2.Current Status and the Future Prospect of Rehabilitation Nursing in Korea.

The history of rehabilitation of disabilities in Korea began with the foreigners and missionaries who were interested in it after Korean War. In 1981. Disabled Persons Welfare Act was enacted and the 88 Paralympics brought the nations attention to the welfare and rehabilitation of persons with disabilities. Since then, the facilities and the services for the disabled persons have expanded rapidly and the rehabilitation treatment and nursing intervention are drawing more attention. Against this background, the survey on the current status of disabilities, welfare service, facilities, and rehabilitation nursing was conducted. The results of this survey are as follows 1. According to the 2000 census of disabilities. the number of persons with disabilities in Korea is estimated at 1.449.500. or 3.09% of the entire Korean population, 0.74% up from 2.35% in 1995. 2. Disability Types in 2000 The 2000 census showed that the persons with disabilities numbered 1.449, 496 out of the total population and 1.024,371 persons are registered for disability. making up 70.7% of the estimated disabled population. Among them, physically disabled persons accounted for the largest 41. 7% (605.127) and mentally retarded persons stood at the smallest 9% (13.481). 3. Percentage of Disability Presence The survey showed that more than 90% of disability were acquired. However, 44.8% of mental disability and 61.4% of hearing /speaking disability were not acquired after birth. This means that these disabilities happened by congenital cause or birth accident. 4. Yearly Figure of Registered Disabled Persons In 1989. 218,601 persons registered for disability and, in 2000, the number increased by 4.7 times to 1.024.371. These figures are different from the actual number of disabled persons. According to the 1995 census. 1.053.486 were disabled persons but only 378,323 registered for disability. And. in the 2000 census. 1.024,371 out of the 1.449.496 of disabled persons registered for disability. 5. Welfare Service for Persons with Disability 62.6% of the total disabled people are registered and physically disabled persons accounted for the highest percentage of 96.7%. 26.5% of non-registered disabled people said that they didn't know the registration procedure. The rest of them replied that they didn't think they were disabled or that registration didn't seem to give any benefits 6. Welfare Policies for Disabled Persons The welfare benefits given to the disabled are as follows: Issuance of disabled sign for car drivers. Permission to use LPG fuel. Reduction of public facility fees. Household allowance. Tax reduction or exemption, Medical allowance and education subsidy for children, and Housing. 7. Current Condition of Welfare Facilities by Disability Type. The welfare institutions for disabilities numbered 188i in total and they can accommodate 16,823 persons. Categories of these institutions are physical disability(37), visual disability(10). hearing/speaking disability(l4). mental retardation(59), and sanatoriums(68). 6. Human Resource of Rehabilitation of Disabilities Advanced education programs include rehabilitation nursing in its curriculum and this was selected as the program of Korean Academic Society of Nursing in 1990. In November 1997, Korean Academic Society of Rehabilitation Nursing was launched and many academic meeting and seminars were held. This organization is also making efforts to develop the education program for qualified rehabilitation nursing professionals and to develop the standards of rehabilitation nursing practice. In the professionals of the rehabilitation there are rehabilitation specialist, physical therapist, speech therapist, occupational therapist. It is needed to come up with the measures to supply stable human resources following the demand of disabled persons and to recognize the private certificates for rehabilitation professionals as official ones after reviewing the education and training programs of private institutions. 9. Rehabilitation Nursing 1) Rehabilitation nursing was taught as an independent subject in 11 undergraduate programs and 9 graduate programs. 2) Research on rehabilitation nursing in Korea were 24 experimental research and 11 non-experimental research. The intervention of experimental research were mostly education and exercise rehabilitation programs. 3) In the three rehabilitation hospitals, nursmg IS divided into two categories, direct nursing and education & counseling. Direct nursing includes tracheostomy or nasogastric tube care, urination and defication, skin care, pain control, complication, prevention and care, prevention of injury from a fall. etc.

5.The Conceptual Analysis of Spirituality.

