1.National Guidelines for Breast Cancer Screening.

Breast cancer is the school most common cancer in Korea women. The incidence of breast cancer is around 25 per 100,000 women, and more than 5,500 women are diagnosed as breast cancer annually. Epidemics show that the incidence and mortality of breast cancer are increasing due to rapid changes of women's life style and westernized food and so on. The risk factors for breast cancer include hormone-related factors (early menarche, late menopause, no or late birth, hormone replacement therapy) and genetic background. It is hard to change risk factors for breast cancer but early detection strategies are best for decreasing the mortality rate from breast cancer. Breast self examination, clinical breast examination, and mammography can be used for screening of breast cancer. Breast self examination is not sensitive enough to detect a small cancer but monthly exam makes women be awakening about breast cancer and feel changes of her breast. Meta-analysis shows annual mammography reduces breast cancer mortality around 35% in the ages over 50. Some randomized clinical trials also showed mortality reduction by mammography in the ages of 40s. Surprisingly, breast cancer is rapidly increasing in late thirties with a peak incidence in forties among Korean women. Whether the early peak makes mammography less accurate is controversial. However, data from qualified screening centers showed mammography has same sensitivity with that in westerns. Korean Breast Cancer Society and National Cancer Center organized the 1st Consensus meeting for the national guidelines for breast cancer screening on July 24, 2001. We recommend that women should have monthly self examination beginning at age 30, should receive biannual clinical breast exam from healthcare providers from age 35, and should receive clinical breast exam and mammography at 1~2-year intervals after age 40.

2.Prognostic Value and Histologic Correlation of Sonography in Childhood Nephrotic Syndrome.

The uterine cervix is an uncommon site of primary non-Hodgkin's lymphoma (NHL). Although the cytologic findings of NHLs are well known, most cervicovaginal smear of uterine NHLs give lower diagnostic yield than common epithelial malignancy because abnormal cells do not appear in the sample in the absence of surface ulceration. Herein, we describe cytologic findings of a case of uterine cervical NHL which was initially diagnosed by cervicovaginal smear. The tumor cells were relatively uniform, isolated, large-sized with scanty cytoplasm and round or indented nuclei. The nuclei had stippled chromatin and small nucleoli. Histologically and immunohistochemically the tumor was proven to be large cell lymphoma of T-cell lineage.

4.Effects of OSCE Method on Performance of Clinical Skills of Students in Fundamentals of Nursing Course.

PURPOSE: The main purpose of this study was to compare the effects of OSCE method and the traditional evaluation method on nursing skills performance of students enrolled in fundamentals of nursing courses. METHOD: The study was designed as a quasi-experimental, nonequivalent control group post test design with two classes of sophomore students attending fundamentals of nursing courses at one baccaleureate nursing school located in a metropolitan Seoul area. The control group was evaluated using a traditional method and the experimental group was evaluated using the OSCE method. Data were collected from June, 2001 to June, 2002. There were 35 students in the OSCE method group and 39 students in the control group. Data analysis was done using SPSS WIN 9.0. RESULT: The results showed that the students in the OSCE method group did significantly better in clinical nursing skills performance than the students in the traditional method group. And the OSCE method group showed significantly higher satisfaction score in the evaluation method than the control group. CONCLUSION: The OSCE method is an effective tool for evaluating clinical nursing skills performance in student nurses. It is necessary to explore more efficient ways to develop OSCE cases in wider areas of nursing education. It is also recommended to replicate similar studies in nursing education.
5.The Effectiveness of Standardized Patient Managed Instruction in Teaching Foley Catheterization Skills, Communication Skills and Learning Motivation.

