1.A Comparative Study of DNA Quantitation by Image Cytometry and Flow Cytometry.

There are substantial evidences suggesting that DNA content of tumors may provide the prognostic information with independent significances. In this study, the results of DNA ploidy analyzed by image cytometry on touch imprint and by flow cytometry on fresh cell suspension of 78 solid tumors were compared. For 68 cases, there was an excellent correlation between two methods. For 6 cases, an aneuploid population was found by image, but not by flow cytometry one case had an aneuploid peak by flow cytometry. Two methods may use in a complementary fashion m identify as many aneuploid cell population as possible.
2.Improved Reporting Rate of Notifiable Communicable Diseases through a Disease Code-Based Surveillance and In-House Electronic Reporting System.

Background: Reporting rate of notifiable communicable diseases has been assumed to be low in Korea. To encourage timely reporting of notifiable communicable diseases and to improve the reporting process by the Infection Control Unit, a disease code-based electronic surveillance and in-house electronic reporting system was developed and implemented in February 2003. We evaluated the reporting rates of notifiable communicable diseases before and after the implementation of the electronic reporting- system. METHODS: The electronic reporting system, which was integrated to the computerized physician order entry system, was designed to monitor KCD-4 codes at the time of every order entry. Whenever the system recognized the predetermined codes of notifiable communicable diseases, it generated an alert box to recommend reporting: and then a physician could fill up an electronic report form, which was collected and faxed to the local public health authority by the Infection Control Unit. Reporting rate of notifiable communicable diseases in 2002, when the conventional paper-based reporting system was used, was assumed comparing the number of cases reported actually and that of cases identified to be reported by reviewing the medical records and electronic database. RESULTS: From March to July 2003, 181 cases with tuberculosis and 14 with other communicable diseases were reported whereas 106 and 9 cases were reported respectively for the same 5-month period in 2002. Reporting rate using the conventional reporting system in 2002 was assumed to be 61% and 39% respectively. Conclusions: Using a disease code-based electronic reporting system, reporting of notifiable communicable diseases could be improved compared with conventional paper-based reporting system.

4.A Structural model for Quality of Life in Women Having Hysterectomies.

The purpose of this study was to develope and test the structural model for quality of life in women having hysterectomies. A hypothetical model was constructed on the basis of previous studies and a review of literature. The conceptual framework was built around eight constructs. Exogenous variables included in this model were marital intimacy, importance of uterus, professional support, positive coping behavior and pre-operative symptoms. Endogenous variables were spouse's support sense of loss and quality of life. Empirical data for testing the hypothetical model was collected using a self-report questionnaire from 203 women having hysterectomies at the outpatient clinics of four general hospitals and a mail survey in Pusan City. The Data was collected from December, 1997 to January, 1998. Reliability of the eight instruments was tested with Cronbach's alpha which ranged from 0.639-0.915. For the data analysis, SPSS 7.5 WIN Program and LISREL 8.12 WIN Program were used for descriptive statistics and covariance structural analysis. The results of covariance structure analysis were as follows : 1. Hypothetical model showed a good fit with the empirical data. [x2=6.93(df=5, P=.23), GFI=.99, AGFI=.94, RMSR=.019, NNFI=.97, NFI=.98, CN=440, standardized residuals(-2.14-2.10)] 2. For the parsimony of model, a modified model was constructed by deleting 3 paths and adding 1 path according to the criteria of statistical significance and meaning. 3. the modified model also showed a good fit with the data. [x2=5.26(df=7, P=.63), GFI=.99, AGFI=.97, RMSR=.014, NNFI=1.02, NFI=.99, CN=710, standardized residuals(-1.46-1.70)] Results of the testing of the hypothesis were as follows : 1. Marital intimacy(gamma11=.78, t=14.37) and professional support(gamma13=.12, t=2.12) had a significant direct effect on the spouse's support. 2. Pre-operative symptoms(gamma25=.32, t=3.12) , importance of uterus(gamma22=.20, t=2.61) and spouse's support(beta21=-.19, t=-2.43) had a significant direct effect on the sense of loss. 3. Sense of loss(beta32=-.66, t=-9.83) had a direct effect on the quality of life. Marital intimacy had a direct(gamma31=.19, t=3.33), indirect(gamma31=.14, t=2.52) and total effect(gamma31=.25, t=4.41) on the quality of life. Professional support had a direct effect(gamma33=.11, t=2.07) and total effect(gamma33=.13, t=2.31) on the quality of life. The direct effect of pre-operative symptoms(gamma35=-.36, t=-4.02) and positive coping behavior(gamma34=.15, t=2.06) had the insignificant effect on the quality of life while, due to the indirect effect these variables had overall significant effect on the quality of life. The results of this study showed that the sense of loss had the most significant direct effect on the quality of life. Marital intimacy, pre-operative symptoms and spouse's support had a significant direct effect on this sense of loss. These four variables, the sense of loss, marital intimacy, pre-operative symptoms and spouse's support, were identified as relatively important variables. The results of this study suggested that there is needed to determine if nursing intervention would alleviate this sense of loss and promote a greater quality of life in women who have had hysterectomies.

