1.A Case of Bilateral Nevus of Ota Associated with Nevus of Ito.

The nevua of Ota is a congenital pigmentary disorder of the skin and mucaus membrane, partieularly in the periorbital region, the temple, the forehead, the malar area and the nose. The nevus of Ita differs from the nevus of Ota in its distribution in the supraclavicular, scapular, and deltoid regions. It may occur alone or in association with ipsilateral or rarely with bilateral nevus of Ota. We present a 22 year-old female who has had pigmentary anomalies on the both malar areas and left sclera associated with pigmentation of the right shoulder.

2.Photochemotherapy for Psoriasis.

The beneficial effects of sunlight and ultraviolet light for the treatment, of psoriasis vulgaris has long been recognized. Parrish et al were the first to introduce a controlled light source, which allowed the delivery of a known quantity of light energy to the patients skin after the patient had ingested a photosensitizer, 8-methoxypsoralen (methoxsalen) two hours previous to UVA exposure. This successful treatment has become known as PUVA (psoralen and UVA). PUVA photochernotherapy denotes a systemic approach that is based on the interaction of light and a photoactive drug. Parrish et al have described successful treatment of psoriasis with PUVA and other investigators alI over the world have confirmed their beneficial results. The rationale of photochemotherapy in psoriasis is the inhibition of the increased DNA synthesis within the psoriatic lesions by the interaction of psoralen molecule and light energy in the UVA range(320-400nm). The present study was undertaken to evaluate the therapeutic effectiveness of PUVA photochernotherapy combining orally administered methoxsalen and UAA exposure using Dermatron UVA phototherapy system in patients with wi5espread psoriasis. A total of 15 patients with psoriasis over more than 30% of their bodies were selected at the department of dermatology, National Medical Center through March 1978 to September 1978. Oral administration of methoxsalen, 20 to 50 mg doses according to body weight, was followed by exposure to a high-intensity longwave ultraviolet light emitting a continuous spectrum between 320 and 400nm (peak emission, 350 - 365nm). Initial UVA exposure times were based on each patients minimal phototoxicity dose (MPD) and exposure times were increased at each treatment by 1 - 2 minutes to maintain minimal erythema. Frequency of treatment was two to three times weekly. Once the psoriatic lesions had, cleared completely the patient was placed on a maintenance schedule. If psoriasis recurred the frequency of treatment was increased. The results were as follows: l. 5 patients were dropped out due to unknown reasons. 2. 7 cases showed clearing of over 95, of the lesions (Response 5'). Mean number of PU VA therapy was 16. 7. During 1 4 months follow-up period, 4 cases remained free of psoriasis but 3 cases couldn't be followed. 3. Immediate side effects of PUVA were temporary and generally mild. No significant changes in laboratory screenings were noted. Side effects of therapy included transient nausea(33.3%), dizziness(26.7%), erythema.(66.7%), pruritus (60%), increased pigrnentation(100 %). 4. Comparison of pre-and post-PUVA therapy in biopsy specimens by light microscopy revealed histologic improvement evidenced by diminished acanthosis and parakeratosis, and regeneration of granular layers in all of 8 cases. Glycogen accumulation in the affected epiderrnis was significantly decreased or absent after PUVA therapy in all cases.

3.Clinical and Histopathological Observation of Porokeratosis.

We observed clinical and histopathological findings of porokeratosis in 16 cases which comprised 2 linear types, 4 superficial disseminat:ed forms, 4 disseminated superficial actinic porokeratosis and 6 plaque types. From the observation, the following results were obtained. l. Clinical results 1) Eleven cases were men, 5 cases were women, and the sex ratio was 2. 2 to 1, age distribution was from 6 to 61 years (mean age: 34. 2years), and the distribution of age of onsets was from 2 to 52 years(mean age of onset' 23. lyears). 2) Four cases had family history of porokeratosis, but linear forms did not. 3) Nine cases had pruritus especially, all cases of DSAP and DSP except 1 case complained pruritus. 4) Tmo cases of DSAP had tendency of summer exacerbations. 2. Histopathological results 1) Cornoid lamellae were prominent in all cases of plaque types and linear types, and minimal in all cases of DSAP and DSP except 1 case. 2) Epidermis between the cornoid lamellae were normal or acanthotic in all cases except 1 case of plaque types and linear types, and were atrophic in 5 cases of 8 cases in DSAP and DSP.

4.Prevalence of Syphilis in Normal Population in Korea ( 1987 - 1991 ).

