1.The evaluation of radiological and clinical findings of bronchiectasis

Bronchiectasis means a permanent abnormal dilatation of one or more large bronchi owing to destruction of theelastic and muscular components of the bronchial wall. Radiological study is the most importat and mandatory procedure. Especially bronchography is essential for the definitive diagnosis of bronchiectasis and for theprecise delineation of the type and extent of the disease. The radiological and clinical findings of 48 cases ofbronchiectasis diagnosed by bronchography and treated at Chosun University Hospital during the 5 years fromJanuary 1980 to Dec. 1984 were analyzed retrospectively. The results were as follows; 1. Among the 48 cases, 34cases(70.8%) were male and 14 cases(29.2%) were female. Peak incidence was in second decade. 2. Chronic cough,productive sputum and hemoptysis are main symptoms and others are chest pain, dyspnea and recurrent bouts ofpneumonia. The most common physical sign is persistent moist rales over the involved area in 23 cases(47.9%).Others are no sign in 17 cases(35.4%), wheezing in 11 cases(22.9%) and digit clubbing in 3 cases(6.3%). 3. Thepresumed causes were composed of not known in 30 cases(62.5%) and complications of measles in 7 cases(14.6%),pertusis in 5 cases(10.4%) and pneumonia in 4 cases(8.3%). Two cases were Kartagener's syndrom and unilateralhyperlucent lung. 4. Plain chest roentgenographic findings were abnormal in 42 cases(87.5%) but normal in 6cases(12.5%). The most common radiological findings was accentuation of lung markings in 36 cases(85.7%) theothers are include in order of frequency; pneumonic infiltration, linear radiolucencies, cystic radiolucencies,decreased affected lung volume air-fluid level and pleural thickening. 5. Bilateral bronchiectasis was demonstrated in 11 cases(22.9%) and the disease was much more often involved left lung than right. The mostcommonly involved lobe is left lower lobe, and the most common site of involvement was the posterior basal segmentof the lower lobe. The type of bronchiectasis is cylindrical in 22 cases(45.8%), varicose in 11 cases(22.9%),saccular in 9 cases(18.8%) and mixed type in 6 cases(12.5%)

3.Comparison of the results of multistix®-SG and comber-9-Test®RL urine dipstick assay.

Two types of urine dipstick assays, Multistix-SG and Comber-9-Test RL, were compared for compatibility, accuracy, specificity and predictive values of a positive and negative test in 501 patients' urine and artificially prepared specimen. We found that the results of semiquantitative tests of Multistix-SG and Comber-9-Test RL performed were statistically similar in patients' specimen. The urinary leukocyte esterase tests of Comber-9-Test RL assays compared with urine sediment microscopy in regard to compatibility, sensitivity, specificity, and predictive values of a positive and negative test 83.7%, 48.1%, 90.3%, 47.4% and 90.1%, respectively. The urinary nitrite tests of Comber-9-Test RL assays compared with urine culture tests, in regard to compatibility, sensitivity, specificity, and predictive values of a positive and negative test were 90.3%, 19.4%, 84.7%, 53.8% and 94.1, respectively. For the urinary protein, the sulfosalicylic acid method was the most sensitive test for any kinds of protein, and Multistix-SG appeared more sensitive than Comber-9-Test RL for the albuminuria. For the urinary bilirubin and glucose, two dipstick assays were similar in their diagnostic efficiency. Finally in the urinary occult blood tests, Comber-9-Test RL assays was more sensitive than Multistix-SG.

4.The Change of Keratometric VAlue Following Penetrating Keratoplasty.

We investigated mean keratometric vale(K) after penetrating keratoplasty(PKP) and analyzed the factors which affected the postoperative mean K. We obtained mean K at postoperative 3 months in the 65 eyes of 65 patients which PKP of PKP combined with intraocular lens(IOL) implantation was done, and analyzed those values according to sex, age, preoperative diagnosis, preoperative corneal neovascularization, methods of trephination, methods of suture and methods of operation. The postoperative mean K of all patients was 42.83 diopters(D) and 43.32D in the recipient cornea without neovascularizaion, 42.51D with neovascularization, 42.72D in the cases of hand-held trephination and 43.27D in Hessvurg-Varron trephination, which had no statistical significant difference between K and sex, age, preoperative diagnosis, methods of suture, and methods of operation. This results will be helpful to predict the change of K ofter PKP, and give a better refractive results to use the operator's own mean K for the IOL calculation in the case of PKP comvined with IOL implantation.

