximum intravesical pressure and the other 5 (4.O%) were diagnosed as obstruction due to higher than 50 cm H2O of maximum intravesical pressure in eyeball urodynamic study. Five (4.0%) patients had urinary incontinence. Four (3.2%) out of them had detrusor contractility and the other one had obstruction. Alpha blockers or anticholinergics were applied in the patient with increased detrusor contractility, CIC or Foley catheter was indwelled in the patients with decreased detrusor contractility. Repeated TURP or urethral dilation was applied to the patient with obstruction After all symptoms were improved in all patients except 3 (2.4%). In conclusion maximum detrusor pressure measurement with eyeball urodynamic study is very useful and less sophisticated method in the diagnosis and postoperative follow-up in BPH.

8.Anaphylactoid Reaction induced by Muscle Relaxants .

Severe bronchospasm, hypotension and skin rash were observed in a 23 year-old woman after injection of gallamine iodide. Her past history revealed no asthmatic and allergic diathesis. Initial administration of succinylcholine induced a mild degree of bronchspasm and urticaria-like skin rash, which were intensified in severity after gallamine injection under the anesthetic state. Administration of hydrocortisone 500mg I.V. and antihistaminic(Avil), 100pmg I.M., produced successful relief of bronchospasm with circulatory embarrassment. A review was done as to histamine libration and bronchospasm due to succinylcholine, gallamine, d-tubocurarine, thiopental and atropine. The value of intradermal skin rash test was discussed.

9.Genetic Polymorphism of PAI-1 Gene and Cardiovascular Disease: eta-analysis of Case-Control Studies.

BACKGROUND: Previous reports have suggested that alleles at the plasminogen activator inhibitor-1 (PAI-1) gene are associated with increased risk of developing coronary artery disease, including myocardial infarction and stroke through their effect on PAI-1 levels. Method: We attempted to search English literatures for all reports of possible effects of PAI-1 gene on cardiovascular disease in human published prior to November 1998. We used a Mantel-Haenszel method (fixed effect model) and random effect model, respectively, to perform a meta-analysis of 7 case-control studies that provided information related to the effects of PAI-1 gene on risk of cardiovascular disease. RESULTS: From 7 studies for diagnosed cardiovascular disease, the relative frequencies of the three genotypes among controls was (5G/5G) (homozygous normal), 24.5%; (4G/5G) (heterozygous), 48.2%, and (4G/4G) (homozygous for the mutant, 675 GGGG), 27.3%. These relative frequencies in cases were 21.7% for 5G/5G, 48.0% for 4G/5G, and 30.3% for 4G/4G. In fixed effect model, compared with those with genotype (5G/5G), the overall odds ratio (OR) for cardiovascular disease among those with (4G/5G) was 1.12 (95% CI, 0.93 to 1.34), and it was 1.20 (1.01 to 1.44) for the (4G/4G) genotype. For five studies with myocardial infarction as the outcome, the overall OR of myocardial infarction was 1.20 (0.99 to 1.47) for those with (4G/5G) and 1.24 (1.00, 1.54) for those with (4G/4G) genotypes, respectively. CONCLUSION: Our findings provide support for the weak association between PAI-1 gene and cardiovascular disease, in particular, myocardial infarction.

10.Evaluation of Cognitive Functions, Depression, Life Satisfaction among the Elderly Receiving Visiting Nursing Services.

PURPOSE: The purpose of this study was to examine cognitive functions, depression, and life satisfaction among the elderly receiving visiting nursing services. METHODS: The subjects of this study consisted of 221 elderly individuals who were receiving visiting nursing services from a public health center located in Gyeonggi-do. The Mini-mental State Examination Korea version (MMSE-K), Geriatric Depression Scale-short form-Korea version (GDS-K), and Life Satisfaction Scale were utilized to determine cognitive functions, depression, and life satisfaction. Collected data was analyzed by the SPSS PC Win 12.0 program. RESULTS: The prevalence of cognitive impairment (MMSE-K< or =24) was 86.4%, and the proportion of severe depression (GDS-K> or =10) was 22.6%. Older age, lower education, and low income were associated with cognitive impairment, and low education, no religion, and low income were associated with depression. Cognitive function negatively correlated with depression. Life satisfaction level was fairly low and was not associated with any variables. CONCLUSION: The high prevalence of cognitive impairment and low life satisfaction status were determined among low-income elderly who were registered at a visiting health care center. Thus, in the future at visiting health care services, efficient and various programs for the elderly should be tried, to improve cognitive functions and reduce depression. Additionally, consistent evaluation studies for those programs will be required.