6.Effect of Nutritional Education and Exercise Intervention on Reducing and Maintaining Weight in Obese Women.

This study was performed to investigate the effectiveness of nutritional education and exercise intervention on reduction and maintenance of weight. The subjects were 24 obese women whose body mass index (BMI) was over 25 kg/m(2). Nutritional education was performed once a week and swimming was done three times a week throughout this 12 week program. The subjects were recalled 1 month after the program was complete. Nutrient intakes were assessed by 24 hour recall method. Also food habits, dietary behaviors and nutritional knowledge were investigated by self-administered questionnaires before and after the weight control program and one month after completing the program. Height, weight, body composition and blood levels were measured before and after the weight control program and one month after completing the program. During the 12 weeks of the weight control program, body weight significantly decreased from an average of 69.7 kg to 65.8 kg and to 65.1kg after one month (p < 0.05). Body fat and BMI also significantly decreased (p < 0.01). Cholesterol and blood sugar levels decreased after 12 weeks and increased one month after completing the program. After taking nutritional education, the nutritional knowledge scores increased. Calcium intake significantly increased after completing the education and one month after completing the program (p < 0.01). Iron intake significantly decreased from an average of 12.1 g to 11.3 g after completing the program and increased to 15.5 g one month after completing the progrom (p < 0.001). We concluded that our nutritional education and exercise program was effective for reducing and maintaining weight.

7.Factors Related to Quality of Life among Rural Elderly.

PURPOSE: This study was conducted to explore the degree of depression, perceived health status, chronic disease and quality of life(QOL) among rural elderly and to determine the factors related to their QOL. METHOD: The design of this study was a correlational study. The subjects were 423 elderly consisted of 157(37.1%) men and 266(62.9%) women dwelling in a rural area of N City. Data were collected from May to December, 2003 using a structured questionnaire. A stepwise multiple regression analysis was performed to identify the factors related to the QOL. RESULT: It was found that the mean score of QOL was in total with 2.15 out of 5.00 and women elderly's score was significantly lower than men(t=2.20, p=.028). Perceived health status showed statistically significant positive relationship with QOL(r=.608, p<.05), while depression(r=-.751, p<.01) and chronic illness(r=-.336, p<.01) showed statistically significant negative relationship. Depression was found to have the highest correlation with QOL among the subjects. Depression score explained QOL at the most, accounting for 36.8% of the variability, followed by perceived health(8.2%) and the number of chronic illness(.7%). Other factors related to the QOL were economic status and absence of spouse. CONCLUSION: In order to increase the QOL of rural elderly, it is necessary to decrease the depression, to increase their perceived health status and to decrease the number of chronic illness. We suggest the implementation of a program not only to promote physical health status and self-care ability but to take care of mental health for the rural elderly.

8.The Sexual Satisfaction in Married Women.

