1.Cystic Lymphangioma of the Stomach: A case report.

Cystic lymphangiomas of the stomach are extremely rare. These usually present as asymptomatic, polypoid lesion consisting of cystically dilated lymphatic vessels, which are submucosal in location. We report a case of cystic lymphangioma of the stomach. The patient is a 55 year-old woman who has complained of dull pain on epigastrium for 10 years. The mass measures 6x5 cm in cross diameter and is mainly located in the subserosal layer along lesser curvature. Microscopically, the tumor reveals a large number of dilated lymphatic cysts containing serous fluid, lined by a layer of flattened endothelial cells. Ultrastructurally, lining endothelial cells show thin discontinuous basal lamina, in contrast to normal lymphatics.

2.Skin Hydration Status and Skin Surface pH According to the Body Parts of the Aged with Immobilization.

PURPOSE: This study was performed to identify skin hydration status and skin surface pH according to the body parts of the aged with immobilization. METHODS: The subjects were 101 aged patients in a hospital and a institution for the elderly in Seoul. Data for skin hydration and skin surface pH were collected using corneo-meter and skin pH-meter. The body parts of measurement were 10 parts; face, forearm, back of the hand, flank, upper abdomen, leg, back of the foot, heel, scapular, and sacrum. The data were analyzed by SPSS/WIN 12.0 program. RESULTS: Skin hydration status were relatively low especially on lower extremity and back of body, and skin surface pH of scapular and sacrum were high. There was significant negative correlation between skin hydration status and skin surface pH on the 6 body parts. CONCLUSION: Skin hydration status and skin surface pH, especially on scapular, sacrum, and lower extremity need to be considered as important control factors for increasing skin health status of the aged with immobilization.

3.An Analysis of Articles for International Marriage Immigrant Women Related to Health.

OBJECTIVES: This study was for analyzing the research about international marriage immigrant women and a trial to find the right direction for future research. METHODS: Sixty articles published from June, 2004 to June, 2009 were reviewed and analyzed according to the general characteristics, major of author, and theme of health domains. RESULTS: Most of them were master's thesis(71.7%) and journals(21.7%) and doctoral dissertation(6.7%) have been published mostly after thesis. Among 83.3% for quantitative research, descriptive(33.3%) and descriptive correlation(41.7%) methods were the most used and there were some qualitative researches(16.7%). The most frequently used data gathering method was questionnaire(81.7%) and the next was interview(16.7%). The major rates of the author were 61.7% for social welfare and 2.1% for nursing. The investigated variables in social health domain were adaptation(28.3%), and communication(1.7%). In psychological health domain, marriage satisfaction(16.7%), life satisfaction(11.7%), and depression(10.0%) were most researched. Utilization of medical center(5.0%) and health promotion behavior(1.7%) were investigated in physical health domain. CONCLUSIONS: Above this, most articles were researched about the adaptation of international marriage immigrant women. But the life in foreign countries can cause physical and psychosocial unhealthy conditions, so many-sided health related researches are supposed to be conducted for adaptation and prevention health problems of international marriage immigrant women.

4.Expression of GnRH-receptor mRNA in the Placenta during Pregnancy.

OBJECTIVE: Our previous study demonstrated that the placental GnRH and GnRH mRNA did not parallel the time course of hCG secretion, though it is thought to be one of the potential paracrine regulators of hCG secretion from the trophoblasts. The present study was designed to examine the potential variation in GnRH-receptor mRNA expression in the placenta, which may account for the GnRH-mediated action of hCG secretion during pregnancy. METHODS: Human placentas in firt, second, and third trimester of normal pregnancy were obtained. These placentas were fixed with 4% paraformaldehyde and embedded in OCT compound, and sectioned by cryostat. For in situ hybridization, S labeled RNA probes were used and followed by autoradiography. RESULTS: The GnRH-receptor mRNA signals were present in both cytotrophoblast and syncytiotrophoblast cell layers. Signal intensities varied with gestational ages and were abundant at 6-7 weeks, peaked at 9-12weeks, declined at 14 and 24 weeks, and were barely detectable at term. The present study demonstrates that GnRH-receptor mRNA exhibits changes paralleling the time course of hCG secretion during pregnancy CONCLUSION: These data provide mechanistic understanding that the paracrine/autocrine regulation of hCG secretion by placental GnRH is mediated through an increase followed by a decline in GnRH-receptor mRNA expression from the first trimester to term placenta.