The main purpose of this study was to develop and evaluate the effectiveness of the standardized patient method in teaching catherization skills, communication skills and students' learning motivation in undergraduate nursing students. The study employed a quasi-experimental method using a non-equivalent control group post-test design. Data were collected over 2 years from August 2000 to September 2001 with 75 sophomore student nurses in one baccaleureate program. The results are summarized as follows; The scores in catheterization skills and communication skills were statistically significantly higher in the experimental group than the control group. Learning motivation score were also statistically significantly higher in the experimental group than the control group. In conclusion, this study suggests that a standardized patients managed instruction which emphasize authenticity and contex- tualization is very effective in teaching nursing students, the mastery and application of nursing skills, and the utilizing therapeutic communication skills. It is recommended that more standardized patients cases be developed for wider areas of nursing education and this study be replicated with more students using a longitudinal design.

7.The evaluation of radiological and clinical findings of bronchiectasis

Bronchiectasis means a permanent abnormal dilatation of one or more large bronchi owing to destruction of theelastic and muscular components of the bronchial wall. Radiological study is the most importat and mandatory procedure. Especially bronchography is essential for the definitive diagnosis of bronchiectasis and for theprecise delineation of the type and extent of the disease. The radiological and clinical findings of 48 cases ofbronchiectasis diagnosed by bronchography and treated at Chosun University Hospital during the 5 years fromJanuary 1980 to Dec. 1984 were analyzed retrospectively. The results were as follows; 1. Among the 48 cases, 34cases(70.8%) were male and 14 cases(29.2%) were female. Peak incidence was in second decade. 2. Chronic cough,productive sputum and hemoptysis are main symptoms and others are chest pain, dyspnea and recurrent bouts ofpneumonia. The most common physical sign is persistent moist rales over the involved area in 23 cases(47.9%).Others are no sign in 17 cases(35.4%), wheezing in 11 cases(22.9%) and digit clubbing in 3 cases(6.3%). 3. Thepresumed causes were composed of not known in 30 cases(62.5%) and complications of measles in 7 cases(14.6%),pertusis in 5 cases(10.4%) and pneumonia in 4 cases(8.3%). Two cases were Kartagener's syndrom and unilateralhyperlucent lung. 4. Plain chest roentgenographic findings were abnormal in 42 cases(87.5%) but normal in 6cases(12.5%). The most common radiological findings was accentuation of lung markings in 36 cases(85.7%) theothers are include in order of frequency; pneumonic infiltration, linear radiolucencies, cystic radiolucencies,decreased affected lung volume air-fluid level and pleural thickening. 5. Bilateral bronchiectasis was demonstrated in 11 cases(22.9%) and the disease was much more often involved left lung than right. The mostcommonly involved lobe is left lower lobe, and the most common site of involvement was the posterior basal segmentof the lower lobe. The type of bronchiectasis is cylindrical in 22 cases(45.8%), varicose in 11 cases(22.9%),saccular in 9 cases(18.8%) and mixed type in 6 cases(12.5%)

10.Effects of Music Therapy and Rhythmic Exercise on Quality of Life, Blood Pressure and Upper Extremity Muscle Strength in Institution-Dwelling Elderly Women.

PURPOSE: The purpose of this study was to determine the effects of music therapy and rhythmic exercise on health related quality of life, blood pressure and upper extremity muscle strength in the institution-dwelling elderly women. METHODS: The study was designed using a nonequivalent control group pretest-posttest design. The participants consisted of 35 elders (18 in the experimental group and 17 in the control group). The music therapy and rhythmic exercise were developed by the investigators. The experimental group took part in this program twice a week for 8 weeks. The Short Form 36 health survey questionnaire, blood pressure and grasp power scale were used as instruments. The data were analyzed using SPSS 14.0. RESULTS: Repeated measures ANOVA revealed that music therapy and rhythmic exercise had positive effects on quality of life, especially on vitality, general health and mental health. Also, there were statistically significant differences in diastolic blood pressure and upper extremity muscle strength between the pretest and posttest in the experimental group. CONCLUSION: The study suggests that this program can be applied for older women in long-term facilities to improve quality of life, blood pressure and upper extremity muscle strength.