The purpose of this is to understand the aspect of human spirituality and its general meaning to nursing practice. This conceptual analysis has been defined thus : 1) spirituality is a basic human attribute 2) spirituality gives meaning to life 3) spirituality is a relationship between the omnipotent and the supernatural. Spirituality assumes these : 1) all human beings possess a spiritual nature 2) it is the source of human power 3) it assumes mutual relationship between the individual and others 4) conviction etc. And the result of spirituality leads to : 1) petition 2) peacefulness 3) hope 4) love between the self and the surrounding others and etc. The experience of the spirituality refers to the relationship between the self, others, and with the environment as well as the relationship with God. And that positive and assertive mentality and attitude will result in a desirable spiritual relationship between the caregiver(the nurse and the receiver.

6.VEGF Expression and Angiogenesis in Uterine Cervical Carcinomas.

Angiogenesis is a critical factor in the progression of solid tumors, including cervical cancers. The mechanisms responsible for angiogenesis in uterine cervical neoplasia are not well defined. To determine the relationship between angiogenesis and the expression of vascular endothelial growth factor (VEGF) in the cervical neoplasia, the author studied 63 cases of the cervical neoplasia diagnosed between the years 1993 to 1997 at Pusan National University Hospital. The expression of VEGF was semiquantitatively analyzed in paraffin sections by immunohistochemical method. Histologic sections immunostained for factor VIII-related antigen were evaluated for microvessel density. Increased expression of VEGF and microvessel counts was significantly correlated with depth of invasion. Increased microvessel counts were also significantly associated with increased VEGF expression. These results suggest that VEGF is an important angiogenic factor and associated with progression of the cervical neoplasia.

7.Effectiveness of Video-Record Method on Fundamental Nursing Skill Education: Focused on Enema.

Effectiveness of the video-record learning method in teaching bowel elimination nursing skill was investigated using an experimental research methodology. Data was collected from 63 female students attending Fundamental Nursing class from a nursing college in Seoul. The subjects were randomly assigned to two groups, one is the experimental group of 29 and the other the control group of 34. The independent variable was video-record learning method and the dependent variable were the degree of knowledge achievement, nursing skill achievement, competence on practicing elimination skill, and satisfaction about the learning method. The hypotheses of the study were as following. 1) There will be significant difference between the experimental group and the control group in dependent variables. 2) There will be significant positive correlations between nursing skill achievement and other three dependent variables-interest in nursing, adaptation in nursing, and preference of nursing job. Data was analyzed using descriptive statistics, chi-square test, t-test, and Pearson's correlation coefficient with SPSS PC+ program. Findings of the study are: 1) There was no significant difference between the experimental group and the control group in knowledge achievement using P<.05. 2) There was significant difference between the experimental group and the control group in nursing skill achievement using P<.05. 3) There was no significant difference between the experimental group and the control group in competence on practicing elimination skill using P<.05. 4) There was no significant difference between the experimental group and the control group in satisfaction about learning method using P<.05. 5) There was positive correlation between nursing skill achievement and the other variables but no significant difference was shown. 6) This study suggests that video-record learning method is an effective learning method for achieving basic nursing skills but is not effective in other areas such as knowledge achievement, competence in performing nursing practice, and satisfaction about the learning method. Further study with more developed research design and statistical analysis should be done to investigate the effectiveness of video-record learning method in learning basic nursing skill more accurately.

8.Relationships between Health, Depression, Memory Self-Efficacy and Metamemory in Adults.