This study was conducted to explore the level of sexual satisfaction in married women. This is a descriptive study. The data was collected from July 19 to Aug 10, 1997 utilizing a questionnaire developed by researchers. Sexual satisfaction(alpha=.926) was measured by the Sexual satisfaction scale(Kim at el, 1997) which is consisted of 17-items. And data was analysed using SPSS/PC+ 7.5. The following is a summary of the study. 1. The study surveyed a total of 400 married women from Seoul, Kyungki do, and Pusan. The mean age of the subjects was 38.61 years with the range from 22 to 69 years. A large proportion of the subjects(69.3%) had received a high school education or further education. In terms of the marital satisfaction, 68.6% of subjects were moderately or highly satisfied; 6.8% were dissatisfied. 2. The mean score and standard deviation of the sexual satisfaction was 42.74+/-10.42 with the range from 17 to 68 score. 3. The variables influencing on sexual satisfaction were age, education level, income, job, stress, and perceived marital satisfaction.

9.The Effect of Reminiscence with Audio-Visual Stimulation on Senile Dementia.

The purpose of this study was to identify the effect on improvement of the Activity of Daily Living (ADL) and decrease the cognitive function and agitation behaviors by reminiscence with audio-visual stimulation for senile dementia. The quasi-experimental design was used in this study. Subjects were 26 with mild senile dementia who were cared for at a Day Care Center for Dementia in Seoul. The data were collected from March to July, 1999. Subjects were divided into three groups: Control I group with 10 subjects, reminiscence group(Control II group with 8 subjects), and reminiscence with audio-visual stimulation group(experimental group with 8 subjects). The Control I group got routine care as usual. Control II group participated in reminiscence sessions for one hour a day, five times a week , for a period of 4 weeks. The experimental group participated in reminiscence with audio-visual stimulation sessions for one hour a day, five times a week, for a period of 4 weeks. Instruments of this study were color photography with sound that was developed through an open questionnaire about events, objects, humans in action and animals that 100 Korean elderly over 60 would like to memorize. This was referred from the Sensory Stimuli Package by Namazi and Haynes(1994). The effects of treatment was evaluated through MMSE-K by Kwon & Park(1989). Also the Brief Cognitive Rating Scale(BCRS) by Reisberg et al(1983) for the cognitive function, through Agitation Inventory by Cohen- Mansfield and Colleague(1989) for behavioral response and through the Rapid Disability Rating Scale-2(RDRS-2) by Linn & Linn(1982) for the activity of daily living respectively. Data analysis was done using SPSS for X2-test, ANOVA, repeated measures ANOVA. The results were as follows : 1. Reminiscence with audio-visual stimulation did not improve cognitive function for senile dementia, but significantly improved verbal expression, the subscale of cognitive function. 2. Reminiscence with audio-visual stimulation reduced agitation behavior of experimental group significantly, but there was no significant difference between groups. 3. Reminiscence with audio-visual stimulation did not significantly effect the activity of daily living after treatment. In conclusion, it was shown that the reminiscence with audio-visual stimulation was an effective therapy to improve verbal expression and to reduce agitation behaviors of senile dementia. Further research with more indepth approach is needed, considering characteristic and level individualized for each senile dementia.

10.A Study on the Degree of Self-care Activities of Stroke Patients, the Family Burden, and the Educational Needs of Primary Caregivers.

The purpose of this study was to identify the degree of self-care activities of stroke patients, the family burden, and the educational needs of primary caregivers. A descriptive survey research was conducted in 97 stroke patients and their respective primary caregivers. Data were collected the using 26th to by interviews a structure questionnare from July August 5th, 1999. NIH stroke status was 12.15+/-8.40, the degree of self-care activities of stroke patients was 26.85+/-9.39. The score of objective burden of primary caregivers was 31.24+/-4.81 and subjective burden was 24.30+/-6.99. The score of the educational needs was 89.78+/-9.99. We present several imformations about clients from these scores as follows: The patients were convalescent and their depentant level of self-care was middle range: The objective burden was higher than the subjective burden: And the educational needs of family caregivers were very high. The Elain Mattis Educational Wants of Family Caregivers of Disabled Adults Questionnaire was used to assess the educational needs of primary caregivers of stroke patients. The results of this research are offered to help nurses understand the feelings of uncertainty about the new and unexpected role that family caregivers face and to help nurses meet the needs of families preparing to care stroke survivors at home.