5.Mannitolization during Operation of Patients with Stone .

It is well known that general anesthesia and surgery cause acute depression of renal function. Also, the occurrence of frank bleeding from the urinary tract can form a blood clot which may block an indwelling catheter or even a normal urethra with development of urinary infection. Many studies have reported that prophylactic use of hypertonic mannitol will prevent acute functional renal impairement during general anesthesia and surgery, and reduce the incidence of clot formation and acute pyelonephritis. We believe that diuresis itself is a reneprotective mechanism and polyuria may yield two other advantages; 1. Fewer plugged catheters by decreasing the incidence of clots. 2. Decreased incidence of acute pyelonephritis by preventing stasis and washing out bacteria. since 1972, in our hospital, 100cc or 200cc of 20% mannitol was used for increasing the urine output during operation of urinary stone in 33 patients, We have shown that the prognosis of patients was good.

7.Effects of Noxious Peripheral Nerve Stimulation on the Activity of Respiratory Neurons in the Ventrolateral Medulla of Cats.

Noxious peripheral nerve stimulation causes changes of arterial blood pressure by way of somatosympathetic reflex, and may play an important role in determining the periodicity of respiratory neurons at the same time. However, little has been known about how noxious stimulation does mediate respiratory control. In the present study, the respiratory neurons of the ventrolateral medulla were identified and characterized, and the responses of these neurons to noxious stimulation were observed, to investigate the role of noxious stimulation in determining respiratory rhythm. Cats were anesthetized with alpha-chloralose and mechanically ventilated after muscle relaxation. Occipital craniectomy was performed and the floor of the fourth ventricle was exposed. Single cell activities of the ventrolateral medulla were recorded using carbon-filament microelectrodes. Neurons related with respiratory rhythm were identified and the responses to vagal and sciatic nerve stimulation were observed. The following results were obtained. 1) A total of 46 neurons related with mechanical ventilatory cycle was identified, including 13 expiratory augmenting neurons, 11 inspiratory decrementing neurons, and 9 unclassified neurons. 2) The activity of inspiratory augmenting neurons was increased by A-delta intensity stimulation of the vagal and sciatic nerves, and inhibited by C-intensity stimulation of the sciatic nerve. 3) The inspiratory decrementing neurons did not show a significant response to vagal nerve stimulation, but the cell activity was inhibited by A-delta and C-intensity stimulation of the sciatic nerve. 4) The expiratory neuron did not show a significant response to peripheral nerve stimulation. 5) 27 neurons had slower rhythmic activity than mechanical ventilation. Sciatic nerve stimulation increased frequency of rhythmic discharges in these cells. 6) Most of the neurons with slower rhythmic activity during spontaneous respiration followed faster rhythm of mechanical ventilation after muscle relaxation. From these results, it is concluded that the activities of the respiratory neurons, especially inspiratory ones, in ventrolateral medulla are under influence of noxious peripheral stimulation, and they are dependent on the state of the experimental animal.

10.Effect of restoration type on the stress distribution of endodontically treated maxillary premolars; Three-dimensional finite element study.

The purpose of this study was to investigate the effects of four restorative materials under various occlusal loading conditions on the stress distribution at the CEJ of buccal, palatal surface and central groove of occlusal surface of endodontically treated maxillary second premolar, using a 3D finte element analysis. A 3D finite element model of human maxillary second premolar was endodontically treated. After endodontic treatment, access cavity was filled with Amalgam, resin, ceramic or gold of different mechanical properties. A static 500N forces were applied at the buccal (Load-1) and palatal cusp (Load-2) and a static 170N forces were applied at the mesial marginal ridge and palatal cusp simultaneously as centric occlusion (Load-3). Under 3-type Loading condition, the value of tensile stress was analyzed after 4-type restoration at the CEJ of buccal and palatal surface and central groove of occlusal surface. Excessive high tensile stresses were observed along the palatal CEJ in Load-1 case and buccal CEJ in Load-2 in all of the restorations. There was no difference in magnitude of stress in relation to the type of restorations. Heavy tensile stress concentrations were observed around the loading point and along the central groove of occlusal surface in all of the restorations. There was slight difference in magnitude of stress between different types of restorations. High tensile stress concentrations around the loading points were observed and there was no difference in magnitude of stress between different types of restorations in Load-3.