Defining prediction variables related to meta memory for the adults in aging process has worthwhile meaning from the perspective that the produced results can be helpful to reducing the difficulty of memorizing efforts and it can also enhance quality of life of aged. This study attempted to analysis relationship between perceived health status, depression, memory self-efficacy and metamemory for the subjects of middle age and old age adults. This study was designed by adopting descriptive correlational analysis method for the 468 middle and old age adults who are living in Seoul. Samples were selected by convenience sampling. Data collection was done over I month period in june 1998. The instuments used in this study were health status measuring scale including depression measuring scale, memory self efficacy measuring scale and metamemory measuring scale which were verified for reliability. Data collected were analized by using SPSS for frequency, Peason correlation, t-test and ANOVA according to the variables character and the study purposes. Results of the study were as follows. 1. Relationship between perceived health status, depression, memory self-efficacy and metamemory. Relational analyses between perceived health status, depression, memory self-efficacy and metamemory supported the hypotheses of 1st, 2nd and 3rd(p .001) These results suggested that the aged perceived great health status then their memory self-efficacy, and metamemory showed the high scores. In the case of depression when its level became decreased metamemory was inclined to increased. Thus, it is identified that strong relationship exists between these variables. 2. Perceived health status, depression, memory self-efficacy by subject's general characteristics. Scores of perceived health status were high in the group of man compared to the group of women, and also highly educated group showed great perceived health status. Group of persons having occupation showed high score of perceived health status and low depression score. The score of memory self-efficacy and metamemory showed higher in the middle aged than the old aged. The high scores of memory self-efficacy and metamemory were found in the group of highly educated people and who have continuing education. The high scores of memory self-efficacy were found in the group of persons having their job and high metamemory scores found in the group of persons having religion. In summary, the greater perceived health status and memory self-efficacy, the more metamemory scores were likely increased and the more depression level was decreased, the more metamemory was likely increased. Also it was found that general characteristics like educational level, continuing education and religion influenced the metamemory of the aged. Therfore, prevention the aged from getting depression and activation of health promotion are needed to delay time of memory loss.

9.The Effects of Sensory Stimulation for Ingestion in Dysphagic Patients with Stroke.

The purpose of this study is to identify the effects of a nursing intervention using sensory stimulation in dysphagic patients. Quasi-experimental with a nonequivalent control group pretest-posttest desigr, was used. 15 patients of each group were assigned for this study, who were hospitalized in the oriental medicine hospital of K. University. Specific stimulation plans were devised based on a dysfunctional area of the subjects. For each modality. subjects were systematically stimulated for 30 minutes. in the experimental group prim to each meal and 3 times per day for a week. The data were analyzed by SPSS PC program using chi2test. Wilcoxon Signed Rank test and Mann-Whitney U test. The results of this study are as follows: 1. The tongue contra! score of the experimental group is greater than that of the control group(u=52.0. p= .007). 2. The score of chewing ability. lip control swallowing ability of the experimental group is greater than that of the control group But no difference is statistically seen between the experimental group and control group. Therefore, this study shows that sensory stimulation using icing. quick stretch. etc., was effective in improving oropharyngeal function in patients with dysphagia.

10.A Comparison of the Sexual Concerns of Men and Women with Spinal Cord Injuries.

The purpose of this study is to compare sexual concerns and adjustment between men and women with spinal cord injuries. A total of 108 persons (67 men and 41 women), registered to the Spinal Cord Injury Organization or admitted at rehabilitation unit from February 1999 to February 2000, were selected as subjects of the study, Participants answered the questionnaires concerning importance of life events, sexual concern, sexual adjustment, relationship with a sex partner, and depression. The results are as follows 1) There were no significant differences between men and women in the general characteristics except sexual dysfunction. 2) With respect to eleven different life areas, sex life ranked the fifth among men, whereas it occupied the lowest position for women among the 11 areas. However economic status was the most highly-ranked for both sex. 3) Men took much interest in the methods and techniques for achieving sexual satisfaction, He]ping a partner cope emotionally with limitations on sexual activity, whereas women were interested in Helping a partner cope emotionally with limitations on sexual activityand ''Ability to have children 4) The means of sexua] adjustment, relationship with a sex partner, and depression among men were higher than women. But there was no significant difference between two groups. As a result, the study suggests that there are no significant differences in the general characteristics between men and women with spinal cord injuries except sexual dysfunction. Men evaluated the sexual events highly compared to women, and differences were also shown in terms of sexual concerns between men and women. Therefore differential approaches to sexual rehabilitation are required in both socio-psychological and physical aspects for men and particularly in socio-psychological respect for women.