BACKGROUND: According to the areas and people that are examined, there are many differences in the prevalence of syphilis in Korea. However, the reactive rate of the serologi test for syphilis showed an increase in the 1960s, and it has deerea@sed from the beginning of the 1970s. OBJECTIVE: Comparing the results of tke serologic test for syphilis which lotad been performed for the 5 years(1987-1991) on the physical examines and pregnant women who were considered as healthy, with the results with normal people, who were considered as healthy, which had been already reported, we tried to observe the change of reactive rate of serologic test for syphili. METHODS: The serologic test for syphilis, including serum VDRL test and serum TPHA test, were carried out on 25,254 healthy adults(20,405 were physical examineea eamined at National Medical Center and 4,849 were pregnant women delivered at National Medical Cer ter). RESULTS: The reaetive rate of serum VDRL test was 0.8% in 25,25 the althy adults(physical examinees 0.8%, pregnant women 0.7%). There were no stistically differerces between the annual incidences. 85.5% of VDRL-reactive subjects showed low VDRL titers(below 1:4) by the quantitative serum VDRL test, The biologic false positive reaction of serum VDRL test was 29(19.5%) in 149 VDRL-reactive subjects, using serum TPHA test as a standard. CONCLUSION: This study suggested thai, there was a decrease in the prevslence of syphilis in Korea.

5.Therapeutic Effect of Levamisole in Recurrent Aphthous Stomatitis.

There have been no standard treatments for recurrent aphthous stomatitis, and clinical management is usually directed toward symptomatic relief. Recent immunological investigations have focused on possible imrnunopathogenesis of the disease. Several reparters suggested that levamisole, nonspecific immune-stimulator, had a beneficiaI effect in controlling attacks of recurrent a,phthous stomatitis and in reducing subsequent episodes. The present study was undertaken to evaluate the therapeutic effectiveness of levamisole in patients with recurrent aphthous stomatitis. A total of 8 patients who had had recurrent aphthous stomatitis for 2 to 20 years and who had experienced at least one episode per month were selected for this study from the department of dermatology, National Medical Center, through March 1979 to September 1979, Levamisole (Decaris') was given 150mg, p.o., once daily on 3 consecutive days every week for 2 months. Tbe results were as follows. 1) One patient had to have levamisole discontinued due to a high fever and exacerbations of tbe oral ulcerations. 2) 5 patients showed beneficial effects in reducing tbe number, frequency, pain and severity of recurrent aphthous stomatitis. 3') One patient showed no therapeutic response. 4) One patient, who has experienced new oral ulcerations continuously before starting levamisole, showed no recurrences of the lesions during the follow-up period of 3 months. 5) Side effects during levamisole administration were transient and generally mild, They included nausea, headache, dizziness and high fever.

6.Clinical Observations on Behcet's Syndrome.

The clinical triad of relapsing iritis, ulcers of the mouth and genitalia was first described in 1937 by Halusi Behcet. This entity, originally confined to the above triad of symptoms, appears to be a systemic disease manifested by skin lesions, thrombophlebitis, neurologic or visceral syptoms. The exact etiology of this syndrome has not been established. However, in recent years, many investigators suggested that the autoimmune mechanism could be the cause of this disease, and imrnunotherapeutic agents have been shown to have beneficial effects in controlling recurrent episodes of major manifestations. 7.A Case of Epidermolysis Bullosa Simplex.

Epidermolysis bullosa aimplex is a dominantly inherited syndrome characterized by the dissolution of the basal epidermal cells with mechanical stress. A 23 year-old man developed recurrent bullous eruptions with pain on hands, feet and face since childhood, which healed spontaneously without scarring. Grandmother of his father's side, father, aunt of his father's side and two of his three brothers had suffered similar skin lesions. Light and electron microscopy showed degeneration and bulla formation in the basal cell layer of epidermis and intact basement membrane attached to dermis.

8.Histopathological Observation of Cafe au Lait Spots.

We observed 17 cases of cafe au lait spots histopathplogi ally. Out of 17 cases, 9 cases were neurofibromatosis with neurofibromas, 6 cases were neurofibromatosis without neurofibroma, and 2 cases were normal healthy persons. The results were as follows. l. In all 17 cases, melanin pigment was increased in melanocytes and keratinocytes. 2. Griant pigment granules were identified only in 3 cases of neurofibromatosis with neurofibromas.

9.Nausea/Vomiting and Anxiety of Hospitalized Cancer Patients Receiving Chemotherapy.

PURPOSE: The purpose of this study was to describe the pattern of changes on the score of nausea/vomiting and anxiety during one cycle of chemotherapy. METHOD: A total of 53 subjects who were admitted to C University Hospital for a period of 3 days and 2 nights for chemotherapy were selected from February to April, 2003. Total scores of nausea/vomiting were measured twice a day 3 days for a total of 6 measurements. Anxiety, anorexia, and fatigue were also measured at the first and last measurement points. Data were analyzed by one-way repeated measures, ANOVA, t-test, paired t-test, & Pearson's correlation. RESULT: The score of nausea/vomiting increased over time except for the 4th measurement point but no changes were significant over time. There were the significant differences between 1st and 2nd, and 2nd and 3rd nausea/vomiting score at p < 0.05. The scores of anxiety, anorexia, and fatigue between the first and 6th points were significantly different(t=-5.69, p=.001; t=6.25, p=.0001; t=3.65, p=.0007). CONCLUSION: Further studies are needed to identify the relationship between anxiety, and anticipatory and acute nausea/vomiting respectively.