5.A Case of Furosemide Induced Acute Interstitial Nephritis.

Drug-induced acute interstitial nephritis is characterized by renal interstitial inflammatory cell infiltration and commonly presents as acute renal failure. This is caused mainly by methicillin, non-steroidal antiinflammatory drugs, sulfonamide diuretics such as thiazide, but cases induced by furosemide are rare. We report a patient with acute interstitial nephritis causing reversible acute renal failure and dermatitis while she was taking furosemide. A 37-year old woman was referred to our hospital because of generalized skin rash and non-oliguric acute renal failure. She had peripheral eosinophilia (1,577/mm3) and serum creatinine level of 6.8mg/dL. Skin biopsy showed leukoclastic vasculitis and percutaneous renal biopsy showed severe interstitial infiltration of lymph ocyte and mild interstitial fibrosis with focal tubular atrophy. After withdrawal of furosemide, renal function and skin lesions were completely recovered.

6.Successful Angioplasty using Gadopentetate Dimeglumine in a Patient with Chronic Renal Insufficiency.

Iodinated contrast-induced acute renal failure is estimated to occur in 0.15 to 2% of all patients undergoing contrast imaging studies. Incidence is higher in patients with renal insufficiency, diabetes mellitus, dehydration, multiple myeloma, congestive heart failure, advanced age. We here report successful vascular interventional procedure by using gadopentetate dimeglumine(Gd-DTPA) as a contrast agent in a patient with chronic renal insufficiency and right superficial femoral artery stenosis. The patient had a history of iodinated contrast-induced acute renal failure. Gd-DTPA(0.17mmoVkg) diluted 1: 1 with 0.9% norrnal saline was used as contrast agent for the interventional procedure. Percutaneous transluminal angioplasty was successfully performed and there was no evidence of contrast material- induced acute renal failure after the procedure. Gd- DTPA is an alternative contrast agent for patients with chronic renal insufficiency.

7.The Analysis of Trends in Complementary and Alternative Therapy (CAT) in Nursing Research in Korea.

PURPOSE: The purpose of this study was to analyze trends in complementary and alternative therapy in nursing research in Korea. METHOD: The researchers examined academic theses and dissertations published from 1990 to 2002, and 151 articles were used for the analysis. RESULT: Nursing research on CAT increased rapidly from 1995. Articles with quantitative research designs made up 93.9% of the total and there were more experimental studies than non-experimental studies. Patients who had surgery, mental disorders, renal failure, hypertension, arthritis, dementia & cancer were the most frequently participants in studies on CAT. The type of CAT used in nursing research were mind-body therapy (65.8%), manual healing therapy (28.7%), phamacologic & biological therapy (3.7%), bioelectromagnetics (0.9%) and herbal therapy (0.9%). In 44 articles both psychological and physiological parameters were used as dependent variables. In 34 articles only physiological parameters were used and in 13 only psychological parameters. The most frequently used physiological parameters in CAT were pain, physiological function and vital signs, while the most used psychological parameters were anxiety, depression and stress. CONCLUSION: More studies about CAT are needed to extend the role and fields for professional nursing. There is a need to conduct qualitative studies in nursing about the experiences of patients who receive CAT and nurses who use CAT.

8.Liquid-Based Cytology Using MonoPrep2(TM) System in Cervicovaginal Cytology: Comparative Study with Conventional Pap Smear and Histology.

We compared the diagnostic accuracy of liquid-based cervicovaginal cytology using MonoPrep2(TM) system (Monogen, Herndon, Virginia, USA), a manual system based on membrane filtration method, with conventional Pap smear. Study population included 92 patients visiting the gynecologic department under the suspicion of uterine cervical disease. In thirty of them, surgical biopsy was performed. MonoPrep2(TM) system provided well-preserved monolayer specimen with good nuclear morphology. However, about 19% of specimens were inadequate to interpret due to low cellularity. The detection rate of abnormal cells more than ASCUS (atypical squamous cells of unknown significance) was 23.9% and higher than 19.4 % of conventional Pap smear. Diagnostic concordance rate with conventional Pap smear was 81%, and severe discordance rate influencing on the management of patient was 7.6 %. Among these seven cases, MonoPrep2(TM) system was more diagnostic only in four. In comparison with histology, the sensitivity of diagnosis of MonoPrep2(TM) system was 78.9% and slightly higher than 73.5% of conventional Pap smear. However, the specificity was 81.1% and lower than 90.9% of Pap smear. In conclusion, MonoPrep2(TM) system provided diagnostic accuracies similar to the conventional Pap smear. The inexpertness of slide preparation and the low cellularity were considered to endow a limitation in more accurate evaluation.

9.Descriptive Correlational Study on Personality Type using MBTI and Job Satisfaction.

PURPOSE: This study was done to examine the correlations between personality type and job satisfaction of nurses. METHODS: Subjects included three hundred twelve nurses ages from 21 to 55. The Myers-Briggs Type Indicator(MBTI) was administered to determine the personality types of the nurses(n=312). A job satisfaction survey including a demographic and career questionnaire was also used. Data were analyzed using ANOVA. RESULTS: The mean score for job satisfaction was 113.68 out of 205. The result showed that there was no statistically significant correlation between job satisfaction and information gathering functions ; S: Sensing, N: Intuition (t=.667, p=.499), T: Thinking, F: Feeling (t=.822, p=.412). The results, however, indicated that style for gaining energy(I: Introversion, E: Extroversion)(t=2.008, p=.046) and life dealing style(J : Judgement, P : Perception)(t=2.290, p=.023) were significantly correlated with job satisfaction. The extrovert focuses on the outside and get energy through interacting with people and doing things. Judgement type is described as organized, decisive, and orderly. CONCLUSION: Nurses in the extrovert and judgement types were more satisfied with their jobs than those in the introvert and perceptive type. Other results support that educational level, marital status, job characteristics, clinical experiences, motivation for nursing, and night duty were correlated with job satisfaction.

10.The Injury and its Related Factors in the Elderly Using the Data of 2008 Community Health Survey.

OBJECTIVE: The objectives were to estimate the rate of the injury in the elderly over the past year and to identify factors related to injury in the elderly in South Korea. METHOD: Using data from the 2008 Community Health Survey, 43,049 elderly persons, aged 65 years and older, were selected as study subjects. Their experience of injury during the past year and other variables, including socio-demographic factors, health-related factors, and diagnosed chronic diseases, were used. The chi-squared test and multiple logistic regression analysis with weighted analysis were conducted and statistical significance was set at p<0.05. RESULT: The rate of injury in the elderly during the past year was 5.1%. The most common types of the injury were falling/slipping down and traffic accidents. Factors related to injury in the elderly were living alone, current drinking, depression, poor self-rated health, stroke, and osteoporosis. Living alone (odds ratio 1.23, 95% CI: 1.05-1.45), current drinkers (OR 1.19, 95% CI: 1.05-1.35), poor self-rated health (OR 1.72, 95% CI: 1.43-2.08), depression (OR 1.23, 95% CI: 1.17-1.68), and history of stroke (OR 1.40, 95% CI: 1.17-1.68), and history of osteoporosis (OR 1.45, 95% CI: 1.26-1.66) were related to an increased risk of injury. CONCLUSIONS: Intervention programs that consider the risk factors related to injury should be developed and implemented to decrease and prevent injuries in the elderly.
